Limitations Of The Review
Low levels of evidence in this review were mainly caused by the poor quality and small sample size of original included trials. The inconsistency of findings of herbal medicines effect on improving main symptoms of prostate cancer among these trials further reduced the internal validity of the evidence.
Although we searched both Chinese and English databases, all of the included trials were retrieved from Chinese literature, which may have introduced potential selection bias and limited the external generalization of the evidence.
Types of clinical stages of prostate cancer determine the details of treatment. In this review, almost half of the included trials did not report or classify clinical stages of the participants, which made the subgroup analysis regarding to the different clinical stages of the disease unavailable.

Implications For The Future Research
First of all, the protocol of the clinical trial should be pre-defined according to the research question. Ideally, preclinical data should be provided to support the design of the clinical trials. Authors are encouraged to register their study protocols before trial implementation to ensure the research can be conducted according to the pre-defined standard. In the protocol, the primary and secondary outcomes should be defined for the research. While one can argue whether PSA is a good marker for prostate cancer, it is the only approved marker to indicate the development of prostate cancer, and testing of serum PSA has been standardized. Therefore, PSA values should be included as a parameter to evaluate the outcome of prostate cancer trials. If possible, PSA doubling time during treatments should be determined. A documentation of a decrease of PSA could be more informative than a single measurement of PSA value. Secondly, there should be unified scales for assessing the quality of life of patients with prostate cancer . Therefore, those scales should be used and interpreted appropriately. Furthermore, in order to observe the long-term efficacy of treatment with assessment of patients survival time or time to progression, follow-up with adequate duration should be considered during clinical study.

Surgical procedures to remove the diseased prostate are usually necessary. Surgical procedures are not always necessary. If the disease is caused by bacterial infections, a doctor can treat the symptoms using alpha-blockers or surgery. Physical therapy, relaxation exercises, and warm baths are all recommended. A physician may also prescribe antibiotics to cure the infection. A bacterial infection can also cause a recurrence of the condition.

Results Of The Search
The search of 6 databases identified 1657 citations for further evaluation. Full texts of 109 articles were read, and 18 trials met our inclusion criteria. However, one of the trial tested a herbal capsule PC-SPES , which contains Ganoderma lucidum, Scutellaria baicalensis, Rabdosia rubescens, Isatis indigotica, Dendranthema morifolium, Seronoa repens, Panax pseudoginseng, and Glycyrrhiza uralensis. This herbal product was reported to be contaminated with diethylstilbestrol , thus the product may have a similar effect with diethylstilbestrol on inhibiting growth of both androgen-sensitive and androgen-independent prostate cancer cells. Due to the massive controversy of potential contamination of PC-SPES, the drug is no longer available in the U.S. Since it is hard to define whether it still belongs to herbal product, we excluded the trial from this review. Thus, finally 17 trials were included in the review. Details of the study flow were shown in Fig 1.
Serum testosterone. Only one trial reported the change of serum testosterone after treatment, the result showed no difference between a combination of herbal decoction and MAB and MAB alone for this outcome after six months of the treatment.
Assessment for publication bias.
Since none of the meta-analysis in this review included more than 10 studies, funnel plot was not conducted to assess the potential publication bias.

Traditional Medical Practitioners Engaged In The Management Of Prostate Diseases
In Ghana, it is estimated that the ratio of traditional medical practitioners to the population is 1: 400.35,36 This group is a heterogeneous one with different categories of healers. They include bonesetters and herbalists who specialize in the use of herbs and plants, with or without fetish in treating patients.
It has been observed that each village has its traditional practitioner who uses local plant remedies and in many cases rituals as well, to achieve healing for the population.
The TMPs have been noted to play a very important role in health delivery in the rural areas where there is less access to orthodox facilities with 70 80% of Ghanaians using traditional medicines.18,19 Its importance in Ghana is attributed to the fact that a number of Ghanaians believe that some diseases are not curable with orthodox western medicines.
The combination of mystical powers and the herbs is believed to offer a more lasting solution or cure for some of these diseases. Formal training in the use of traditional herbal medicines has been institutionalized in some of the tertiary institutions in Ghana. These graduates have also set up various scientific herbal clinics that are apparently using more standardized remedies in their traditional medicine practice. This group has been noted not only to manage the primary health care diseases but also tertiary care ones such as hypertension, diabetes mellitus and prostate diseases.
Agreement And Disagreement With Other Studies
We found one review assessed the complementary and alternative medicine for prostate cancer, in which herbal preparations were involved. However, only PC-SPES and Zyflamend were mentioned in that review, and no data from a complete clinical trial could be used for evaluating those herbal preparations. The author demonstrated that the available data provided insufficient evidence to support the use of CAMs for the treatment of prostate cancer patients.
Compared to the above review, our study included 17 trials with different types of herbal preparations. Though the strength of the evidence was low, we found potential effect of oral herbal preparations for patients with prostate cancer on some key symptoms improvement.

Introduction
The prostate is one of the glands in the reproductive system of men, and its lesions and diseases are of great prevalence and importance . The prostate is a small gland under the bladder that covers the upper part of the urethra. In developed countries, prostate cancer is the second most common cancer and the second deadliest cancer in men . Due to the importance of prostate diseases, especially carcinoma, and laboratory research to find a drug or a new method in the treatment of endocrine disorders, has a lot of value. On the other hand, common treatments for prostate carcinoma are currently associated with a variety of limitations, failures, and side effects . Due to the high cost of treatment and side effects of chemical drugs, it is very important to find herbal medicines that are less expensive, have side effects, and can be antioxidant activity.

Rye Grass Pollen Extract
Rye grass pollen extract is made from three types of grass pollen: rye, timothy, and corn. A review of herbal studies published in BJU International found that in one study, men who took rye grass pollen extract reported an improvement in their symptoms compared to those who were taking a placebo. However, this study lasted only 6 months. It didnt look at how well the supplement worked compared to prescription medications.
Chinese Medicine Formulas In The Treatment Of Prostate Cancer
PC-SPES is a Chinese medicine formula containing eight herbal medicines. It was launched into the market as a health care product in 1996 by Botanic Lab . The New England Journal of Medicine first reported the biological activity and the clinical effect of PC-SPES in 1998 . A series of reports showed that PC-SPES can significantly reduce serum testosterone and prostate-specific antigen in patients with prostate cancer in a time- and dose-dependent manner . However, in 2001, patients taking PC-SPES showed diffuse intravascular coagulation and bleeding tendency . In 2002, some batches of PC-SPES were found to contain diethylstilbestrol, warfarin, and indomethacin, which made the efficacy and side effects of PC-SPES controversial . Considering the stability and safety of Chinese herbal remedies, National Center for Complementary and Alternative Medicine suspended research on PC-SPES and its related compounds and withdrew it from the USA market in February 2000 .
From 2010 to 2012, Lin et al. reported a series of clinical data on prostate cancer treated with Chinese medicines. Firstly, they found that traditional Chinese medicine treatment was only a complementary treatment rather than an alternative treatment. Then, they found that TCM treatment showed an increasing trend in popularity among prostate cancer patients in Taiwan. Later, they confirmed that TCM treatment was popular among prostate cancer patients in Taiwan .
