What Is An Artificial Urinary Sphincter And How Does It Help With Urinary Incontinence

What Is a Normal PSA for a Man Without Prostate Cancer? | Ask a Prostate Expert, Mark Scholz, MD

An artificial urinary sphincter can help men who have moderate to severe urinary incontinence due to poorly functioning muscle or sphincter valve after prostate cancer surgery.

The AUS has three parts:

  • An inflatable cuff that is placed around the upper urethra. The cuff closes off the urethra to prevent leakage of urine.
  • A pump that is inserted into the scrotum. It’s completely on the inside and not visible, and the pump controls the opening and closing of the cuff.
  • A small pressure-regulating balloon that is placed in the abdomen, under the muscles. The balloon maintains fluid under pressure within the urethral cuff to pressurize the system and hold urine back.

If you have this surgery, youll press on the pump when you feel the need to pee. This opens the cuff to allow urine to pass. When youre done peeing, the cuff automatically closes again on its own.

The AUS procedure provides a very good and satisfactory result in 90% of cases. Risks are uncommon and include:

  • Failure of the device .
  • Erosion of the cuff into the urethra.
  • Infection.

All of these would require additional surgery.

Other Prostate Removal Side Effects Include:

  • Changes in orgasm
  • Loss of fertility
  • Lymphedema swelling in one or both legs after the removal of lymph nodes during prostate removal
  • Change in penis length
  • Inguinal hernia a bulging of fatty tissue or a loop of the small intestine protrudes through a weak area of your groin

Life after prostate cancer can be as fulfilling as before. Always remember that other men have walked down the same path and faced similar obstacles. Side effects dont have to rob you of your quality of life. Advocate for yourself. Ask questions. Seek help.

About Half Of Men Older Than 50 Have An Enlarged Prostate Here Are Some Of The Basic Facts You Need To Know About This Common Condition

As men age, many experience prostate gland enlargement. This condition is known as benign prostatic hyperplasia .

The prostate gland surrounds the urethra, the hollow tube that carries urine out of the body. When the prostate gets bigger, it can squeeze or partially block the urethra, which leads to problems urinating.

BPH is quite common in older men. In fact, the condition impacts about 50% of men between the ages of 51 and 60. For men 80 and older, the prevalence of BPH is approximately 90%, according to the National Institute of Diabetes and Digestive and Kidney Diseases.

Where Did 21 Times A Month Come From

A Daily Mail headline from 2017 reads, Ejaculating at least 21 times a month significantly reduces a mans risk of prostate cancer.

The article details the results of a study of 31,925 men published in the December 2016 issue of European Urology.

Although the studys findings do suggest that theres a direct relationship between ejaculation frequency and prostate cancer risk, additional research is needed to fully explore this possibility.

The study in question relied on self-reported answers once in 1992 and once in 2010 about how often they ejaculated each month and whether they developed prostate cancer.

This means that the results could be skewed by the subjects memories or awareness of their habits.

Its also important to note that the study didnt specify whether ejaculation resulted from sex with a partner or masturbation. The reason for emission may play a role in any potential benefits.

    Problems With Enlarged Prostate Gland

Benign enlargement of the prostate gland is more common as men get older. It can cause troublesome symptoms, although it doesnt always.

The urethra passes through the prostate gland, so men may have problems urinating if the enlarged gland restricts the flow of urine. If the flow stops completely, a catheter is required to empty the bladder. It is rare for this form of acute urinary retention to cause kidney damage.

    An enlarged prostate doesnt always cause urinary problems. Studies indicate that the size of a mans prostate gland has little influence on the type or severity of his urination problems. BPH is just one possible cause of urinary symptoms.;

Another cause of urinary symptoms can be changes to the muscular wall of the bladder, which may cause spasms of the bladder or weaken the bladder, causing problems passing urine.

