What Does 4kscore Measure
The 4Kscore takes the results of four blood tests and factors in a mans age, DRE results, and if he had a prior biopsy. A number is created. It reflects the percentage likelihood that if that patient had a biopsy, an aggressive prostate cancer would be found.
The tests included in a 4Kscore assessment are measures of Total PSA , Free PSA , Intact PSA and of Human Kallikrein 2 a genetic cousin to PSA.
Depending on the result, a low-risk patient can continue to have his PSA results monitored, without proceeding to biopsy or treatment. A high-risk patient can choose to have a biopsy performed. The results of the biopsy then help guide and direct the most appropriate treatment plan.
Dr. Kryvenko offers that the 4Kscore test has also been shown to predict the presence of aggressive cancer cells not seen in a biopsy.
As pathologists, we interpret what we see under a microscope as best as we can. But sometimes, there are things we cannot see or predict. The 4Kscore test can tell us if a man may have aggressive prostate cancer cells growing somewhere else, but that have not yet surfaced. It is data, not human interpretation.
Biomarkers Aid Tumor Detection
The 4Kscore and SelectMDx tests are validated to predict detection of significant prostate cancer on biopsy . The 4Kscore measures four prostate-specific biomarkers: total PSA, free PSA, intact PSA, and the protein kallikrein 2, which has been shown to be a marker for prostate cancer. SelectMDx is a urine test that measures the expression of two genetic markers for prostate cancer . Both scores also consider other factors, such as the patients age, family history, and prior biopsy results.
Several previous studies have looked at the tests individually and reported comparable sensitivity and specificity for detecting clinically meaningful prostate cancer, the NYU Langone investigators note. However, before this study there have been no data directly comparing performance of the two tests.
Each of these new tests aim to fine-tune the diagnostic decision associated with an elevated PSA, namely whether or not to proceed with prostate biopsy, says the studys lead author, James S. Wysock, MD, MS, a urologic oncologist at the Smilow Comprehensive Prostate Cancer Center and assistant professor in the Department of Urology at NYU Langone. A strength of our study is that all patients underwent both types of tests as well as MRI before biopsy.
What Is The Role Of The 4kscore Test In Prostate Cancer Screening
The 4Kscore test helps clarify the biopsy decision-making process by determining a patient-specific probability for finding aggressive, Gleason score 7 or higher prostate cancer upon biopsy. These are the aggressive prostate cancers that always require medical treatment or intervention. The blood test results are combined in an algorithm with patient age, digital rectal exam findings , and prior negative biopsy to give physicians a personalized score for individual patients. The 4Kscore test predicts the risk percent score from less than 1% to greater than 95% of a man having aggressive cancer in a prospective biopsy.

Study Confirms 4kscore Accurately Predicts High
The 4Kscore blood test has been shown to accurately detect the presence of high-grade prostate cancer, according to a study presented at ASCO GU.
A blood test called the 4Kscore results in accurate detection of high-grade prostate cancer. In a prospective study of 1,012 men, this test more accurately predicted the presence of high-grade disease compared with a commonly used risk calculator in men with elevated prostate-specific antigen levels.
The results from this study were presented by Sanoj Punnen, MD, a urologic oncologist at the University of Miami in Florida at the 2015 Genitourinary Cancers Symposium.
In this study, men scheduled for a prostate biopsy were enrolled, regardless of their PSA level or clinical findings, at 26 centers across the United States between October 2013 and April 2014. A total of 231 high-grade prostate cancers were detected. The 4Kscore showed a higher net benefit in comparison with the PCPTRC at all threshold possibilities for high-grade disease used in clinical practice.
The test is able to detect aggressive prostate cancer with high accuracy, Punnen told Cancer Network. The area under the curve is better than any other biomarker in this area. There was a significant reduction in biopsies that could be attained if we used this test, he said.
Transrectal Ultrasound With Prostate Biopsy
Transrectal ultrasound is most often used to examine the prostate. In a transrectal ultrasound, the health care provider inserts a transducer slightly larger than a pen into the mans rectum next to the prostate. The ultrasound image shows the size of the prostate and any abnormal-looking areas, such as tumors. Transrectal ultrasound cannot definitively identify prostate cancer.
To determine whether a tumor is cancerous, the health care provider uses the transducer and ultrasound images to guide a needle to the tumor. The needle is then used to remove a few pieces of prostate tissue for examination with a microscope. This process, called biopsy, can reveal whether prostate cancer is present. A transrectal ultrasound with prostate biopsy is usually performed by a doctor in a health care providers office, outpatient center, or hospital with light sedation and local anesthesia. The biopsied prostate tissue is examined in a laboratory by a pathologista doctor who specializes in diagnosing diseases.

