Using MRI, doctors determine whether or not something in the MRI, some quantitative signal, gives a sense of which cancers are at risk for metastasis. There can be quantitative metrics of diffusion that can be measured to help. Taneja predicts the combination of MRI signal with genetic findings in the sampled tissue to create what would be called a radio-genomic that may be able to predict who is at risk. However, this is still in development.

Imaging And Interventional Radiology

Our radiologists are experts in magnetic resonance imaging , ultrasound, computed tomography , and fluoroscopy, and combine their clinical expertise with advances in technology including MR-US fusion for prostate imaging, as well as positron emission tomography scans and advanced nuclear medicine scans. In addition, Blue Light Cystoscopy, an advanced endoscopic technology, is employed to improve bladder cancer detection and reduce risk of cancer recurrence.

A Biopsy Is Done To Diagnose Prostate Cancer And Find Out The Grade Of The Cancer

A transrectal biopsy is used to diagnose prostate cancer. A transrectal biopsy is the removal of tissue from the prostate by inserting a thin needle through the rectum and into the prostate. This procedure may be done using transrectal ultrasound or transrectal MRI to help guide where samples of tissue are taken from. A pathologist views the tissue under a microscope to look for cancer cells.

Sometimes a biopsy is done using a sample of tissue that was removed during a transurethral resection of the prostate to treat benign prostatic hyperplasia.

If cancer is found, the pathologist will give the cancer a grade. The grade of the cancer describes how abnormal the cancer cells look under a microscope and how quickly the cancer is likely to grow and spread. The grade of the cancer is called the Gleason score.

To give the cancer a grade, the pathologist checks the prostate tissue samples to see how much the tumor tissue is like the normal prostate tissue and to find the two main cell patterns. The primary pattern describes the most common tissue pattern, and the secondary pattern describes the next most common pattern. Each pattern is given a grade from 3 to 5, with grade 3 looking the most like normal prostate tissue and grade 5 looking the most abnormal. The two grades are then added to get a Gleason score.

Proton Therapy Vs Traditional Radiation

Compared to conventional radiation treatment including Cyberknife/SBRT, proton therapy can deliver high radiation doses to the prostate and pelvic lymph nodes while better sparing the healthy bowel and bladder. For men with prostate cancer, the side effects of conventional radiation treatment can have serious quality of life implicationsparticularly relating to urinary, bowel and erectile function.

Because proton therapy reduces the amount of radiation to healthy organs near the prostate by 80%allowing significantly less unnecessary irradiation to the bladder, bowels, and surrounding healthy tissuesthere is reduced risk of secondary cancers and other long-term effects compared to conventional radiation.

For these reasons, proton therapy is particularly beneficial for men with high-risk prostate cancer that has spread or is at risk of spreading to the lymph nodes and in treating patients with prostate cancer that has returned after previous radiation therapy or prostatectomy.

Proton therapy treatments are delivered on an outpatient basis. Most patients continue their normal activities throughout treatment, without having their quality of life impacted by their proton therapy. Entire treatment courses in only five days with proton SBRT and novel clinical trials are offered to appropriate candidates.

Men who are candidates for a treatment called focal ablation recover quickly and typically do not develop urinary incontinence or sexual dysfunction. A requirement of this treatment is careful follow up to ensure that the cancer has truly been destroyed and new cancers do not develop in the untreated prostate.
