What to Expect from a Prostate Exam

Results for simple medical tests such as some urodynamic tests, cystoscopy, and abdominal ultrasound are often available soon after the test. The results of other medical tests such as PSA blood test and prostate tissue biopsy may take several days to come back. A health care provider will talk with the patient about the results and possible treatments for the problem.

The Prostate Biopsy Procedure

The most common method for a prostate biopsy uses transrectal ultrasound to guide a hollow needle to the prostate.

The ultrasound wand, an instrument just a little larger than a pen, is inserted into the rectum and moved into place near the prostate, where it can transmit images of the prostate and surrounding tissues. A spring-loaded needle is then inserted into the rectum and guided to the prostate. Several small pieces of tissue are sampled from both sides of the prostate, then sent to a lab to be examined by a pathologista doctor trained in diagnosing disease based on tissue and other biological samples.

The prostate biopsy, which takes around 30 minutes, can be done under local anesthesia at a doctors office or an outpatient clinic. If necessary, a sedative can also be prescribed to help a patient relax before the procedure starts.

Not everyone should get a PSA test. Why? Because many men in this country are treated for low-risk prostate cancer that is discovered through the PSA test, even when it is unlikely that the disease will ever cause symptoms or lead to death. And treatment is associated with significant side effects, including impotence and incontinence . So a PSA test is recommended only for men who might be at greater risk for dying of prostate cancer for example, younger men with a family history of prostate cancer or African-American men.

To avoid the risks of over-treatment, Roswell Park follows the guidelines established by the National Comprehensive Cancer Network . The NCCN brings together world-renowned experts from 30 of the nations top cancer centers to write guidelines that specify the best ways of preventing, detecting and treating cancer. The guidelines are updated at least every year, on the basis of the latest research.

The NCCN recommends that only men in these categories receive a PSA test for early detection of prostate cancer:

A PET scan is similar to a bone scan, in that a slightly radioactive substance is injected into the blood, which can then be detected with a special camera. But PET scans use different tracers that collect mainly in cancer cells. The most common tracer for standard PET scans is FDG, which is a type of sugar. Unfortunately, this type of PET scan isnt very useful in finding prostate cancer cells in the body.

However, newer tracers, such as fluciclovine F18, sodium fluoride F18, and choline C11, have been found to be better at detecting prostate cancer cells.

Other newer tracers, such as Ga 68 PSMA-11 and 18F-DCFPyl , attach to prostate-specific membrane antigen , a protein that is often found in large amounts on prostate cancer cells. Tests using these types of tracers are sometimes referred to as PSMA PET scans.

These newer types of PET scans are most often used if its not clear if prostate cancer has spread. For example, one of these tests might be done if the results of a bone scan arent clear, or if a man has a rising PSA level after initial treatment but its not clear where the cancer is in the body.

The pictures from a PET scan arent as detailed as MRI or CT scan images, but they can often show areas of cancer anywhere in the body. Some machines can do a PET scan and either an MRI or a CT scan at the same time, which can give more detail about areas that show up on the PET scan.;

What Happens Before The Examination

Its normal to feel a bit anxious about this test and it might be uncomfortable. But it usually only takes a few minutes and you shouldnt feel any pain. Tell your doctor if you do.

You can ask for a man or woman doctor if youd prefer. Or you can have someone else in the room, such as a family member.

How Accurate Is A Prostate Ultrasound

A prostate ultrasound is more accurate than an X-ray. This is because your technician can see the images as the transducer moves through your rectum rather than having to take a snapshot and develop the images. Ultrasound tests are also safer than X-rays because they dont produce any dangerous radiation.

A prostate ultrasound is also faster than a computed tomography test, which provides 3-D images of your prostate and the areas around it. CT scans require more preparation and time for testing, and they dont provide real-time images.

Do All Men Who Are Treated For Prostate Cancer Lose Sexual And Urinary Function

Its definitely not true that all men are incontinent and impotent after treatment for prostate cancer. Urinary incontinence is usually temporary.;

There can be a sexual impact for guys who have normal function. If the cancer is near their nerve bundle, theyre going to have a decrease in sexual function. If its not, and we can do bilateral nerve sparing surgery, studies show 70 percent can get back their normal sexual function. It all depends on where the cancer is. But the truth is that we cant predict very well who will be the 30 percent who will haveor still haveED; some already do have ED because of age, diabetes, hypertension or renal failure.

