Salvage Androgen Deprivation Therapy
Recurrence following RP can potentially be managed with salvage ADT, although data supporting this use is generally obtained from retrospective studies . Not all patients with BCR after primary curative treatment benefit from salvage ADT however, a favourable effect is observed in a high-risk group, which may be defined as having a short PSA-DT and/or by tumour characteristics . Factors that may favour ADT after RP include a very high risk of clinical recurrence, good recovery of continence, long life expectancy, and the patient being anxious about the future or not being ready to accept the idea of sRT.
The National Cancer Institute of Canada PR-7 trial compared intermittent with continuous ADT in men with BCR and no evidence of metastatic disease after definitive or salvage RT and RP. OS in the intermittent arm was not inferior to that in the continuous arm, and intermittent therapy was associated with beneficial effects on certain domains of QoL. Salvage ADT for BCR may therefore be most appropriately delivered in an intermittent fashion, with the possible exception of patients with a Gleason score of 8 or higher .
What Are Antiandrogens And Should They Be Part Of My Treatment Plan
Antiandrogens can be helpful in preventing the flare reaction associated with LHRH agonists resulting from an initial transient rise in testosterone. Their use for at least the first 4 weeks of LHRH therapy can relieve the symptoms often seen from the flare reaction, ranging from bone pain to urinary frequency or difficulty. You should ask your doctor whether continuing these pills for longer-term cancer control might be beneficial for you. There is some information to suggest that combination testosterone blockade and testosterone-lowering therapy may be better than either one alone for long term control, but at the cost of additional side effects like breast tenderness and enlargement, hot flashes, and higher costs.

How Prostate Cancer Recurrence Is Detected
Following surgery for prostate cancer removal, the majority of patients will have regular appointments to check in and assess recovery progress with their healthcare provider or preferred oncologist. At each of these visits, your doctor will draw blood to run tests on various levels of biomarkers, hormones, and immunological markers, including PSA. You may also undergo minor physical assessments as part of your post-prostatectomy preventive maintenance care plan.
If prostate specific antigen levels are found to be rising, staying the same as pre-operative quantities, or slowing their rate of decrease in an unexpected way, your provider may suggest imaging tests or an X-ray of the prostate and pelvic area. A few other alternative options for detecting prostate cancer recurrence are available, although some are in the early stages of development or are not yet widely available, so should not be used as a default or go-to option for determining whether metastatic resurgence is a clear and present health risk.

Can I Have Radiation First And Then Surgery
With 3D conformal radiotherapy, intensity modulated radiation therapy , and brachytherapy, local tissue damage is often kept at a minimum, and surgeons at some of the larger cancer centers have been seeing improved results with salvage prostatectomy performed after radiation. But even under the best of circumstances, post-radiation surgery is a very difficult operation to perform, and few surgeons across the country perform it regularly.

What Does Psa Velocity Mean
PSA velocity or PSA doubling time, both of which measure the rate at which your PSA rises, can be a very significant factor in determining is the aggressiveness of your cancer. Men with a shorter PSA doubling time or a more rapid PSA velocity after initial therapy tend to have more aggressive disease, and are therefore more likely to need more aggressive therapies. Likewise, men who have recurrence soon after surgery have a higher risk of aggressive disease.

Ive Had A Prostatectomy Am I At Risk Of Cancer Returning
Although all forms of cancer are technically a cellular mutation that cannot necessarily be prevented or controlled, there are factors that may increase the risk of prostate cancer recurrence, even after surgery. These include, but are not limited to, the following:
- Stage of Prostate Cancer
- If the prostate cancer was detected in a later stage, or if cancerous tissue or tumor growth is significantly progressed, a prostatectomy may be slightly less effective for the purposes of controlling future susceptibility to recurrence.
Treatment Options: When Cancer Returns After Radiation
After prostate cancer is treated, patients usually undergo a blood test called PSA. PSA stands for Prostate Specific Antigen and is a simple blood test that gives a number. After treatment, that number should be low. If it starts to increase, that can be a signal of cancer coming back, also known as cancer recurrence.
Often doctors will watch a series of PSA values to identify the trend. If the PSA number continues to increase, this is called biochemical failure, and your doctors will investigate the cause. Sometimes doctors will order scans to see if there is cancer recurrence locally , or distantly . Treatment is different depending on where the cancer is found.
If there is high suspicion for local recurrence, your doctors may encourage a prostate biopsy to see if there is tumor identified under the microscope. If so, there are several local treatment options including cryotherapy and prostatectomy. Some patients may choose observation.
Brachytherapy Option
What about using brachytherapy on the prostate? This has not been as popular given the concern about risk of toxicity for patients who have used external beam radiation previously
More follow up is needed and the complete results will be reported with longer follow up, but this study shows that salvage brachytherapy may be another option in the treatment of recurrent prostate cancer after radiation therapy.

Treatment Options For Men With Urinary Symptoms After Radiation For Prostate Cancer
Many men benefit from medical treatment to help them with urinary symptoms during IMRT, CyberKnife, proton beam, or brachytherapy. These medications include alpha-blockers such as Flomax, alpha-reductase inhibitors such as finasteride or Proscar, and anticholinergic medications such as Detrol, or Vesicare.
When medical treatment does not provide sufficient benefit or of the symptoms persist, other treatment options offered at New York Urology Specialists include Urolift procedure, Rezum procedure, iTind for prostate, Greenlight laser of the prostate, Botox injections in the bladder, and TURP.
It should be noted that men with urinary symptoms after radiation therapy for prostate cancer should have a cystoscopy at a minimum to exclude urethral stricture disease and bladder tumors. Some men may also benefit from a urodynamic evaluation.

