When Do I Need A Biopsy

Your doctor may order a biopsy if your prostate-specific antigen level in your blood work is elevated or there is an abnormal lump found during a digital rectal exam. During a digital rectal exam, your doctor inserts a finger up your bottom to feel if your prostate is enlarged or has bumps. Another option before a biopsy is an ultrasound. Instead of a finger, a small probe is inserted to take pictures of the prostate.

An MRI-guided prostate biopsy may be used in patients who have a rising PSA level, yet a negative ultrasound-guided biopsy. It also may be used in situations where a diagnostic prostate MRI performed because of rising PSA demonstrates a very small abnormality that may not be easily targeted by ultrasound. An MRI image to guide a biopsy can improve the ability to detect prostate tumors which may require treatment.

What Is Prostate Ultrasound & Biopsy

We perform prostate ultrasound and biopsy through the rectum using a special probe about the size of an index finger. The doctor removes small samples of tissue from your prostate to test for cancer. It takes only a few minutes and requires no anesthesia. We use prostate ultrasound and biopsy to evaluate abnormal results from a digital rectal exam or an elevated prostate-specific antigen blood test.

A prostate ultrasound probe produces harmless high-frequency sound waves that image the surface of the prostate. The results are recorded and video or photographic images of the prostate gland are created. The probe can image the prostate from different angles to help us determine the size of your prostate and detect abnormal growths.

During prostate ultrasound and biopsy we use tiny needles to sample tissue from several areas of the prostate then analyze those samples in the lab. The results of this test procedure help us diagnose diseases of the prostate. If we find prostate cancer, we will stage the cancer and determine its likelihood of metastasizing or spreading to other parts of the body.

Prostate Massage And Cancer

Men suspected of having prostate cancer should not be treated with prostate massage, as this may cause tumor cells to break off and spread to nearby tissues.

There’s some evidence that prostate massage, prior to certain tests for prostate cancer, may increase the sensitivity of the test, making it more likely the cancer will be detected.

However, a prostate-specific antigen blood test should not be conducted immediately after a prostate massage, as this could lead to false-positive results.

Even if cancer is not an issue, it’s important to avoid injuring the prostate. The thin, pliable membrane covering the prostatethe prostatic plexusis full of nerves that serve the sponge-like corpora cavernosa of the penis.

Massaging the prostate too intensely can damage nerves in the corpora cavernosatwo chambers composed of erectile tissue that run the length of the penis. Damage to the corpora cavernosa can result in pain and erectile dysfunction.

What Is External Prostate Massage

An external prostate massage is the process of massaging the prostate gland from outside the body. This can be an effective alternative to a prostate massager that you insert into your backside.

With an external prostate massager nothing is inserted into the body. There is no need for messy lubricants, and you can be fully clothed with this form of prostate massage.

You sit on the external prostate massager, rather than insert a prostate massager into your body. This is the easiest way get an effective and stimulating external prostate massage. Its that easy.

With this form of prostate massage, you get light to medium pressure to give your prostate gland and perineum a therapeutic prostate massage.

Chronic Prostatitis

Screening finds prostate cancer in some men who would never have had symptoms from their cancer in their lifetime. Treatment of men who would not have had symptoms or died from prostate cancer can cause them to have complications from treatment, but not benefit from treatment. This is called overdiagnosis.

Prostate cancer is diagnosed with a prostate biopsy. A biopsy is when a small piece of tissue is removed from the prostate and looked at under a microscope to see if there are cancer cells. Older men are more likely to have a complication after a prostate biopsy.

A prostate biopsy can cause

  • Pain.
  • Blood in the semen or ejaculate.

What Is A Transperineal Biopsy

This is where the doctor inserts the biopsy needle into the prostate through the skin between the testicles and the back passage . In the past, hospitals would only offer a transperineal biopsy if other health problems meant you couldnt have a TRUS biopsy. But many hospitals have stopped doing TRUS biopsies and now only do transperineal biopsies.

A transperineal biopsy is normally done under general anaesthetic, so you will be asleep and wont feel anything. A general anaesthetic can cause side effects your doctor or nurse should explain these before you have your biopsy. Some hospitals now do transperineal biopsies using a local anaesthetic, which numbs the prostate and the area around it, or a spinal anaesthetic, where you cant feel anything in your lower body.

The doctor will put an ultrasound probe into your back passage, using a gel to make this easier. An image of the prostate will appear on a screen, which will help the doctor to guide the biopsy needle.

If youve had an MRI scan, the doctor may just take a few samples from the area of the prostate that looked unusual on the scan images. This is known as a targeted biopsy.