The urologist may insert a camera catheter, also known as a cystoscopy procedure, to do a prostate examination if they suspect a serious problem.;

Also known as a bladder scope, the camera allows the doctor to see the inside of the prostate and bladder. An enlarged prostate can cause problems in the bladder, and a cystoscopy procedure can enable your doctor to see whats going on inside your prostate and bladder.;

This procedure enters not through the rear as in a normal DRE but the front down the penis inside the urethra through the prostate gland and into the bladder.;

Cystoscopes come in different sizes: from the thickness of a pencil, to approximately 9mm and have a light at the tip. Many cystoscopes have extra tubes to guide other instruments for surgical procedures to treat urinary problems.;

Yup, this one can hurt especially if done fast, as my urologist did! The catheter and camera size are not small. They did use a local anesthetic gel, xylocaine, but it did not make much difference to me.;

Results And Follow Up

After a DRE, your healthcare provider will know right away if there are any problems like lumps; or a prostate that is larger than usual . You will discuss any findings with your provider and plan the next steps. The results from a PSA test typically take around two weeks to come back, and your healthcare provider will go over these with you.;

If all of your test results are normal, and you and your provider decide to continue screening, you will most likely need testing every two years. However, if the PSA test is abnormal, your healthcare provider may refer you to a urologist for additional testing.;

Who Should Get A Prostate Exam

Starting at age 50, all men should discuss prostate cancer screening with their doctor. The American Cancer Society advises men at higher risk to have this conversation at age 45.

Youre considered to have an increased risk if youre African-American or if a first-degree relative had prostate cancer before age 65. If more than one first-degree relative had prostate cancer before age 65, you might want to consider beginning prostate cancer screening even earlier.

The ACS estimates there will be about 180,890 new cases of prostate cancer in the United States in 2016. About 26,120 men will die from it.

Prostate cancer is easier to treat before it spreads. However, some prostate cancers are so slow-growing that they dont always require treatment. A lot depends on your age and other factors.

Discuss your risk factors with your doctor, and ask if you should have a prostate exam as part of your yearly checkup.

The doctor can detect different prostate problems through a prostate exam. It is often considered the initial diagnostic option. The prostate exam itself can give a doctor an indication if there is an enlargement or swelling in the prostate gland. It also helps to determine prostate health.;

Lymph Node Biopsy As A Separate Procedure

A lymph node biopsy is rarely done as a separate procedure. Its sometimes used when a radical prostatectomy isnt planned , but when its still important to know if the lymph nodes contain cancer.

Most often, this is done as a needle biopsy. To do this, the doctor uses an image to guide a long, hollow needle through the skin in the lower abdomen and into an enlarged node. The skin is numbed with local anesthesia before the needle is inserted to take a small tissue sample. The sample is then sent to the lab and looked at for cancer cells.

Not all medical institutions agree on when men should begin screening for prostate cancer or even if a DRE should be part of the screening.

To help detect prostate cancer in its early stages, the American Cancer Society recommends that men talk to their doctors about the benefits, risks, and limitations of prostate cancer screening before deciding whether to be tested.

For most men at average risk, discussions about screening begin at age 50. However, some doctors recommend that men at higher risk of prostate cancer — African-American men or men with a family history of prostate cancer — start screening earlier.

What Happens During A Digital Rectal Exam

Your health care provider will insert a gloved finger into the rectum and feel the prostate for hard, lumpy, or abnormal areas. The test takes only a few minutes to complete.

You may feel slight, momentary discomfort during the test. The procedure does not cause significant pain or any damage to the prostate.

Can You Walk Us Through the Process of a Rectal Exam for Prostate Cancer?

Rettig said that someone who has a strong family history of early onset prostate cancer might want to talk to their primary care provider or other health care practitioner earlier in life, while someone with less risk might prefer to wait.

The guidelines for how often men should be screened again also vary. If you have a high prostate-specific antigen , a protein made by cells in the prostate gland, you may be recommended to come back for more frequent screenings, but those with lower PSA levels might only be advised to come back every four years or so.

If youre 55 and have you have a PSA of less than one, you can wait four years to get screened again, Rettig explained. Alternatively, if youre 45 and have a PSA of two and a half, that might be someone who might get a biopsy or be re-screened within the year. How frequently one would be prescreened is really contingent upon the specifics of the patient.

For African-American men and men with a family history of prostate cancer, it is recommended that tests be given starting at age 40. If you have urination problems or if the DRE or PSA test indicates that you might have a problem, you will probably be given additional tests that may require some preparation.