Psa Levels After Treatment
A continuous rise in your PSA level can be the first sign that your cancer has come back. This should be picked up by your regular PSA tests.
The exact change in PSA level that suggests your cancer has come back will depend on which treatment you had. Speak to your doctor or nurse about your own situation.
Your PSA level should drop so low that its not possible to detect it at six to eight weeks after surgery. This is because the prostate, which produces PSA, has been removed. A rise in your PSA level may suggest that you still have some prostate cancer cells.
After radiotherapy or brachytherapy, your PSA should drop to its lowest level after 18 months to two years. Your PSA level wont fall to zero as your healthy prostate cells will continue to produce some PSA.
Your PSA level may actually rise after radiotherapy treatment, and then fall again. This is called PSA bounce. It could happen up to three years after treatment. It is normal, and doesnt mean that the cancer has come back.
If your PSA level rises by 2 ng/ml or more above its lowest level, this could be a sign that your cancer has come back. Your doctor will continue to check your PSA level and will talk to you about further tests and treatment options.

How Is A Recurrence Detected
After prostate cancer treatment, you will go for medical check-ups every few months as determined by your doctor. At each follow-up appointment, your doctor will order a blood test to measure PSA levels. This test helps your doctor detect a cancer recurrence. You will also be examined. New symptoms should be reported to the doctor, as these may prompt other testing.
When PSA test results suggest that the cancer has come back or continued to spread, X-rays or other imaging tests may be done, depending on your situation and symptoms. Your doctor may use a radioactive tracer called Axumin with a PET scan to help detect and localize any recurrent cancer so that it could be biopsied or treated.
Your doctor may also use a new drug called Ga 68 PSMA-11 in the scan which binds to PSMA-positive prostate cancer lesions in the tissues of the body so they can be targeted for treatment.
Why Does It Take So Long To Recover Erections After The Very Best Surgery
A number of explanations have been proposed for this phenomenon of delayed recovery, including mechanically induced nerve stretching that may occur during prostate retraction, thermal damage to nerve tissue caused by electrocoagulative cautery during surgical dissection, injury to nerve tissue amid attempts to control surgical bleeding, and local inflammatory effects associated with surgical trauma.

How Common Is Recurrence Of Prostatecancer
Early diagnosis and treatment can cure localized prostate cancer, and most men will remain free of cancer for many years, potentially the rest of their life. Despite these favorable statistics and improvements in the available local therapies, up to 40% of men treated for prostate cancer will experience a recurrence.
Since the majority of these patients are already elderly, they are more likely to die from causes other than prostate cancer. This age-dependent effect reduces the need for immediate clinical intervention following recurrence.
It is noteworthy that more than 90% of prostate cancer cases present while the cancer is either localized in the prostate gland or has spread to areas around the gland .
Therefore, these men can expect to live at least five years after diagnosis, which is a much better outcome than other cancers. Among the 10% with advanced disease, about 30% can expect to reach the five-year survival mark.

Ben Brewer is the name of a brave young man featured on 9 News for his 11-year battle with cancer. The report states that young Ben was diagnosed with blastoma at the age of two. He had undergone various treatments including chemotherapy and as of last year, liquid radiation. He has relapsed three times prior to the radiation therapy.
Ben, his family, and his medical team truly believed they had seen the last of the blastoma after the radiation treatment. He was featured on 9 News as the radioactive boy due to his bravery with the complex and often, isolating treatment.
Ben was cancer free from 2014 until September 2015, when scans confirmed the presence of five new spots in various parts of his body. As he and his family discuss options with the doctors at Childrens Hospital Colorado, we cant help but wonder why cancer sometimes comes back after radiation therapy, as stringent as the treatment is.

When Is Radiation Therapy Used
There are some instances where the practitioners opt for radiotherapy for prostate cancer as opposed to other forms of treatment. Here are some of the situations in which radiation therapy may be used:
- As the first treatment of cancer, which is still confined to the prostate gland.
- It is used along with hormone therapy during the first treatment for prostate cancer that has extended the nearby tissues.
- After the reoccurrence of cancer in the area, it was before surgery.

Focal treatment for recurring prostate cancer after radiotherapy.
Following radiotherapy for prostate cancer, there is a 1 in 5 chance of recurrence where the cancer returns inside the prostate. Most men in this situation have been traditionally treated with a palliative strategy with watchful waiting and hormones. Hormones that suppress testosterone, the male hormone, keep the cancer in control for 2-3 years but long term use can lead to diabetes, heart disease, bone thinning as well weight gain, tender enlarged breasts and tiredness.
The alternative of surgical removal prostatectomy carries significant problems. Treating prostate cancer that recurs locally in the prostate following radiotherapy can be problematic as the tissue becomes sticky. Surgery results in side effects including incontinence and bowel damage requiring a stoma and subsequent major open surgery and total loss of erectile function. More radiotherapy can also increase toxicity with the same result and because the cancer has resisted radiotherapy once, there is concern further radiotherapy may not work a second time. Having said that there is work is starting on using MR guided Linac to treat recurrence, but this is its very early stages.
