Active Surveillance And Watchful Waiting
Active surveillance means that no treatment is administered. Instead, youre given routine tests to monitor how the cancer changes over time.
Doctors usually recommend active surveillance if your cancer is unlikely to spread to organs far away from your prostate. Active surveillance typically involves regular PSA screenings and visits with a doctor. You may also get a prostate biopsy every few years to look for the presence of cancerous cells.
Watchful waiting may also be used if youre older or have any other serious health issues. You wont receive treatment or routine tests during watchful waiting, but you may be given treatment to help relieve symptoms and improve your quality of life.
Da Vinci Robotic Prostatectomy Procedure
One of the many advantages to da Vinci robotic prostatectomy for prostate cancer is that it can be performed through five tiny incisions and the prostate removed through the umbilicus at the end of the procedure. This leads to less post-operative pain and scarring and a quicker return to regular activities. Additionally, the view of the surgical field afforded by the 3D stereoscopic display means that the surgeon has a much clearer view of the nerves and blood vessels surrounding the prostate which helps to minimize blood loss and reduces the risk of damaging the nerves responsible for continence and erectile function.

If Your Prostate Cancer Comes Back
If your cancer goes into remission but later returns, follow-up treatments will depend on where the cancer is located and which treatments youâve already tried.
- If the cancer is contained in your prostate, surgery or a second attempt at radiation is suggested. If youve had a radical prostatectomy, radiation therapy is a good option. If you had radiation, radical prostatectomy might be the best approach. Cryosurgery might also be an option.

Proton Beam Therapy Vs Imrt For Prostate Cancer
But what about proton beam therapy, which costs even more than IMRT — but which did no better in the Chen study?
“This is not to say that at the end of the day, proton beam radiation is not right for prostate cancer,” Potters says. “It’s just we need to do additional study of it. And given the rate of construction of proton machines on a national level, the data should be more forthcoming to show whether it’s worth the expense for prostate cancer.”
Chen notes that doctors switched from 3D conformational therapy to IMRT even before there was hard data to show it might be better. As more and more medical centers build proton beam facilities, he suggests that doctors might again switch to the next new thing.
“We are saying, ‘Wait a minute, let’s compare outcomes before we jump to a new therapy,'” Chen says. “I don’t think this study prevents a prostate cancer patient from choosing proton beam therapy. But it does tell him to ask his doctor about showing him data on whether protons are better.”
Much of the data that is available contradicts the Chen study, says Nancy Mendenhall, MD, medical director of the University of Florida Proton Therapy Institute and associate chair of the university’s department of radiation oncology.
Mendenhall says the Chen study fails to collect important data, such as the doses radiation patients received, exactly how well their tumors responded to treatment, and exactly which toxicities the patients suffered.

April 16, 2012 — The most popular form of radiation therapy for prostate cancer may also be the best, a new comparison study finds.
Nearly all men with prostate cancer who opt for external beam radiation get a treatment called intensity-modulated radiation therapy or IMRT. IMRT has almost entirely replaced another type of radiation treatment called 3D conformational therapy — even though it’s much more expensive, with little head-to-head evidence showing it works better.
And there’s an even newer and even more expensive prostate cancer treatment: proton beam therapy. More and more medical centers are spending hundreds of millions of dollars to build new proton beam therapy centers.
Which treatment really offers a man the best chance of a prostate cancer cure with the fewest side effects? IMRT, according to Ronald C. Chen, MD, MPH, and colleagues at the University of North Carolina at Chapel Hill.
“For prostate cancer in this country, we have completely adopted IMRT,” Chen tells WebMD. “I don’t see anybody going back to 3D therapy. And thankfully, our study does show it is better in terms of cancer control and reducing long-term side effects. So IMRT is here to stay.”
The study analyzed data from 6,666 men treated with IMRT, 6,310 men treated with 3D conformational therapy, and 684 men treated with proton beam therapy.

The Prostate Cancer Study III examined the addition of ADT to SDRT and DERT in intermediate-risk patients . The preliminary results of this trial have now been published in abstract form. A total of 600 patients were enrolled. Intermediate-risk prostate cancer was defined as T1/T2 disease, GS 6, PSA level 1020 ng/mL or T1/T2 disease, GS of 7, PSA level 20 ng/mL. Patients were randomly assigned to one of three arms: 6 months of ADT plus 70 Gy to the prostate , 6 months of ADT plus 76 Gy , or 76 Gy alone . ADT consisted of bicalutamide and goserelin for 6 months. RT was delivered using a 3D conformal technique and started 4 months after the beginning of ADT. Median follow-up was 6.75 years. Primary endpoints were biochemical failure and disease-free survival . Secondary endpoints included OS, as well as hormonal and radiation-related toxicities. Biochemical failure was defined as 2 ng/mL above the PSA nadir.
What Are The Signs And Symptoms Of Prostate Cancer
For most men, prostate cancer does not cause symptoms but is detected because of an elevation in prostate specific antigen . Largely due to the widespread use of PSA tests, most prostate cancers are now diagnosed at an earlier stage and younger age compared to 20 years ago. However, symptoms of the lower urinary tract, such as hematuria , frequency and dysuria may be signs of prostate problems, including prostate cancer. How Is Prostate Cancer Diagnosed
The first line of defense against prostate cancer is a prostate exam. During a prostate exam, your doctor feels your prostate for any abnormalities or lumps. If your prostate exam shows any abnormalities, the next step is to confirm your diagnosis using a prostate biopsy or prostate ultrasound.
