Looking Into The Causes Of Oral Cancer Pain

The most common cancer types, such as breast, lung, prostate, and colon cancer, rarely cause pain at the site where they originate. However, pain in the head and neck from oral cancer is notable for its intensity and prevalence, with approximately 70%75% of patients with oral cancer experiencing pain, said Brian Schmidt, D.D.S., M.D., Ph.D., of New York University’s College of Dentistry.

Dr. Schmidt noted that oral cancer pain is persistent because the tongue and floor of the mouth are disturbed when a patient talks, chews, or swallows. Pain occurs when substances produced and secreted by a tumor activate nearby pain-sensing neurons, which then transmit pain signals to the central nervous systemand the stimulation from simply talking or swallowing exacerbates this effect.

To study oral cancer pain, Dr. Schmidt collects tumor-produced, pain-causing substances during surgery with a technique called microdialysis. The cancer tissue degenerates rapidly after surgery. By sampling the cancer microenvironment with the tumor in place, we glean a more accurate understanding of how the tumor causes pain, Dr. Schmidt said.

In other cases, Dr. Schmidt removes small pieces of tumor as well as normal tissue from an adjacent area in the mouth. From these samples, his lab team works to identify pain-promoting substances found in the cancerous tissue but not the normal tissue.

What Other Problems Might An Enlarged Prostate Cause

A small number of men may find it difficult to empty their bladder properly this is called urine retention. If youve been diagnosed with an enlarged prostate, your doctor will look at your test results to see if youre at risk of urine retention. You may be more likely to get urine retention if:

  • youre aged 70 or over
  • your prostate is very large
  • you have a raised prostate specific antigen level
  • you have severe urinary symptoms and a very slow flow.

Chronic urine retention

This is where you cant empty your bladder fully, but can still urinate a little. It usually develops slowly over time. Chronic means long-lasting. The first signs often include a weak flow when you urinate, or leaking urine at night. You may feel that your abdomen is swollen, or that youre not emptying your bladder fully.

Chronic urine retention is usually painless. But the pressure of the urine can slowly stretch your bladder muscle and make it weaker. This can cause urine to be left behind in the bladder when you urinate. If you dont empty your bladder fully, you might get a urine infection, need to urinate more often, leak urine at night, or get painful bladder stones. You might also see some blood in your urine. Chronic urine retention can damage your bladder and kidneys if it isnt treated.

There are treatments for chronic urine retention, including:

  • passing a thin, flexible tube called a catheter to drain urine from your bladder
  • surgery to widen the urethra.

What Types Of Testing Should I Expect For Monitoring My Condition

Since metastatic prostate cancer isnt curable, your doctor will most likely set up regular visits to check the cancers location, and to manage any long-term side effects from the cancer or any medication youre taking.

And since treatments for advanced prostate cancer are changing so fast and need to be given in a certain sequence to be the most effective, youll probably have not only a prostate cancer doctor but other specialists taking care of you. Your care team should coordinate closely, say the authors of a major study of such teams published in August 2015 in the journal Annals of Oncology.

Along with regularly testing your prostate-specific antigen levels, your care team may request blood tests that measure such prostate cancer indicators as alkaline phosphatase and lactate dehydrogenase. Magnetic resonance imaging or PET scans of the spine or other bones can also help identify how your cancer responds to treatment.

If youve had radiation, youre at an increased risk for bladder and colorectal cancer and should get screened regularly for these as well.

The tests youll have and how often youll need them should be customized to you. Your care team will consider your overall health, medications that are safe for you to take, other health conditions you might have, and what stage your cancer was when you were diagnosed.

Other Challenges Of Studying And Managing Pain

When it comes to controlling pain, its preferable to start treatment early in the course of the diseaseand, ideally, before pain develops in the first place, Dr. Mantyh said. But that can be challenging, he said, because oncologists tend to be more focused on treating the tumor and it can be hard to get buy-in from them to give early in the disease to control pain.

One concern among oncologists, Dr. Dougherty said, is that drugs to prevent pain might interact and interfere with anticancer treatments. You want patients to survive the therapy first, and an agent that might interact with a cancer therapy would have a clear downside, he said.

However, he added, its important to address nerve-related pain such as CIPN as soon as possible, because once pain becomes chronic, a patients nervous system starts to change and try to adapt to that condition now you have a new problem of trying to get the nervous system to go back to where it was.

One thing is clear, scientists working in this area agree: Improving the understanding of pain and using that knowledge to guide the discovery and development of new treatments is critical for patients physical and mental well-being.
