Cancer May Spread From Where It Began To Other Parts Of The Body
When cancer spreads to another part of the body, it is called metastasis. Cancer cells break away from where they began and travel through the lymph system or blood.
- Lymph system. The cancer gets into the lymph system, travels through the lymph vessels, and forms a tumor in another part of the body.
- Blood. The cancer gets into the blood, travels through the blood vessels, and forms a tumor in another part of the body.
The metastatic tumor is the same type of cancer as the primary tumor. For example, if prostate cancer spreads to the bone, the cancer cells in the bone are actually prostate cancer cells. The disease is metastatic prostate cancer, not bone cancer.
Denosumab, a monoclonal antibody, may be used to preventbone metastases.
Stage Iv Of Prostate Cancer:
Stage IV of prostate cancer is a very advanced stage, with the cancer spreading to the lymph nodes and distant organs like the liver, bones or lungs. It is categorized into the following stages:
Stage IVA:
In this stage, the cancer has spread to the nearby tissues and lymph nodes, but not to any distant organs. It is staged as Any T, N1, M0, Any Grade Group and any PSA levels.
Stage IVB:
These cancers have spread beyond the prostate to the nearby tissues, distant lymph nodes and other organs. It is staged as Any T, any N, M1, Any Grade Group with any levels of PSA.
Treatment Of Metastatic Stage Iv Or D2 Prostate Cancer
Prostate cancer that has spread to distant organs and bones is treatable, but not curable with current standard therapies. Hormone therapy has been the standard treatment of metastatic prostate cancer for many years. Metastatic prostate cancer can be controlled with hormone therapy for many years and new treatment options continue to become available.

What Is Stage 4 Prostate Cancer
Stage IV prostate cancer is an advanced and aggressive cancer that spreads beyond the prostate gland. Prostate cancer cells are very different from the original tissue, spreading to other organs or lymph nodes. It is usually associated with a very high PSA level, too.
Prostate cancer grouping in 4 stages takes into consideration different staging systems. One of them is the TNM staging, which describes the size and extent of cancer. In stage IV prostate cancer, the tumor has grown very large, usually beyond the gland.
However, regardless of the size and extent of cancer, it will be stage 4 when it has already spread to distant tissues. The disease usually takes lymph nodes in stage 4 prostate cancer. And metastasis is the spread of cancer to other parts of the body. It can be distant lymph nodes, bone tissue, or any other organ.
Doctors also consider the Gleason score to define stage 4 prostate cancer. This score takes into consideration how cancer cells look like under the microscope. In stage 4, prostate cancer, cancer cells are aberrant and very different from healthy tissue. They lose all of their characteristics, including the adhesion to other cells. Thats why they are more likely to spread to other tissues .
We can practically summarize stage 4 prostate cancer by saying that it is metastatic prostate cancer. It is a late stage of the disease, associated with various complications and a poor prognosis.
Hormone Therapy Also Called Androgen Deprivation Therapy
Hormone therapy is used to control the production or block the impact of male hormones, called androgens, on theprostate cancer cells. However standard hormone therapy may lead to side effects such as hot flashes, weight-gain,dizziness, and osteoporosis, often diagnosed through a bone scan. To avoid these side effects, we always useappropriatelevels of immunotherapy as a follow-up to hormone therapy.
There are several types of hormone therapy to lower testosterone levels and other androgens, produced by thetesticles,adrenal glands, and even the prostate cancer cells. Some of the different types of hormone therapy include:
- Orchiectomy, which is the surgical removal of testicles
- Luteinizing hormone releasing hormone agonists and antagonists
- Anti-androgens or androgen receptor antagonists

What Treatments Are Available
If you have advanced prostate cancer, treatment wont cure your cancer. But it can help keep it under control and manage any symptoms.
If youve just been diagnosed with advanced prostate cancer, you may be offered the following treatments:
- chemotherapy with hormone therapy
- clinical trials.
Research has found that having radiotherapy together with one of the main treatments listed above can help some men with advanced prostate cancer to live longer. But radiotherapy isnt suitable for all men with advanced prostate cancer.
If you live in Scotland, you may also be offered a type of hormone therapy called abiraterone acetate together with standard hormone therapy. In the rest of the UK, abiraterone is currently only given to men with advanced prostate cancer that has stopped responding to other types of hormone therapy. The National Institute for Health and Care Excellence is currently deciding whether to make it available for men who have just been diagnosed with advanced prostate cancer.

Choosing To Stop Treatment Or Choosing No Treatment At All
For some people, when treatments have been tried and are no longer controlling the cancer, it could be time to weigh the benefits and risks of continuing to try new treatments. Whether or not you continue treatment, there are still things you can do to help maintain or improve your quality of life.
Some people, especially if the cancer is advanced, might not want to be treated at all. There are many reasons you might decide not to get cancer treatment, but its important to talk to your doctors and you make that decision. Remember that even if you choose not to treat the cancer, you can still get supportive care to help with pain or other symptoms.

