Dont Miss: Steps To Prevent Prostate Cancer
Active Surveillance And Watchful Waiting
Active surveillance means that no treatment is administered. Instead, youre given routine tests to monitor how the cancer changes over time.
Doctors usually recommend active surveillance if your cancer is unlikely to spread to organs far away from your prostate. Active surveillance typically involves regular PSA screenings and visits with a doctor. You may also get a prostate biopsy every few years to look for the presence of cancerous cells.
Watchful waiting may also be used if youre older or have any other serious health issues. You wont receive treatment or routine tests during watchful waiting, but you may be given treatment to help relieve symptoms and improve your quality of life.
Da Vinci Robotic Prostatectomy Procedure
One of the many advantages to da Vinci robotic prostatectomy for prostate cancer is that it can be performed through five tiny incisions and the prostate removed through the umbilicus at the end of the procedure. This leads to less post-operative pain and scarring and a quicker return to regular activities. Additionally, the view of the surgical field afforded by the 3D stereoscopic display means that the surgeon has a much clearer view of the nerves and blood vessels surrounding the prostate which helps to minimize blood loss and reduces the risk of damaging the nerves responsible for continence and erectile function.
During da Vinci robotic prostatectomy for prostate cancer, the prostate gland and seminal vesicles are separated from the bladder and urethra and then these two structures are reconnected over a foley catheter to maintain a contained urinary channel. For advanced disease, the pelvic lymph nodes can be sampled to verify that the disease has not spread beyond the prostate.

What Are The Causes And Risk Factors Of Prostate Cancer
Prostate cancer tends to occur most commonly in men over the age of 50, and greater than 65 percent of all cases are diagnosed in men 65 years and older. The incidence of prostate cancer increases with age the lifetime risk for the average American man is about 1 in 6, but only 8 percent of men develop prostate cancer between the ages of 50 and 70 years. Well-established risk factors include older age, family history and race . Other potential risk factors thought to be associated prostate cancer include a Western diet high in saturated fat, and obesity.

If Your Prostate Cancer Comes Back
If your cancer goes into remission but later returns, follow-up treatments will depend on where the cancer is located and which treatments youâve already tried.
- If the cancer is contained in your prostate, surgery or a second attempt at radiation is suggested. If youve had a radical prostatectomy, radiation therapy is a good option. If you had radiation, radical prostatectomy might be the best approach. Cryosurgery might also be an option.
- If the cancer has spread to other parts of your body, hormone therapy might be the most effective treatment. External or IV radiation therapy or bisphosphonate drugs can relieve your bone pain.
What Does Prostate Cancer Treatment Involve
While early-stage prostate cancer might involve a “wait and see” approach, patients who require immediate prostate cancer treatment may begin with surgery to remove cancerous growths from the prostate. At Moffitt, our surgeons utilize robotic technology and muscle-sparing techniques to achieve optimal results while preserving healthy nerves and tissues.
Other therapies may be administered before or after surgery to help destroy cancer cells and prevent their growth. These include:
- Chemotherapy, which targets cancer cells that have spread to other areas of the body
- Radiation therapy, which utilizes radioactive beams to shrink tumors in a specific area
- Biological therapy, which leverages the healing abilities of the bodys immune system
- Hormone therapy, which limits the prostates ability to produce cancer-feeding hormones
Moffitt also spearheads an ambitious clinical trial program that gives our patients access to trailblazing prostate cancer treatments before they are made widely available. Thanks to our unrelenting research efforts, Moffitt has achieved designation as a Comprehensive Cancer Center from the National Cancer Institute.
Men who are traveling to receive prostate cancer treatment at Moffitt from West Palm Beach, FL, are encouraged to take advantage of discounted rates at our partner hotels. In compliance with our rigorous hygiene standards, these hotels are committed to providing comfortable accommodations to our patients and their families.

Choosing A Prostate Cancer Surgeon
When you are choosing a surgeon to treat your prostate cancer, it is important to select someone you trust and have confidence in. He or she should have enough experience to not only perform the operation you need but also to make an informed clinical judgment and change course, if necessary.
The prostate cancer experts at Seattle Cancer Care Alliance recommend choosing a surgeon who has done at least 250 prostatectomies total and who does at least 40 a year.
As you consider your options, you might want to ask your surgeon these questions.

Proton Beam Therapy Vs Imrt For Prostate Cancer
But what about proton beam therapy, which costs even more than IMRT — but which did no better in the Chen study?
“This is not to say that at the end of the day, proton beam radiation is not right for prostate cancer,” Potters says. “It’s just we need to do additional study of it. And given the rate of construction of proton machines on a national level, the data should be more forthcoming to show whether it’s worth the expense for prostate cancer.”
Chen notes that doctors switched from 3D conformational therapy to IMRT even before there was hard data to show it might be better. As more and more medical centers build proton beam facilities, he suggests that doctors might again switch to the next new thing.
“We are saying, ‘Wait a minute, let’s compare outcomes before we jump to a new therapy,'” Chen says. “I don’t think this study prevents a prostate cancer patient from choosing proton beam therapy. But it does tell him to ask his doctor about showing him data on whether protons are better.”
Much of the data that is available contradicts the Chen study, says Nancy Mendenhall, MD, medical director of the University of Florida Proton Therapy Institute and associate chair of the university’s department of radiation oncology.
Mendenhall says the Chen study fails to collect important data, such as the doses radiation patients received, exactly how well their tumors responded to treatment, and exactly which toxicities the patients suffered.

