PSA levels may be above the baseline for various reasons other than prostate cancer.

Other factors that can raise PSA levels include:

  • older age
  • an enlarged prostate â because of benign prostatic hyperplasia , for example

In addition, some medications may reduce PSA levels, including:

High PSA levels alone do not indicate cancer. However, if a DRE also reveals changes, a doctor may recommend a biopsy for a more accurate result.

Prostate Exams: Do You Really Need One

Prostate cancer, although the second most common cancer in men in the United States, is extremely treatable when it is detected in its early stages.

Recently, there has been debates in the medical community about whether or not the benefits of a prostate exam outweigh the risks. We asked Jeffery Spencer, MD, a Urologist at Finger Lakes Urology Institute for his expert opinion on prostate exams and prostate cancer screenings.

What Is A Prostate Cancer Screening Like

A prostate cancer screening can be conducted in one of two ways. The first, a PSA test, is a simple blood draw. The second is a brief rectal exam that takes less than 30 seconds to perform.

“For a screening, if a patient comes and asks for a prostate cancer screening, it begins with a blood test,” said Ehdaie. “It’s a small vial of blood, and then a medical history and physical examination. In the physical examination there will be a digital rectal examination in which the physician’s finger is inserted into the rectum to feel the prostate.”

Should You Know Your Psa Level

Instead of a national screening programme, there is an informed choice programme, called prostate cancer risk management, for healthy men aged 50 or over who ask their GP about PSA testing. It aims to give men good information on the pros and cons of a PSA test.

If you’re a man aged 50 or over and decide to have your PSA levels tested after talking to your GP, they can arrange for it to be carried out free on the NHS.

If results show you have a raised level of PSA, your GP may suggest further tests.

Unfortunately, many men neglect the task of having periodic examinations of the prostate. Often, this is due to a misunderstanding about the mechanics of the exam, as well as a lack of information about prostate cancer and what the disease can do if left unchecked.

Prostate exams do not require much, other than a visit to the family physician or urologist. The exam takes place in the doctors office, and involves nothing more than a lubricant and a rubber glove. The physician dons the glove, lubricates the index finger, and inserts the finger into the anus. The prostate can be easily examined through the thin walls of the rectum, allowing the physician to check for growths, texture, and any other abnormalities. Generally, the examination takes only a moment or two and is relatively painless.

How Do Men Feel About The Rectal Exam

I think most men are willing to do it, but some men are exceptionally opposed to it. They might be happy to hear that there are studies ongoing in the United Kingdom looking at using magnetic resonance imaging as a screening tool to be able to avoid doing a digital rectal exam in the future. There is a large African immigrant population in London with a higher incidence of prostate cancerthey didnt want to have the prostate exam because they had a lot of opposition to it. They were uncomfortable with it.

Can A Swollen Prostate If Untreated Cause Prostate Cancer

We have no evidence of that. Theyre completely unrelated problems. The area of the prostate that causes urinary symptoms is usually a different part of the prostate than where cancer is likeliest to develop.

Doctors divide the prostate into different zones. The zone that is associated with BPHand the majority of prostate growthis the transition zone. Prostate cancer occurs there much less often than in the peripheral zone, which is the outer area.

If your PSA is not normal, you will probably have a biopsy. The doctor puts a needle through the wall of the rectum and into the prostate to take a few samples. Biopsies can be painful and cause bleeding. Men can get serious infections from biopsies, and they may need hospital care.

Surgery or radiation are the usual treatments for prostate cancer. They can do more harm than good. Treatment can cause serious complications, such as heart attacks, blood clots in the legs or lungs, or even death. In addition, 40 men out of 1,000 will become impotent or incontinent from treatment.

When To Startand Stopscreening

The doctors and researchers who recommend screening argue that cases of prostate cancer found very early can be cured more quickly, with less chance of relapse or spread. Those who recommend against routine screening point to the slow-moving nature of prostate cancer and the side effects of surgical and medical treatment, which can be considerable.