How To Do Prostate Milking Yourself

Self-prostate milking can be performed in the privacy of your home. You can do it alone, or with the help of your partner / spouse. If youre going solo, make sure to trim your nails and wash your hands and body beforehand. Here are the steps youll need to take:

  • You might want to consult your doctor before performing prostate milking. In some cases milking is NOT recommended and may cause harm particularly in case with acute or severe Prostatitis or being under increased risk of developing prostate cancer.
  • Make sure to empty your bladder and ideally have a bowel movement to eliminate discomfort and provide relaxation.
  • Wash your hands and put on a rubber glove. Use water-based lubricant on your fingers.
  • Make sure your finger nails are trimmed to avoid injury.
  • Carefully insert your finger or fingers into your anus. Move your fingers slowly and gently at first in the direction of your navel. Be careful and get used to the sensation.
  • Move your fingers gradually back and forth along the area of the rectum.
  • Find the prostate gland, which is like a small round ball, size of a large walnut. Gently massage and dont press too hard, as it is very sensitive.
  • Continue for a few minutes. You may feel like you want to go to the bathroom, but ignore that feeling. When the prostate gland is stimulated enough for ejaculation to occur, you might even experience sexual pleasure or orgasm.

    Where Is The Prostate?

Most men do not find prostate biopsy excessively painful or uncomfortable, and the complications are usually not seriousbut can be. Certain steps taken before, during, and after the procedure can improve the outcome:

Take antibiotics. Taking preventive antibioticsbefore and after the procedurecuts the risk of infection substantially. Most infections are not dangerous but could become so if they get out of control. The overall chance of being hospitalized with an infection after prostate biopsy is 1% to 3%.

Review medications. Before the biopsy, your doctor may advise you to stop taking daily low-dose aspirin or an anticoagulant such as warfarin , dabigatran , edoxaban , rivaroxaban , or apixaban . These drugs reduce the bloodâs ability to clot. Your doctor will weigh the chance of bleeding against the need for anticoagulants to prevent heart problems or stroke.

Expect anesthesia. Get local anesthesia for the biopsy. This means an injection of a numbing drug into the prostate gland to reduce pain during the biopsy.

Why Might I Need Cryotherapy For Prostate Cancer

Cryotherapy may be a good treatment option for prostate cancer treatment in the following situations:

    • Men with cancer in the prostate gland that hasnt spread to other parts of the body

    • Men who arent well enough to get radiation or procedure

    • When the goal isnt to cure, it may be useful for men who have cancer that has spread beyond the prostate gland and need treatment for symptoms

    • Sometimes its used for men who have had unsuccessful results with radiation therapy

Some experts believe cryotherapy can be helpful when the prostate cancer cells arent as sensitive to radiation.

Cryotherapy may not be recommended for men who have a very large prostate gland.

Cryotherapy is less invasive than standard procedure. It involves needles that are put in through the skin under the scrotum, called the perineum. There is less blood loss, a shorter hospital stay, faster recovery, and less pain. It can be repeated, if needed.

There may be other reasons for your healthcare provider to recommend cryotherapy.

    Why Does Prostate Cancer Happen

The causes of prostate cancer are largely unknown. But certain things can increase your risk of developing the condition.

The chances of developing prostate cancer increase as you get older. Most cases develop in men aged 50 or older.

For reasons not yet understood, prostate cancer is more common in men of African-Caribbean or African descent, and less common in Asian men.

Men whose father or brother were affected by prostate cancer are at slightly increased risk themselves.

Recent research also suggests that obesity increases the risk of prostate cancer.

Biopsy During Surgery To Treat Prostate Cancer

If there is more than a very small chance that the cancer might have spread , the surgeon may remove lymph nodes in the pelvis during the same operation as the removal of the prostate, which is known as a radical prostatectomy .

The lymph nodes and the prostate are then sent to the lab to be looked at. The lab results are usually available several days after surgery.

Do We Know Which Treatment Is Better For Prostate Cancer Brachytherapy Or External Beam Radiation

    Enlarged Prostate Gland

Its not a question of which therapy is better but rather which therapy is the most tailored, pinpointed radiation for the patients specific disease.

When it comes to early stages of disease, patients very frequently do well with either brachytherapy or external beam radiation. Success rates of around 90% or higher can be achieved with either approach. When the disease is somewhat more advanced based on the PSA level or the Gleason score or visible evidence of disease on an MRI we have learned over the years that higher doses of radiation are critical to achieving better results. Some evidence suggests that for patients with intermediate- or high-risk prostate cancer, a combined approach using brachytherapy along with external beam radiation may be best.

Data that we have published recently show that for patients with intermediate-risk disease, the combination of external beam radiation with brachytherapy not only provides better biochemical control, in terms of PSA level, but also reduces the risk of distant metastases, or spread of the disease. Another recent study from Canada, which compared outcomes in patients who were treated with external beam radiation or a combination approach, found superior results when the combined approach was used. These studies provide strong evidence that higher doses of radiation provide an important benefit to patients with intermediate-risk and high-risk prostate cancers.