Metastatic Prostate Cancer Treatment
If a patients prostate cancer has spread beyond the prostate and the surrounding area, he is given systemic therapies like hormone therapy and possibly chemotherapy. While cancer responds to hormone therapy, it is called castrate-sensitive disease. Over time, the disease may become less responsive to hormone therapy and start growing again. This is called castrate-resistant disease. Patients with castrate-resistant disease can be treated with a number of additional therapies. Many are eligible for clinical trials with newer drugs or drug combinations, including immunotherapy.

There Are Three Ways That Cancer Spreads In The Body
Cancer can spread through tissue, the lymph system, and the blood:
- Tissue. The cancer spreads from where it began by growing into nearby areas.

How Psma Lights Up Cancer Cells
In 2021, the U.S. Food and Drug Administration issued national approval to two new prostate cancer imaging tests based on similar technology. On a PET scan, the test lights up the cancerous cells that would otherwise be hidden, enabling doctors to precisely target treatment.
Both advances in imaging and therapy rely on targeting PSMA, which is not found on most normal cells but is overexpressed in cancer cells, especially those that have spread. The PSMA molecule was cloned at MSK in the early 1990s.
The Molecular Imaging and Therapy Service, led by Heiko Schöder, played a key role in the development and testing of a slightly different PSMA-directed imaging technology at MSK.
This advance is the result of years of work by the community of physicians promoting the use of PSMA agents, Dr. Schöder says. Its gratifying to see a collaborative effort result in a breakthrough that has the potential to make a difference for so many patients with advanced prostate cancer.
Before receiving the therapy, patients in the VISION trial were scanned with PSMA-directed PET imaging to make sure enough PSMA was present in the cells to make them likely to respond to the treatment. If so, they received the radioactive drug by injection over four to six sessions, spaced six weeks apart.
As a next step, Dr. Morris and colleagues are looking into using the PSMA-directed therapy earlier rather than only after the prostate cancer has spread.

General Prostate Cancer Survival Rate
According to the American Cancer Society:
- The relative 5-year survival rate is nearly 100%
- The relative 10-year survival rate is 98%
- The 15-year relative survival rate is 91%
Note: Relative survival rate means the percentage of patients who live amount of years after their initial diagnosis.
Keep in mind, however, that because the compiled list figures are of cancers diagnosed up to 15 years ago, you may have an even greater chance of survival than these indicate due to advances in prostate cancer treatment technology

Gleason Prostate Cancer Score
1960s as a way to measure how aggressive your prostate cancer may be.
A pathologist determines your Gleason score by looking at a biopsy of your prostate tissue under a microscope. They grade the cells in the biopsy on a scale of 1 to 5. Grade 1 cells are healthy prostate, whereas grade 5 cells are highly mutated and dont resemble healthy cells at all.
The pathologist will calculate your Gleason score by adding together the number of the most prevalent type of cell in the sample and the second most prevalent type of cell.
For example, if the most common cell grade in your sample is 4 and the second most common is 4, you would have a score of 8.
A Gleason score of 6 is considered low-grade cancer, 7 is intermediate, and 8 to 10 is high-grade cancer.
Treatment Of Stage Iv Prostate Cancer

Standard treatment of stage IV prostate cancer may include the following:
- Hormone therapy.
- Hormone therapy combined with chemotherapy.
- Bisphosphonate therapy.

- Active surveillance. If the cancer begins to grow, hormone therapy may be given.
- A clinical trial of radical prostatectomy with orchiectomy.

According to the Prostate Cancer Foundation, a few minor changes to eating habits can be helpful for those dealing with the effects of prostate cancer. The organization recommends incorporating vegetables into many of your meals and minimizing fat intake from red meat and dairy products. The group also recommends getting vitamins from food sources like whole grains and vegetables as opposed to vitamin supplements. It is particularly important to get calcium from food sources, namely dark green leafy vegetables and low-fat dairy foods.
Coping and support
If you or a loved one has prostate cancer, it’s important to remember that you are not alone: More than 150,000 men are diagnosed with prostate cancer every year, according to the Prostate Cancer Foundation.
Still, it is normal for men diagnosed with prostate cancer to feel alone, uncertain or fearful. Support groups are an excellent resource for men with prostate cancer, providing a community with whom to share encouragement, information and emotional support. These groups range from online communities with people who can help answer questions to groups of men diagnosed with prostate cancer or family members of those diagnosed. They are sometimes facilitated by mental health professionals and experts in the field of prostate cancer.

When deciding what treatment is best for you, your doctors will consider:
- the type and size of the cancer
- what grade it is

Stage Iv Prostate Cancer Prognosis
Prostate cancers detected at the distant stage have an average five-year survival rate of 28 percent, which is much lower than local and regional cancers of the prostate. This average survival rate represents stage IV prostate cancers that have metastasized beyond nearby areas to lymph nodes, organs or bones in other parts of the body.
