Mayo Clinic Researchers Identify Drug Resistance Factors For Advanced Prostate Cancer
Newswise ROCHESTER, Minn. In a new study published in Molecular Cancer Research, Mayo Clinic researchers identified critical genomic changes in response to abiraterone acetate/prednisone, a standard treatment option for men with progressive, incurable and castration-resistant prostate cancer.
“We defined a potential strategy for both responders and nonresponders of the drug that may help men overcome resistance and prolong survival,” says Liewei Wang, M.D., Ph.D., the Bernard and Edith Waterman Director, Pharmacogenomics Program, Mayo Clinic’s Center for Individualized Medicine. Dr. Wang is the corresponding author of the study.
Dr. Wang explains that while several drug choices are available to control disease progression, many questions remain over which drugs to use in individual cases. Also, predictive biomarkers for drug resistance and sensitivity remain primarily unknown.
Abiraterone acetate is a standard treatment option for men with castration-resistant prostate cancer. However, the response rate is limited, no known biomarkers predict prognosis, and alternative therapies for those who failed treatment are unavailable.
Dr. Sicotte says biomarkers based on the stage-specific landscape of genomic changes in prostate cancer are under investigation.

New Therapy May Fight Prostate Cancer
Researchers Say Experimental Treatment Is Effective for Advanced Prostate Cancer
June 19, 2009 — Mayo Clinic researchers say an experimental treatment may have cured two patients whose prostate cancers were so advanced they had been considered inoperable.
Both patients were reportedly free of cancer after treatment with a combination of hormone therapy, an experimental immunotherapy, and surgery.
No one was more surprised than their doctors. “This is certainly not the way we thought it would go,” Mayo urologist and immunologist Eugene Kwon, MD, tells WebMD.
He adds that the point of the treatment had been to buy some time for patients whose cancers appeared to be incurable.
The two men were originally enrolled in a study designed to determine if treatment to suppress testosterone , followed by treatment with an experimental immunotherapy called ipilimumab, could slow the progression of advanced prostate cancer.
“The goal was to see if we could modestly improve upon current treatments,” Kwon says.
The 85 patients in the study were not considered candidates for surgery, but several showed such dramatic regressions in their cancers that they left the trial in order to have it.
Kwon admits that this was done in the first patient not because the study investigators thought it was a good idea, but because the patient’s wife insisted.
More than a year and a half later, that patient shows no signs of prostate cancer, Kwon says.

What Galleri Could Mean For Cancer Patients
Cancer is projected to become the worlds leading cause of death this year.
While the cancer death rate in the United States has declined, more than 600,000 Americans will die from cancer in 2021, according to the American Cancer Society .
This is largely because the majority of cancers are found at later stages when treatment may be more difficult.

We are still diagnosing the disease far too late, when its in its metastatic state. Recommended screening tests save lives, but they only cover five cancer types and screen for a single cancer at a time.
Ofman said that the cancers responsible for 71 percent of cancer deaths have no recommended early detection screening. By adding Galleri to existing screening tests, Ofman noted, theres the potential to reduce late-stage cancer diagnoses by two-thirds.
This would translate to a reduction of 5-year cancer deaths by 39 percent among those detected earlier, equating to an overall reduction of all 5-year cancer deaths by 26 percent, he said.
Mayo Clinic Discovers Biological Markers That Could Guide Treatment For Prostate Cancer
Genetic alterations in low-risk prostate cancer diagnosed by needle biopsy can identify men who harbor higher-risk cancer in their prostate glands, Mayo Clinic has discovered. The research, which is published in the January edition of Mayo Clinic Proceedings, found for the first time that genetic alterations associated with intermediate- and high-risk prostate cancer also may be present in some cases of low-risk prostate cancers.
The study found the needle biopsy procedure may miss higher-risk cancer that increases the risk of disease progression. Researchers say that men diagnosed with low-risk cancer may benefit from additional testing for these chromosomal alterations.
We have discovered new molecular markers that can help guide men in their decisions about the course of their prostate cancer care, says George Vasmatzis, Ph.D., Co-Director of the Center for Individualized Medicine Biomarker Discovery Program and lead author on the study. Overtreatment has been an issue for the group of men that our study targets. We found that the presence of genetic alterations in low-risk cancer can help men decide whether treatment or active surveillance is right for them.
Prostate cancer is assessed by Gleason patterns and a score that indicate grade. The Gleason patterns are strongly associated with risk of disease progression. Gleason pattern 3 prostate cancer is considered to be low-risk. Gleason patterns 4 and 5 cancer carry a higher risk of aggressive behavior.

ANSWER: I’m sorry to hear that your husband is dealing with prostate cancer, which is the most common cancer in American men. Approximately 192,000 men will be diagnosed with prostate cancer in 2020, according to the American Cancer Society.
Prostate cancer is more likely to develop in older men, typically those over 60, and it is more common in African American men. Given advances in care, most men will not die from prostate cancer.
There are many options for treating prostate cancer, depending on the grade and stage of the cancer. In recent years, the guidelines for treating prostate cancer have changed. Also, technological advances in imaging and treatment have made it possible for some men to avoid radical surgery that may come with adverse side effects. In recent years, surgeons are finding that ablation is a viable alternative to traditional surgery.
The ability to control the application of the heat or cold, and get the energy to stay within an intended boundary and not spread even 1 or 2 millimeters beyond, has proven difficult.