The introduction of PSA screening in the US led to an initial increase in the number of prostate cancer cases diagnosed each year, even though many of these new cases were non-aggressive or low-risk prostate cancer. The issue was not that screening was harmful, it was that many of these low-risk cancers did not necessarily need immediate treatment. It seems strange to say that a patient might be better off leaving cancer untreated, but in some cases, it can be true. For a few years, the United States Preventative Services Task Force recommended against PSA screening. We are now seeing more cases of advanced prostate cancer diagnosed in recent years. This may be a long-tail effect of that USPSTF recommendation. It has now been changed to note that for men aged 55 to 69 years, the decision to undergo PSA screening is an individual one and should be discussed with your doctor. USPSTF continues to recommend against screening for men aged 70 and over.

What to Expect from a Prostate Exam

Starting at age 50, all men should discuss getting a prostate exam with their doctor.

The reason for this is prostate cancer. In the UK, about one in eight men will be diagnosed with this in their lifetime. It mainly affects men aged 50 plus, but your risk increases as you get older, and the most common age to be diagnosed is between 65 and 69 years. Most men with early prostate cancer dont have any noticeable signs or symptoms.

The exception to this rule is if you are experiencing symptoms, or if your genetics predispose you as higher risk. Doctors are increasingly finding the tendency towards some prostate cancers can be inherited from your fathers family. Additionally, black men are at a higher risk, with one in four getting prostate cancer in their lifetime.

If youre experiencing no symptoms, heres the recommended age for prostate exam:

  • If you have a family history, first prostate exam at age 40
  • If you are black, first prostate exam at age 45
  • If you have no family history and youre not black, first prostate exam at age 50

What Is A Digital Rectal Exam

The most common way for doctors to check on the health of your prostate is with a DRE. Its a fairly quick and simple procedure.

For the exam, youll bend at the waist while standing or lie on your side with your knees bent toward your chest.

Your doctor will lubricate a gloved finger and gently place it inside your rectum. Theyll press one hand on your prostate, and their other hand will feel your pelvic area. It should only take a few moments.

You may experience momentary discomfort. You may also feel the urge to urinate, especially if your prostate is enlarged or inflamed.

Your doctor will be able to tell you if your prostate seems to be a normal size and shape. In general, a DRE has no risks.

When To Get A Prostate Cancer Screening

A prostate screening can help your doctor find prostate cancer early, but youll need to decide if the benefits of the exam outweigh the risks. Have a discussion with your doctor about prostate cancer screenings.

The U.S. Preventive Services Task Force now recommends that men ages 55 to 69 decide for themselves whether to undergo a prostate-specific antigen screening test, after talking it over with their doctor.

They recommend against screening for men at or above the age of 70.

The American Cancer Society strongly recommends that no one be screened without discussion of the uncertainties, risks, and potential benefits of prostate cancer screening.

They give these specific recommendations for the date at which these discussions with a healthcare provider should take place:

  • Age 50 for men who are at average risk of prostate cancer and are expected to live at least 10 more years.
  • Age 45 for men at high risk of developing prostate cancer. This includes African Americans and men who have a first-degree relative diagnosed with prostate cancer at an early age .
  • Age 40 for men at even higher risk .

You should also speak with your doctor about a prostate exam if youre experiencing symptoms of a prostate problem, such as frequent or painful urination or blood in your urine.

After this discussion, if you decide to get a prostate cancer screening, the ACS and the American Urologic Association recommend getting a prostate-specific antigen blood test.

Why Should I Get Both Prostate Cancer Screenings

Both screenings provide valuable information about your health that can detect prostate cancer, even at an early stage. One result without the other may not provide enough indication of whether or not treatment is necessary.

There are many reasons why a mans prostate may be enlarged, and prostate cancer is only one. When paired with a PSA blood test, results can better indicate whether further testing is needed to determine a diagnosis.
