Researchers have identified various biological and genetic subtypes of prostate cancer. Although these subtypes are typically not yet used to guide treatment recommendations, they are the subject of active research funded by the Prostate Cancer Foundation.

How Do I Screen For Prostate Cancer

There are two main early stage screening methods a digital rectal exam , and a blood test measuring PSA levels. PSA screening is regarded as the best method to screen for prostate cancer in men over 40 or those of a certain risk factor.

While men might be intimidated by a DRE, itâs a quick and safe screening technique used by a physician, and should cause no significant pain.

A Digital Rectal Exam is a simple, painless and quick procedure. A physician inserts a gloved and lubricated finger into the rectum and feels the prostate to identify if the prostate is enlarged, has lumps or is an abnormal texture compared to a healthy example.

Although this procedure is a very clear indicator of prostate health, the entire prostate canât be examined during a DRE. This is why physicians will also take into account PSA blood work, health history, and other risk factors. Overall, itâs often difficult to detect prostate cancer early, itâs mostly found through PSA testing – so PSA screenings should be done regularly, starting at the age of 40-50.

When To See A Doctor

If you dont see a reduction in symptoms or if your symptoms worsen, you may need to undergo a different treatment plan.

Prescription Medications

The first higher-level treatment is to begin taking prescription drugs for an enlarged prostate. One class of medication is an alpha blocker. These medications, such as Flomax, Rapaflo, and Cardura, work by relaxing the affected muscles around the prostate to encourage urine flow.

Another type of medication is a 5-alpha reductase inhibitor. Examples of these medications include Proscar and Avodart, which are long-term medications that help to block the production of dihydrotestosterone and shrink the size of the prostate.

Surgery and Minimally Invasive Procedures

For moderate to severe BPH, you may need a medical procedure to relieve your symptoms. There are a variety of procedures, including laser therapy, microwave heat, or prostate tissue compression. Your doctor or nurse will also ask you how youre feeling and if you have any symptoms, such as pain or tiredness. Doing A Prostate Self Exam:

You could insert your finger as described in my page on self prostate massage, and you may get some idea but nothing definitive. It should be soft not hard, painless to the gentle touch, and feel smooth not lumpy. Make sure you use gloves and lubrication to make insertion easier. Be gentle!

Getting a DRE or digital rectal exam by a trained urologist will give you the information you want. Just be very hesitant if he wants then to do a biopsy as there are many risks to this procedure that he may downplay including the path to invasive action if he finds the slightest sign of cancer. Possible Inaccurate Or Unclear Test Results

As an example, neither the PSA test nor the DRE is 100% accurate. These tests can sometimes have abnormal results even when a man does not have cancer , or normal results even when a man does have cancer . Unclear test results can cause confusion and anxiety. False-positive results can lead some men to get prostate biopsies when they dont have cancer. And false-negative results can give some men a false sense of security even though they might actually have cancer.

Further Tests For An Enlarged Prostate

Depending on your test results, you may have further tests to find out what is causing your symptoms.

Bladder pressure test

This shows how well your bladder is working. The doctor passes thin tubes up your penis and fills your bladder with a clear liquid. Thin tubes are also placed in your back passage . The tubes measure the pressure in your bladder, back passage and urethra. You will then be asked to empty your bladder, and the pressures will be measured again while you urinate. You may have this test if youre thinking about having surgery to treat an enlarged prostate, or if youve had surgery but your symptoms havent improved or are getting worse.

Flexible cystoscopy

This shows whether you have a blockage or any abnormal tissue in your urethra or bladder. A doctor or specialist nurse will pass a thin tube up your penis into your bladder. Youll be able to feel the tube moving, but anaesthetic gel will be used to make the test more comfortable. The tube has a light and camera on the end so the doctor or nurse can see the inside of your urethra and bladder on a screen. You may have this test if you have severe urinary symptoms, blood in your urine or pain, or if you often get urine infections. You may also have this test if your doctor thinks your urethra or the opening of your bladder may be too narrow this is called a stricture.

If You Do Choose Screening Should You Have A Rectal Examination

Doctors have a hackneyed saying about rectal examinations: if you dont put your finger in it, youll put your foot in it.

The point is that we can miss important things by not doing a rectal examination. This can be true for some people with symptoms, such as rectal bleeding. But on balance, it seems its not the case for prostate cancer screening.

We need to decide where to set the threshold for what is considered normal for diagnostic tests. This is the case for the PSA blood test, where traditionally we use a cut-off of four nanograms per millilitre as dividing normal from abnormal .

Set the cut-off too low, close to zero, and well send nearly all men for biopsies that are largely unnecessary. Set the cut-off too high and well miss many prostate cancers. Unfortunately, theres no magic threshold that perfectly divides the men with the cancers we want to catch from the men we neednt trouble.

Does the digital rectal examination improve matters when added to the PSA test? Not really. The rectal examination does pick up slightly more cancers, but these seem to be mostly the less aggressive cancers, less likely to be the ones we need worry about. In doing so, it causes more false alarms, by creating two or more false positive results for every cancer found.

Why would we want to perform an examination that many men find unpleasant when we could instead simply interpret the blood test a little differently?

Transitional Cell Prostate Cancer

Itâs Time For a New Dialogue About Prostate Cancer Screening

This is also known as urothelial carcinoma. This cancer starts in the cells that line the urethra . Transitional cell cancer can start in the bladder and spread to the prostate or, more rarely, it can start in the prostate itself.

Studies of men with transitional cell prostate cancer show that PSA levels can be low or high. More research is needed before we can know whether PSA tests can help to diagnose transitional cell prostate cancer.

Men with this cancer often have difficulty urinating and find blood in their urine. This is because the cancer grows around the urethra, causing it to narrow. This means transitional cell carcinoma is often diagnosed when men have surgery called transurethral resection of the prostate to treat their urinary problems, as the tissue removed during surgery is looked at under the microscope.

If the cancer started in the prostate and has not spread outside the prostate, then you may be offered surgery and radiotherapy. If the cancer has spread to areas just outside the prostate or to more distant areas of the body such as the bones then chemotherapy and/or radiotherapy may be an option.
