Large Prostate With Few Symptoms
Patients with very enlarged prostates may have few symptoms or mild symptoms. Doctors verify that there are no problems with kidney or bladder function. Requirements For Normal Voiding
1.Average Bladder Capacity 350-550 cc.
In the elderly, the capacity may decrease to 250 – 300 cc. Approximately 150-250 cc may be stored before there is a sensation of bladder pressure. As the bladder fills beyond 150-250 mL the individual begins toilet seeking.
2. Anatomical integrity of the bladder, urethra and external sphincter.
The bladder and external sphincter actvity must be coordinated to store and release urine. The healthy bladder muscle is stable and does not contract until the person relaxes the external sphincter. Post void residual of 50 to 100 mL is generally accepted as normal in the elderly. The literature suggests that younger people empty their bladder every 4 to 5 hours and older people empty their bladder every 3 to 4 hours within a 24 hour period.

Does Bph Increase Your Risk Of Developing Prostate Cancer
Based on research to date, the answer is no. However, BPH and prostate cancer have similar symptoms, and a man who has BPH may have undetected cancer at the same time.
The American Cancer Society recommends that men make an informed decision with their health care provider whether to be screened for prostate cancer. For men at average risk, this discussion should start at age 50. They also say that for men who are at high risk, such as African-American men and men with a family history of prostate cancer, screening should be considered at age 45. Men at an even higher risk, such as having more than one relative with a history of prostate cancer at an early age, should consider earlier testing.
The American Urological Association recommends against routine screening for men age 40 – 54 who have an average risk of prostate cancer. The association recommends that men ages 55 – 69 should weigh the risks and benefits of screening and treatment. For those who choose screening, the AUA suggests that they may be screened every two years rather than annually. The AUA does not recommend PSA screening in men over age 70 or any man with less than a 10-15-year life expectancy.

There was no association between body mass index and prostate size , nor were men with a BMI above the median more likely to experience BPH , controlling for age . Likewise, body fat percentage was not associated with prostate volume , or BPH , controlling for age and height. Neither systolic nor diastolic blood pressure was associated with prostate volume or BPH , nor were individuals with high systolic or high diastolic blood pressure more likely to have larger prostates or BPH , controlling for age and height. However, men with higher HbA1c had larger prostates and trended towards presentation with BPH controlling for age and height. Men with HbA1c greater than 6% trended toward a higher likelihood of BPH , controlling for age and height.

Comparison Of Measured And Trus
The next step was to examine differences in individual prostate dimensions according to the method used to obtain them. We compared each dimension estimated by ultrasound prior to surgery vs. the same respective prostate dimensions measured post-surgery from our initial cohort of 153 patients. This comparison is shown in Fig.;. We observed that the mean length of the prostate was overestimated by 10.8% using ultrasound . The mean height was overestimated using TRUS by 11.1% . Finally, the width of the gland estimated by TRUS was fairly similar to the measured value .

In some ways, the male prostate gland is equivalent to the female mammary glands. Managing The Enlarged Prostate Gland In Elderly Men
Case Presentation
A 67-year-old male is seen in the Emergency Department for urinary retention. He has a history of moderate lower urinary tract obstructive and irritative symptoms consisting of frequency, urgency, poor force and caliber of his urinary stream, and nocturia. In the last few days prior to his admission to the hospital, he had dribbling of urination and lower abdominal pain. A physical exam revealed a bladder palpable above the pubic symphysis, the rectal exam revealed a markedly enlarged prostate gland, and a urinalysis revealed 5-7 white blood cells/high power field. The patients prostate-specific antigen was 6.5 ng/mL. The blood urea nitrogen was 40 mg/dL, and serum creatinine was 2.6 mg/dL. An abdominal ultrasound revealed hydroureteronephrosis with a markedly distended bladder. A Foley catheter was inserted, and he had approximately 900 cc of residual urine. He experienced a post-obstructive diuresis with normalization of his BUN and creatinine levels. A cystoscopy revealed coaptation of the lateral lobes of the prostate and grade 3 trabeculation of the bladder. The patient had GreenLight laser treatment of his outlet obstruction and used a catheter for 3 days after the procedure. He was able to void with a good stream and had less than 75 cc of post-void residual. Follow-up BUN and creatinine levels 3 weeks after the GreenLight laser procedure were 15 mg/dL and 1.7 mg/dL.

Relationship Between Measured And Estimated Prostate Volumes
The estimated volume from TRUS imaging assumes an ellipsoid geometrical shape of the prostate using the formula . In order to identify the best coefficient for this series of 153 fresh prostate specimens, we used the measured prostate weight converted to measured prostate volume by using 1.02g/cc as the density, as defined above. Thus, the mean measured volume of our cohort was 48.1cc .
Measured prostate volumes along with TRUS-obtained prostate dimensions were then used to calculate a new coefficient from the rearranged algebraic formula: Coefficient=V/L x H x W, where L, H and W were all obtained by TRUS, and V is the measured prostatic volume of fresh prostates, obtained just after surgery, as mentioned above. This calculation was performed for each of the 153 prostates, which led to a calculated mean coefficient of 0.66.
Linear regression plots were created in order to compare the newfound coefficient of 0.66 with the ellipsoid coefficient of 0.52. Figure;a shows that plotting estimated prostate volumes against measured prostate volumes using 0.66 as a coefficient yielded an equation of y=0.892x+8.8829 with an R2 value of 0.42 . By performing the same analysis using 0.52 as the coefficient, the equation generated was y=0.5652x+13.028 with R2=0.32 .

Since everyone is different, it should be no surprise that every mans prostate is a different size.; Heres why it can matter and how it can help to know: What IS surprising to me after over a decade as an advocate, and on a Helpline, is that most men dont know what size their prostate is, and most men are never even told.
SCREENING For men who are PSA testing and screening for prostate cancer, it is very helpful to know how large or small your prostate is because it bares on how much PSA you should be making.; I have seen different experts use different formulas, but in simple terms, a 60cc prostate should make twice the PSA of a 30cc prostate.; I know there is an age-specific PSA, but honestly, it is based on the reality that a prostate gets larger as you get older, but if thats the case, why not find out what size it really is?; Ive had a couple of Helpline callers in their 70s one with a 17 cc prostate and the other with a 200 cc prostate.; And, yes they were both in their 70s.; So how does age-specific PSA work for them?; So ask your urologist for a number What is the size of my prostate?; This may be an estimate, but that is still helpful.; An urologist can estimate on DRE , and if youve ever had a biopsy, there is an ultrasound report from the biopsy that has a measurement and should have a number a prostate volume or size.; Ask for it.