How May Erectile Dysfunction Affect My Sex Life

Most men find that their sex life is different after prostate cancer treatment. Some men question their manliness when they cannot have an erection or find that they are not interested in sex. This can happen even if you are not currently in an intimate relationship. You may find this upsetting. Even if one of the medications or erection aids is helpful, having sex using these things may take some getting used to. It may not feel entirely natural. You can talk with your doctor or healthcare team about these feelings. Counseling may also help.

If you have an intimate partner, it is important for you to talk to your partner about how you are feeling. There is an old saying that a problem shared is a problem halved. Not everyone wants a sexual relationship. Dont try to guess or assume what your partner wants. Have an open and honest discussion with your partner.

This may seem unnecessary in long-term relationships as people tend to assume they know all there is to know about their partner but this is not always the case. With time, you and your partner may be able to find satisfying ways to have a sex life even though you have erectile dysfunction. Your partner will also have concerns about your sex life as well as concerns about your health. Talking about your feelings is very important during this time.

    The Prostate Produces Seminal Fluid

Semen is produced by both the prostate gland and the seminal vesicles, a pair of glands attached to the prostate.

The fluid produced by the prostate gland mixes with the sperm and produces semen. During sex, the muscles of the prostate contract and ejaculate the prostate fluid and sperm into the urethra.

Hormone Adjustments Can Help With Orgasm

Sometimes hormone levels contribute to orgasmic problem. Many men with low testosterone have problems with orgasm, but treating prostate cancer patients with testosterone is still very controversial. Other hormones can also play a role. For example, a low thyroid hormone level or a high prolactin level can make it more difficult to reach orgasm. Even if you have a normal hormone levels, there are a few hormonal medications that may be able to help.

Another hormone that plays a role in orgasm is oxytocin. The level of oxytocin increases in both men and women during sexual arousal. Taking an under-the-tongue formulation of oxytocin five to 10 minutes before sexual activity can help some men achieve orgasm. Its safe and no side effects have been reported, says Dr. Kacker. You need a prescription for oxytocin, and its available only through special compounding pharmacies, not through retail pharmacies.

Cabergoline is another medication that can help with orgasm problems. It blocks the release of prolactin, a hormone that appears to play an important role in the refractory period after orgasm when men cant have another orgasm for a while.

Is There Any Reason To Control Your Ejaculation Frequency

Discover How Many Nigerian Have Been Able To Shrink Their ...

Theres an old Taoist belief that controlling how often you ejaculate helps you preserve whats believed to be a finite amount of energy. Abstaining from ejaculation is thought to allow the energy contained in sperm to return to the brain and supply it with energy.

This practice is the origin of the 24 times a year idea. In fact, some Taoist teachers recommend that you only ejaculate 20 to 30 percent of the times you have sex. That translates to 2 or 3 times out of every 10 sessions.

But these ideas arent backed by any hard science. And many Taoists teachers urge people to focus on personal feelings of strength and refreshment after ejaculation rather than specific figures.

Life After A Radical Prostatectomy: A Real

    Reposted from Greater Boston Urology.

In October of 2013, Les Cavicchi underwent a;radical prostatectomythe removal of the prostate gland.;Les had been diagnosed with prostate cancer;a month earlier after undergoing a TRUS biopsy.

Less cancer was localized, meaning it was contained within the prostate gland itself. He didnt need further treatment for prostate cancer after the surgery, only regular monitoring of his PSA.

However, Les did suffer from common side effects associated with radical prostatectomies, specifically;urinary incontinence;and;erectile dysfunction.

Q: So lets get right to it: You were dealing with incontinence after your radical prostatectomy?Les:;The thing about any kind of prostate procedure is youve got to deal with incontinence. This was a big issue for me for a while. I had purchased padded undergarmentsa whole case of them, because I didnt know how long this was going to last.

Men wonder why this particular side effect happens. The prostate gland is the primary organ for controlling urinary flow in normal physiology. When you are rising frequently during the night, and/or having difficulty starting urination, these are signs that the prostate is beginning to grow in size and interfere with this specific function. Every mans prostate gets larger with age; it does not necessarily have anything to do with having cancer. It is a separate reality.

