What is Testicular Pain & How Can It Be Treated?

The diagnosis of prostatitis can be made by a physician based on symptoms and a physical exam. The prostate is sometimes swollen and tender. A urine sample may also be collected to look for infection or inflammation. Sometimes a prostate massage is performed and the fluid expressed from the penis after a prostate massage is examined.

How Is Epididymitis Diagnosed

Your doctor will first complete a physical examination. Theyll look for swelling of the testicles, swelling of the lymph nodes in the groin area, and abnormal discharge from the penis. If theres discharge, your doctor will use a cotton swab to collect a sample and test for STIs.

Your doctor may also perform the following tests and procedures:

  • rectal examination, which can show if an enlarged prostate caused your condition
  • blood tests, such as a CBC , to determine whether theres an infection in your system
  • urine sample, which can indicate if you have a urinary tract infection or an STI

Imaging tests may be done to rule out other conditions. These tests produce detailed images that allow your doctor to see structures in the body very clearly. Your doctor might order a testicular ultrasound to get images of the testicles and the surrounding tissues in the scrotum.

Treatment for epididymitis involves treating the underlying infection and easing symptoms.

Common treatments include:

  • antibiotics, which are administered for 4 to 6 weeks in chronic epididymitis, and can include doxycycline and ciprofloxacin
  • pain medication, which can be available over-the-counter or can require a prescription
  • anti-inflammatory medication like piroxicam or ketorolac
  • bed rest
  • wearing an athletic cup for support
  • avoiding lifting heavy objects

In cases of an STI, you and your partner should abstain from sexual intercourse until youve completed your course of antibiotics and are fully cured.

How We Diagnose Prostatitis:

Prostatitis may be a clinical diagnosis, meaning it is presumed to be the causative issue even when no definitive findings are found. Our urologists may order/perform the following to diagnose prostatitis:

  • History & Physical Exam
  • Digital Rectal Exam: Inserting a gloved, lubricated, finger into the rectum to feel the prostate. The prostate may feel boggy and is usually more tender than normal in men with prostatitis.
  • Urinalysis & Culture: Urine will almost always be positive for infection in men with bacterial prostatitis. However, because not all prostatitis is caused by bacteria, the urine may be negative for infection.
  • Blood cultures/labs
  • Transrectal ultrasound: An ultrasound probe is inserted into the rectum to visualize the prostate.
  • Cystoscopy: A look at the prostate and bladder with a small camera inserted through the urethra.
  • Urodynamics: A study used to exam the function of the bladder and help determine if there is an outflow obstruction.

What Is Painful Ejaculation

Ejaculation is the release of semen from the penis during the male orgasm. It is normally a pleasurable experience and shouldnt be uncomfortable.

Yet ejaculation can sometimes be painful, and its not an uncommon problem. Im a 22-year-old male, otherwise very healthy and active. I have been suffering from what I believe to be as low grade inflammation of my prostate gland.

Before I go into any more detail I feel its important to highlight the following

About a year ago I began to experience sharp testicular pains and had a swollen epididymis which I was eventually prescribed anti biotics for. There appeared to be a couple of cysts, which no longer cause me any discomfort. The sharp pains still remained for another couple of months in total and the swollen epididymis subsided.

I began researching to try and find out what was causing my testicle pains and I concluded I had indulged in far too much masturbation and sex, but mainly masturbation in my adolescent years.

Looking back, I can say that for a period of 5 or 6 years I master bated at least 3 times a day, sometimes 4 or 5, and the most ever was 11 times.

I would masturbate before going to bed and as soon as I woke up at least once, regularly twice, totalling 4 times a day on average.

When the testicle pains started and I began to research, I realized my erections where no were near as hard as they used to be, my ejaculations were unsatisfactory and there were even times I failed to ejaculate upon orgasm

I decided I needed to have a massive break from masturbation or sex, which I found very difficult.

After coming back from the doctor, I had an instinct that If I ejaculated the pain might go away, so I did…

How Is Testicular Disease Treated

Testicular cancer is treated according to the type of cancer and how far it has spread. Cancer that has not spread from the testicle can be cured by orchiectomy, a surgery to remove the testicle. If it has spread outside the testicle testicular cancer treatments may include surgery to remove the abdominal lymph nodes, chemotherapy, radiation therapy, or a combination of two or more.

Testicular cancer is one of the most curable cancers. Even after it has spread, testicular cancer is usually curable. The best chances for cure are when the cancer is detected and treated early.

Epididymitis is usually treated successfully with antibiotics and anti-inflammatory drugs. Bed rest, pain medications, using an athletic supporter and ice packs on the scrotum may help more severe cases. The pain can resolve very slowly, sometimes taking weeks or months.

Testicular torsion is a true medical emergency. If caught in time, the affected testicle can be saved. Emergency surgery is usually required to “untwist” the testicle and to prevent it from happening again. During the surgery, the other side is usually fixed, as the condition tends to be bilateral.

Varicoceles usually don’t require treatment. But for men with varicoceles and impaired fertility, microsurgery to tie off the dilated veins of the varicocele is effective. Varicoceles can also be corrected without surgery by injecting a tiny coil into the abnormal veins.

Why Should I Care About Testicular Disease

Happily, significant testicular disease is uncommon and usually not serious. But if you have any testicular pain or a change in your testicles — such as a lump or a firmness — call your doctor. Even if you’re embarrassed, delaying an evaluation is not worth the risk.

As you might guess, testicular cancer is the most serious form of testicular disease. It’s also the most common cancer in men ages 18 to 35, accounting for 1% of cancer in men in the U.S. It is usually curable.

Risk factors for testicular cancer include:

  • undescended testicle as a child

About 600,000 men get it each year, most commonly between ages 19 and 35. Unprotected sex or having multiple sex partners increases the risk of infectious epididymitis.

As many as one out of every five men has varicocele, which refers to swollen and dilated veins above the testicles , a condition that is usually benign. Hydroceles, which come from increased fluid around the testicle, also pose little risk.

    What Are The Symptoms

    Symptoms of long-term prostatitis are often mild and start slowly over weeks or months. They may include:

    • An urge to urinate often. But you may pass only small amounts of urine.
    • A burning pain when you urinate.
