How Is Lymphedema Treated

At this time, there is no cure for lymphedema. The goal of treatment is to reduce the swelling or keep it from getting worse, and to relieve symptoms. Your doctor can help you figure out which treatments are best for you.

Lymphedema can be treated with

Exercise. Moving your swollen arm or leg may help the lymph fluid to drain, thus reducing the swelling.

A compression sleeve or stocking. This garment fits tightly on your swollen arm or leg, helping the lymph fluid flow out.

Massage therapy. A special kind of massage called manual lymph drainage can help push the lymph fluid out of the swollen part of your body.

A pneumatic pump. A machine inflates a sleeve that you place on your swollen arm or leg, helping lymph fluid flow out.

Weight loss. In patients who are overweight, lymphedema related to breast cancer may improve with weight loss.

Cancer treatment. If a cancerous tumor is causing the lymphedema, the cancer is treated.

The keys to managing lymphedema are preventing infections and allowing fluids to flow through the swollen area.

Prevent Infections

• Keep your skin clean, and use lotion to keep it moist.
• If you get a small cut, clean it right away and apply antibacterial ointment and a bandage.
• Dont get needle sticks in the swollen area.

Risk Factors For Lymphedema

Risk factors depend on the type of surgery as well as the persons individual risk factors. For example, about 16% of melanoma patients develop lymphedema, and roughly 10% of patients with cancers of the urinary system and/or genitals will experience lymphedema.

Someones individual risk factors also make them more likely, or less likely to develop lymphedema. These can include:

• How extensive the surgery was
• Whether the patient had radiation treatment and to what extent the lymph nodes were targeted
• Whether the wound took more time than usual to heal
• Whether the tumor was obstructing the lymph system prior to surgery
• If there was scarring from the surgery scar tissue can act as a barrier to the lymph system
• Pre-existing condition like diabetes, thyroid disease or obesity
• Stress
• People with venous disease in their legs prior to inguinal lymph node removal

Symptoms Of Advanced Prostate Cancer

The symptoms of advanced prostate cancer depend on where the cancer has spread to. Find out about the possible symptoms and when to see your doctor.;

Advanced prostate cancer means that a cancer that began in the prostate gland has spread to another part of the body. If your cancer has spread you might:

• have bone pain
• have weight loss for no known reason

Clinical Features And Imaging

Taken as a group, primary and secondary prostatic leukemia/lymphoma most commonly presents in the seventh decade. Primary prostatic lymphomas, which tend to be high grade on the basis of a large series, usually present with symptoms of a large prostatic mass, including obstructive symptoms and hematuria. Digital rectal examination findings usually are abnormal, showing a diffusely enlarged or nodular prostate in the majority of cases. Serum prostate-specific antigen usually is not elevated .

Secondary prostatic leukemia/lymphoma is often discovered incidental to a diagnosis of prostatic adenocarcinoma. Symptoms at presentation may thus be intermixed with those of prostate cancer and include elevated serum PSA . Secondary prostatic leukemia/lymphoma may also present with obstructive symptoms.

The differential diagnosis for prostatic tumors that have the appearance of diffuse large B-cell lymphomas is wide, including the following :

• High-grade neoplasms
• Small cell carcinoma

The differential diagnosis for prostate leukemia/lymphoma also includes the following non-neoplastic processes:

• Benign chronic inflammation with follcular hyperplasia
• Granulomatous prostatitis

Bone Marrow Trephine Examination

A good core of hypercellular bone marrow showed a predominantly paratrabecular infiltrate of small lymphocytes together with plasmacytoid and plasma cells. The lymphocytes were CD79+, CD20+, Bcl2+, but negative for CD5, CD23 and CD10. The admixed plasma cells were CD138+ with kappa light chain restriction but negative for CD56. There was little trilineage hemopoietic reserve. The features were those of lymphoplasmacytic lymphoma extensively involving the bone marrow.

Prostate Cancer Can Cause Pain In Many Areas And Not Just Because Of Bone Spread But Also Due To Other Features Of The Disease

It can feel different ways depending on the nature of the cause.

Most pain associated with prostate cancer is from bone metastases in stage IV disease.

Even shoulder and arm pain are possible when a metastatic tumor is pressing against the spine.

Bone metastases occur in approximately 80% of patients with advanced prostate cancer, says the Journal of Clinical Oncology.

When prostate cancer spreads to other parts of the body, it almost always goes first to the bones.

Bone pain from prostate cancer can feel like a dull ache or feel like a toothache in the bones, or you may feel a sharper sensation.

Second Cancers After Prostate Cancer

Prostate cancer survivors can be affected by a number of health problems, but often a major concern is facing cancer again. Cancer that comes back after treatment is called a recurrence. But some cancer survivors may develop a new, unrelated cancer later. This is called a second cancer.

Unfortunately, being treated for prostate cancer doesnt mean you cant get another cancer. Men who have had prostate cancer can still get the same types of cancers that other men get. In fact, they might be at higher risk for certain types of cancer.

Men who have had prostate cancer can get any type of second cancer, but they have an increased risk of certain cancers, including:

• Acute myeloid leukemia

This risk is probably related to the dose of radiation. Newer methods of giving radiation therapy may have different effects on the risks of a second cancer. Because these methods are newer, the long-term effects have not been studied as well.