Considering Complementary And Alternative Methods
You may hear about alternative or complementary methods that your doctor hasnt mentioned to treat your cancer or relieve symptoms. These methods can include vitamins, herbs, and special diets, or other methods such as acupuncture or massage, to name a few.
Complementary methods refer to treatments that are used along with your regular medical care. Alternative treatments are used instead of a doctors medical treatment. Although some of these methods might be helpful in relieving symptoms or helping you feel better, many have not been proven to work. Some might even be harmful.
Be sure to talk to your cancer care team about any method you are thinking about using. They can help you learn what is known about the method, which can help you make an informed decision.
What Percentage Of Your Patients Can Continue To Have Sexual Intercourse After Surgery
Memorial Sloan Kettering warns patients that many if not most will not recover full erectile function after surgery. But the Martini-Klinik in Germany says 92 percent of men under the age of 60 report they do have intercourse after surgery, as do 84 percent of men over the age of 70. In fact, they parse the clinics results even more finely:

Best Prostate Cancer Radiation
Nearly all men with prostate cancer who opt for external beam radiation get a treatment called intensity-modulated radiation therapy or IMRT. IMRT has almost entirely replaced another type of radiation treatment called 3D conformational therapy — even though it’s much more expensive, with little head-to-head evidence showing it works better.
And there’s an even newer and even more expensive prostate cancer treatment: proton beam therapy. More and more medical centers are spending hundreds of millions of dollars to build new proton beam therapy centers.
Which treatment really offers a man the best chance of a prostate cancer cure with the fewest side effects? IMRT, according to Ronald C. Chen, MD, MPH, and colleagues at the University of North Carolina at Chapel Hill.
“For prostate cancer in this country, we have completely adopted IMRT,” Chen tells WebMD. “I don’t see anybody going back to 3D therapy. And thankfully, our study does show it is better in terms of cancer control and reducing long-term side effects. So IMRT is here to stay.”
The study analyzed data from 6,666 men treated with IMRT, 6,310 men treated with 3D conformational therapy, and 684 men treated with proton beam therapy.

Can Adt Compensate For Dose Escalation
The Prostate Cancer Study III examined the addition of ADT to SDRT and DERT in intermediate-risk patients . The preliminary results of this trial have now been published in abstract form. A total of 600 patients were enrolled. Intermediate-risk prostate cancer was defined as T1/T2 disease, GS 6, PSA level 1020 ng/mL or T1/T2 disease, GS of 7, PSA level 20 ng/mL. Patients were randomly assigned to one of three arms: 6 months of ADT plus 70 Gy to the prostate , 6 months of ADT plus 76 Gy , or 76 Gy alone . ADT consisted of bicalutamide and goserelin for 6 months. RT was delivered using a 3D conformal technique and started 4 months after the beginning of ADT. Median follow-up was 6.75 years. Primary endpoints were biochemical failure and disease-free survival . Secondary endpoints included OS, as well as hormonal and radiation-related toxicities. Biochemical failure was defined as 2 ng/mL above the PSA nadir.
What Are The Signs And Symptoms Of Prostate Cancer
For most men, prostate cancer does not cause symptoms but is detected because of an elevation in prostate specific antigen . Largely due to the widespread use of PSA tests, most prostate cancers are now diagnosed at an earlier stage and younger age compared to 20 years ago. However, symptoms of the lower urinary tract, such as hematuria , frequency and dysuria may be signs of prostate problems, including prostate cancer. Other uncommon symptoms of prostate cancer can include urinary retention, weight loss, abdominal pain, bone pain, or fracture.
How Is Prostate Cancer Diagnosed
The first line of defense against prostate cancer is a prostate exam. During a prostate exam, your doctor feels your prostate for any abnormalities or lumps. If your prostate exam shows any abnormalities, the next step is to confirm your diagnosis using a prostate biopsy or prostate ultrasound.
The doctors at Urology Specialist Group specializes in MRI fusion, a combination of MRI and ultrasound imaging that gives them an accurate image of your prostate. The technique was developed by one of the practices doctors, Fernando Bianco, MD.
Tampa General Hospital And Florida Urology Partners Are The First In The Nation To Offer A Groundbreaking Procedure To Protect Prostate Cancer Patients Undergoing Radiotherapy
Tampa General is announcing a new FDA-cleared procedure that helps spare healthy tissues when delivering curative radiation treatment.
TAMPA, Fla., Oct. 17, 2022 /PRNewswire/ âTampa General Hospital , in partnership with Florida Urology Partners, are the first in the U.S. to offer a next-generation technology that recently received U.S. Food and Drug Administration 510 clearance.
Barrigel is an injectable, flexible, biodegradable material that provides a barrier between the prostate and rectum to protect the healthy tissue during radiation therapy treatment for prostate cancer. It stays in place for the duration of the radiation treatment and safely dissolves in about three months.
âThis is a game changer,â said Dr. Raviender Bukkapatnam, chief of the Urology Institute at Tampa General. âThe focus of radiation treatment is to cure cancer non-invasively, and this device helps us protect the areas of the healthy tissues around the prostate gland when delivering a curative radiation treatment. Itâs the next big step in cancer care.â
Injected as a gel into the body, Barrigel prevents radiation from penetrating the rectum, reducing potential damage and side effects. The procedure can be done on an outpatient basis in less than 30 minutes and is covered by most insurance plans. The new implant could benefit many of the estimated 268,000 men in the United States diagnosed annually with prostate cancer and who will be treated with radiation therapy.