Women dont have a prostate. What do they use? Kegel muscles!

    How Do You Find It

Youll need to head south along the back road to find it in a magical place called the anal region.

Although entering through the anus is the most direct way, you can also stimulate the prostate indirectly through the perineum, or taint.

This is the skin that runs underneath the scrotum to the anus.

Sex After Prostate Surgery And Achieving Orgasm

Sex after prostatectomy is an important concern for most men. The total removal of the cancerous tissues is the primary goal, but the quality of life after prostate surgery is also important. Worrying about ED makes men be nervous and anxious when thinking about undergoing radical prostatectomy.

However, whilst most patients focus on erectile dysfunction, orgasm is somehow under-considered. But is it possible to achieve orgasm after prostatectomy? Erections and orgasm are equally important for a healthy sex life.

The good news is that sex after prostate surgery is very possible and enjoyable for most men. This is due to the newest robotic technologies that are nerve-sparing and preserve the sexual function.

Dr. Samadis robotic prostate surgery, SMART Surgery, was explicitly designed to spare the tiny nerve bundles surrounding the prostate in order to preserve sexual potency.

Men who undergo Dr. Samadis robotic prostate surgery have a reasonable chance of regaining complete erectile function for sex after prostate surgery.

What Happens After Urethral Sling Surgery

How to Cure Enlarged Prostate without Surgery – Natural BPH Treatment

Patients usually recover from this surgery quickly. It’s best to limit demanding activities for approximately six weeks after surgery to avoid having the sling loosen before healing is complete.

After surgery, there may be swelling that makes it difficult to urinate. You’ll have a catheter coming out of the urethra for two to three days. After this, the catheter is removed and just about everyone is able to void on their own. In rare cases, the catheter may have to be reinserted for a few days or up to a week if you cannot urinate.

After the swelling goes down, youll gradually be able to urinate on your own and empty your bladder. However, your normal urination pattern may not return for a few weeks.

The majority of men who’ve had this procedure have been cured of their urinary incontinence and no longer use pads. The others usually improve to the point where they don’t use as many pads as they were before.

    How To Explore Prostate Orgasm

Prostate orgasm can be explored either solo or with a partner.

If youâre trying it for the first time, you may want to begin with stimulation of the perineum. As you move backwards towards the anus, you will know youâve hit the right spot when the area under your skin feels softer and you feel an increase in sensation. You may also feel like you need to pee at the same time.

If you want to try internal stimulation, start by inserting a well lubricated finger into your rectum. Be gentle. The prostate will feel fleshier and softer than the surrounding area.

You can also use sex toys such as vibrators and plugs in order to achieve prostate orgasm. They may be used either internally or externally.

    What Is Advanced Prostate Cancer

    Prostate Cancer or Not

When prostate cancer spreads beyond the prostate or returns after treatment, it is often called advanced prostate cancer.

Prostate cancer is often grouped into four stages.

    • Stages I & II: The tumor has not spread beyond the prostate. This is often called early stage or localized prostate cancer.
    • Stage III: Cancer has spread outside the prostate, but only to nearby tissues. This is often called locally advanced prostate cancer.
    • Stage IV: Cancer has spread outside the prostate to other parts such as the lymph nodes, bones, liver or lungs. This stage is often called advanced prostate cancer.

When an early stage prostate cancer is found, it may be treated or placed on surveillance . If prostate cancer spreads beyond the prostate or returns after treatment, it is often called advanced prostate cancer. Stage IV prostate cancer is not curable, but there are many ways to control it. Treatment can stop advanced prostate cancer from growing and causing symptoms.

There are several types of advanced prostate cancer, including:

    Biochemical Recurrence

If your Prostate Specific Antigen level has risen after the first treatment but you have no other signs of cancer, you have “biochemical recurrence.”

    Castration-Resistant Prostate Cancer

    Non-Metastatic Castration-Resistant Prostate Cancer

    Metastatic Prostate Cancer

    • Lymph nodes outside the pelvis
    • Bones
    • Other organs