How Will My Cancer Be Monitored

Your doctor will talk to you about how often you should have check-ups. At some hospitals, you may not have many appointments at the hospital itself. Instead, you may talk to your doctor or nurse over the telephone. You might hear this called self-management.

You will have regular;PSA tests. This is often a useful way to check how well your treatment is working. Youll also have regular blood tests to see whether your cancer is affecting other parts of your body, such as your liver, kidneys or bones.

You might have more scans to see how your cancer is responding to treatment and whether your cancer is spreading.

Your doctor or nurse will also ask you how youre feeling and if you have any symptoms, such as pain or tiredness. This will help them understand how youre responding to treatment and how to manage any symptoms. Let them know if you have any side effects from your treatment. There are usually ways to manage these.

While it isnt possible to cure advanced prostate cancer, treatments can help keep it under control, often for several years. Treatments will also help manage any symptoms, such as pain.

No one can tell you exactly what your outlook will be, as it will depend on many things such as where the cancer has spread to, how quickly it has spread, and how well you respond to treatment. Some men may not respond well to one treatment, but may respond better to another. And when your first treatment stops working, there are other treatments available to help keep the cancer under control for longer. Prognosis And Predictive Factors

As in other locations, prognosis in prostatic involvement by leukemia/lymphoma is dependent on the subtype.

The majority of cases are low grade, predominantly chronic lymphocytic leukemia/small lymphocytic lymphoma . As such, the prognosis in these cases is generally favorable. In a large consecutive series with follow-up data, which showed predominantly low-grade leukemia/lymphoma, only one patient had died from disease at follow-up.

In contrast, diffuse large B-cell lymphoma , which is often primary, carries a considerably less favorable prognosis. In a large series of primary prostatic lymphomas to date, which notably included numerous high-grade secondary prostatic lymphomas in survival analysis, 5-year lymphoma-specific survival was only 33%.

Prostate Cancer And The Lymphatic System

The lymphatic system helps protect us from infection and disease. It also drains lymph fluid from the tissues of the body before returning it to the blood. The lymphatic system is made up of fine tubes called lymphatic vessels that connect to groups of lymph nodes throughout the body.

Lymph nodes are small and bean-shaped. They filter bacteria and disease from the lymph fluid. When you have an infection, lymph nodes often swell as they fight the infection.

If;prostate cancer;cells spread to the lymph nodes, they usually go to the nodes close to the prostate. You may have a scan to check if the cancer has spread to these nodes.

In;early prostate cancer, the cells have not spread to the lymph nodes.;Locally advanced prostate cancer;may have spread to these nodes. In;advanced prostate cancer, there are usually cancer cells in the lymph nodes close to the prostate and in lymph nodes further away.

Symptoms From Lymphoma In The Chest

When lymphoma starts in the thymus or lymph nodes in the chest, it may press on the nearby trachea , which can cause coughing, trouble breathing, or a feeling of chest pain or pressure.

The superior vena cava is the large vein that carries blood from the head and arms back to the heart. It passes near the thymus and lymph nodes inside the chest. Lymphomas in this area may push on the SVC, which can cause the blood to back up in the veins. This can lead to swelling in the head, arms, and upper chest. It can also cause trouble breathing and a change in consciousness if it affects the brain. This is called SVC syndrome. It can be life-threatening and must be treated right away.

The Symptoms Of Delirium Are A Lot Like Symptoms Of Depression And Dementia

Early symptoms of delirium are like symptoms of depression and dementia. Delirium that causes the patient to be inactive may appear to be depression. Delirium and dementia both cause problems with memory, thinking, and judgment. Dementia may be caused by a number of medical conditions, including Alzheimer disease. Differences in the symptoms of delirium and dementia include the following:

• Patients with delirium often show changes in how alert or aware they are. Patients who have dementia usually stay alert and aware until the dementia becomes very advanced.
• Delirium occurs suddenly . Dementia appears gradually and gets worse over time.

Older patients with cancer may have both dementia and delirium. This can make it hard for the doctor to diagnose the problem. If treatment for delirium is given and the symptoms continue, then the diagnosis is more likely dementia. Checking the patient’s health and symptoms over time can help diagnose delirium and dementia.

What Treatments Are Available

If you have advanced prostate cancer, treatment wont cure your cancer. But it can help keep it under control and manage any symptoms.

If youve just been diagnosed with advanced prostate cancer, you may be offered the following treatments:

• chemotherapy;with hormone therapy
• clinical trials.

Research has found that having;radiotherapy;together with one of the main treatments listed above can help some men with advanced prostate cancer to live longer. But radiotherapy isnt suitable for all men with advanced prostate cancer.

If you live in;Scotland, you may also be offered a type of hormone therapy called;abiraterone acetate ;together with standard hormone therapy. In the rest of the UK, abiraterone is currently only given to men with advanced prostate cancer that has stopped responding to other types of hormone therapy. The;National Institute for Health and Care Excellence ;is currently deciding whether to make it available for men who have just been diagnosed with advanced prostate cancer.