The results of other medical tests such as PSA blood test and prostate tissue biopsy may take several days to come back. Which Men Need The Test
All men, according to Drs. Kryvenko and Punnen, should continue to follow national guidelines for prostate cancer screening, based on age, race, family history and other risk factors. The digital rectal exam and the PSA test remain the first two steps of screening.
The 4Kscore test is an option after the DRE and PSA tests, says Dr. Punnen. If after PSA blood testing, your doctor says that a surgical biopsy may be needed to check for prostate cancer, that is when a 4Kscore test may help.

What Is The Psa Test
Prostate-specific antigen, or PSA, is a protein produced by normal, as well as malignant, cells of the prostate gland. The PSA test measures the level of PSA in the blood. For this test, a blood sample is sent to a laboratory for analysis. The results are usually reported as nanograms of PSA per milliliter of blood.
The blood level of PSA is often elevated in people with prostate cancer, and the PSA test was originally approved by the FDA in 1986 to monitor the progression of prostate cancer in men who had already been diagnosed with the disease. In 1994, FDA approved the PSA test to be used in conjunction with a digital rectal exam to aid in the detection of prostate cancer in men 50 years and older. Until about 2008, many doctors and professional organizations had encouraged yearly PSA screening for prostate cancer beginning at age 50.
PSA testing is also often used by health care providers for individuals who report prostate symptoms to help determine the nature of the problem.

Select MDx is a biomarker that can be obtained prior to initial biopsy and after negative biopsy. At present, no test has been established to be superior to another. Prior to having such studies done, it is advisable to ensure that your insurance company covers these tests.

How Is The Psa Test Used In Men Who Have Been Treated For Prostate Cancer
The PSA test is used to monitor men after surgery or radiation therapy for prostate cancer to see if their cancer has recurred . If a mans PSA level begins to rise after prostate cancer treatment, it may be the first sign of a recurrence. Such a biochemical relapse typically appears months or years before the recurrence causes symptoms.
However, a single elevated PSA measurement in someone who has a history of prostate cancer does not always mean that the cancer has come back. Someone who has been treated for prostate cancer should discuss an elevated PSA level with their doctor. The doctor may recommend repeating the PSA test or performing other tests to check for evidence of a recurrence. The doctor may look for a trend of rising PSA level over time rather than a single elevated PSA level.

A recent NYU Langone study, led by urologic oncologist Dr. James S. Wysock, demonstrated that further head-to-head biomarker studies are required to better guide clinical decision-making for men with elevated PSA levels.
Prostate-specific antigen screening has helped propel a 50 percent decline in prostate cancer mortality over the past 20 years. However, universal routine PSA testing is no longer recommended because, used in isolation, the test often triggers unnecessary biopsies, over-diagnosis, and potentially unwarranted treatment of low-risk, indolent disease.
As a result, clinicians now employ risk stratification tools such as prostate MRI and novel biomarkers in conjunction with PSA screening to inform decisions about biopsies. In a recent study published in Urology, Department of Urology researchers at NYU Langones Perlmutter Cancer Center and NYU Grossman School of Medicine assessed the accuracy of two commonly used tests4Kscore and SelectMDxin predicting detection of clinically significant disease in men with elevated PSA levels.
The investigators found that the two tests often yield conflicting guidance on whether to proceed with biopsy. However, the 4Kscore performs significantly better than SelectMDx when combined with MRI. Although further comparative studies are needed, the findings suggest that the 4Kscore may be a better option to guide clinical decision-making.

What Happens If You Have A Likert Score Of 1 Or 2
Your doctor may not recommend a biopsy if you have a low Likert score. But you can still have one if you want to. Your doctor will explain the possible benefits and risks of having a biopsy.
When recommending whether you need a biopsy or not, your doctor also looks at other factors. These include:
- other test results such as PSA level and prostate examination
- the size of your prostate and the corrected PSA for its size. This is called the PSA density
- any other health conditions that you might have
- how well youd cope with prostate cancer treatment and whether it would benefit you
They might decide not to recommend a biopsy if you’re unwell or not likely to be able to have treatment.
If you don’t have a biopsy, your doctor may recommend monitoring your PSA level. They will recommend what your PSA level should be. You are usually discharged back to your GP to be monitored. Your GP can refer you back if the PSA levels go up.
