When Do I Need A Biopsy

Your doctor may order a biopsy if your prostate-specific antigen level in your blood work is elevated or there is an abnormal lump found during a digital rectal exam. During a digital rectal exam, your doctor inserts a finger up your bottom to feel if your prostate is enlarged or has bumps. Another option before a biopsy is an ultrasound. Instead of a finger, a small probe is inserted to take pictures of the prostate.

An MRI-guided prostate biopsy may be used in patients who have a rising PSA level, yet a negative ultrasound-guided biopsy. It also may be used in situations where a diagnostic prostate MRI performed because of rising PSA demonstrates a very small abnormality that may not be easily targeted by ultrasound. An MRI image to guide a biopsy can improve the ability to detect prostate tumors which may require treatment.

What Is A Transperineal Biopsy

This is where the doctor inserts the biopsy needle into the prostate through the skin between the testicles and the back passage . In the past, hospitals would only offer a transperineal biopsy if other health problems meant you couldnt have a TRUS biopsy. But many hospitals have stopped doing TRUS biopsies and now only do transperineal biopsies.

A transperineal biopsy is normally done under general anaesthetic, so you will be asleep and wont feel anything. How To Do Prostate Milking Yourself

Self-prostate milking can be performed in the privacy of your home. You can do it alone, or with the help of your partner / spouse. If youre going solo, make sure to trim your nails and wash your hands and body beforehand. Here are the steps youll need to take:

  • You might want to consult your doctor before performing prostate milking. In some cases milking is NOT recommended and may cause harm particularly in case with acute or severe Prostatitis or being under increased risk of developing prostate cancer.
  • Make sure to empty your bladder and ideally have a bowel movement to eliminate discomfort and provide relaxation.
  • Wash your hands and put on a rubber glove. Use water-based lubricant on your fingers.
  • Make sure your finger nails are trimmed to avoid injury.
  • Carefully insert your finger or fingers into your anus. Move your fingers slowly and gently at first in the direction of your navel. Be careful and get used to the sensation.
  • Move your fingers gradually back and forth along the area of the rectum.

    Why Might I Need Cryotherapy For Prostate Cancer

    Cryotherapy may be a good treatment option for prostate cancer treatment in the following situations:

    • Men with cancer in the prostate gland that hasnt spread to other parts of the body

    • Men who arent well enough to get radiation or procedure

    • When the goal isnt to cure, it may be useful for men who have cancer that has spread beyond the prostate gland and need treatment for symptoms

    • Sometimes its used for men who have had unsuccessful results with radiation therapy

    Some experts believe cryotherapy can be helpful when the prostate cancer cells arent as sensitive to radiation.

    Cryotherapy may not be recommended for men who have a very large prostate gland.

    Cryotherapy is less invasive than standard procedure. It involves needles that are put in through the skin under the scrotum, called the perineum. There is less blood loss, a shorter hospital stay, faster recovery, and less pain. It can be repeated, if needed.

    There may be other reasons for your healthcare provider to recommend cryotherapy.

    Why Does Prostate Cancer Happen

    The causes of prostate cancer are largely unknown. But certain things can increase your risk of developing the condition.

    The chances of developing prostate cancer increase as you get older. Most cases develop in men aged 50 or older.

    For reasons not yet understood, prostate cancer is more common in men of African-Caribbean or African descent, and less common in Asian men.

    Men whose father or brother were affected by prostate cancer are at slightly increased risk themselves.

    Do We Know Which Treatment Is Better For Prostate Cancer Brachytherapy Or External Beam Radiation

    Its not a question of which therapy is better but rather which therapy is the most tailored, pinpointed radiation for the patients specific disease.

    When it comes to early stages of disease, patients very frequently do well with either brachytherapy or external beam radiation. Success rates of around 90% or higher can be achieved with either approach. When the disease is somewhat more advanced based on the PSA level or the Gleason score or visible evidence of disease on an MRI we have learned over the years that higher doses of radiation are critical to achieving better results. Some evidence suggests that for patients with intermediate- or high-risk prostate cancer, a combined approach using brachytherapy along with external beam radiation may be best.

    Data that we have published recently show that for patients with intermediate-risk disease, the combination of external beam radiation with brachytherapy not only provides better biochemical control, in terms of PSA level, but also reduces the risk of distant metastases, or spread of the disease. Another recent study from Canada, which compared outcomes in patients who were treated with external beam radiation or a combination approach, found superior results when the combined approach was used. These studies provide strong evidence that higher doses of radiation provide an important benefit to patients with intermediate-risk and high-risk prostate cancers.
