Over the years, evidence has accumulated that men who eat a heart-healthy diet and exercise regularly have a lower risk of developing prostate cancer. But recent findings from the Harvard-based Health Professionals Follow-up Study suggest even after a man is diagnosed with prostate cancer, healthy eating can help stack the odds in his favor.

“It’s one of the first studies to look at dietary interventions after men have been diagnosed with prostate cancer,” says Dr. Marc B. Garnick, a prostate cancer specialist at Harvard-affiliated Beth Israel Deaconess Medical Center. “It supports the idea that you can potentially modify the behavior of prostate cancer that is still confined to the prostate gland.”

The findings suggest that a key dietary step to fight prostate cancer is replacing animal fats and refined carbohydrates with healthier vegetable fats from foods such as avocados, walnuts, and soybean, canola, and olive oils. Men who do this may be less likely to develop aggressive and more dangerous prostate cancer, as opposed to the slow-growing, low-risk type that many men die with but not from.

Study Shows That Lycopene Doesn’t Cut Risk of Prostate Cancer

May 17, 2007 — The news that tomatoes could prevent prostate cancer sounded too good to be true, and apparently it was.

Lycopene, found mainly in tomatoes and tomato products, had little impact on prostate cancer risk in a new study from the National Cancer Institute and Seattle’s Fred Hutchinson Cancer Research Center.

Early research suggesting a protective role for lycopene spurred great commercial and public interest in the antioxidant in the late 1990s.

But subsequent studies have been either contradictory or inconclusive, Fred Hutchinson assistant professor and researcher Ulrike Peters, PhD, MPH, tells WebMD.

The new research, led by Peters, is one of the largest and most rigorously designed trials ever to examine the issue. And the lycopene findings were unequivocal.

“It would be great if it were true. would be a cheap and easy way to lower prostate cancer risk, and it would be a great public health message,” Peters says. “Unfortunately, it’s not that easy.”

The titles and abstracts of papers identified in the initial search were reviewed and classified as potentially relevant by three independent reviewers . In cases in which there was doubt with regard to the relevance of a paper, a full-text copy was examined. How Our Research Was Done

All Adventist Health Study participants agree to fill out self-administered food frequency questionnaires reporting the average number of times per week they ate approximately 200 different foods and beverages, and the serving sizes. After tracking the studys male participants, all of whom were cancer-free when they enrolled in the project, we found that 1,226 of them had been diagnosed with prostate cancer, and 355 of those were aggressive cases.

As we focused on dietary lycopene levels, we found that men who consumed canned and cooked tomatoes five to six times a week had a 28 per cent decreased risk of prostate cancer compared with men who never consumed this food. The effect was still significant even after adjusting for potential confounders including ethnicity, education, obesity, exercise levels, alcohol consumption and others. Interestingly, we found no significant association between prostate cancer and consumption of raw tomatoes, tomato soup, tomato sauce and tomato-based vegetable juice.

While all tomatoes and tomato-based products contain lycopene, other studies have shown that lycopene is absorbed at different rates depending on the product consumed. Lycopene bioavailability is higher when tomatoes have been heated or cooked, and especially if cooked with oil. Processing tomatoes in this way contributes to the separation of the lycopene from the carrier proteins.

Focus On Diet Not Pills

So what can be said about the influence of diet and lifestyle on prostate cancer risk? Not much, says Peters.

There is growing evidence that obesity increases a man’s risk for the disease and some suggestion that a healthy diet may be protective. But neither association has been proven.

American Cancer Society nutritional epidemiologist Marji McCullough, PhD, says it is increasingly clear that cancer prevention efforts should focus more on healthy diets and less on single nutrients like lycopene and beta-carotene.

“Single nutrients don’t always give you the big picture,” she says. “We saw that a few years ago in the beta-carotene studies.”

Researchers thought taking beta-carotene would help prevent lung cancer in smokers, but they ended up finding more cancers in the smokers who took the antioxidant supplements.

McCullough says the message from those studies and this one is that until more is known, individual carotenoids should not be taken in high doses in supplement form.

“I would recommend that people try to get their carotenoids and other nutrients from the foods they eat,” she says. “That ensures that they are getting the nutrient in its natural form along with the other nutrients in those foods.”

His latest research, funded by World Cancer Research Fund , is revealing a fascinating link between tomatoes and prostate cancer.

    Prostate cancer is the second most common cancer worldwide. In the US, there were an estimated 165,000 new cases of prostate cancer and nearly 30,000 deaths in 2018. Since 1989, a research team at Loma Linda University Health has examined a number of dietary practices to see which offer promise in preventing chronic diseases such as heart disease and cancer. Earlier research in the Adventist Health Study and several other projects internationally suggest high lycopene intake is associated with reduced risk of prostate cancer. Tomato and tomato-based products are of particular interest as they are major sources of the carotenoid lycopene.

    Dietary Supplements: No Magic Bullets

    Lycopene is a pigment in red fruits such as tomatoes and watermelon. Eating tomato sauce and other foods with lycopene regularly is associated with a lower risk of prostate cancer. There is no good evidence yet that taking lycopene supplements prevents prostate cancer.

    Vitamin E is an antioxidant nutrient found in a variety of fruits, vegetables, nuts, and seeds. The SELECT study found no evidence that 400 International Units per day of vitamin E in a dietary supplement prevents prostate cancer. Further analysis of the SELECT study findings revealed that men who took vitamin E had a higher risk of prostate cancer than those who took an inactive placebo pill.

    Selenium is a chemical found naturally in soil, groundwater, and certain foods. The SELECT trial found no evidence that 200 micrograms per day of selenium as a dietary supplement prevents prostate cancer.

    Healthy Fats Cut Risk

    The Harvard study involved about 4,500 men who were diagnosed with early-stage, nonmetastatic prostate cancer, meaning it had not yet spread beyond the prostate gland to other parts of the body. Researchers knew what the men ate thanks to detailed food surveys that every study participant filled out periodically.

    Men who ate the most vegetable fats were less likely to develop the more lethal form of prostate cancer, compared with men with diets richer in animal fats. Most of the vegetable fats came from oils in salad dressings and nuts.

    Does that mean that something in vegetable fats helped to keep the cancers in check? We cannot really answer that question based on this study, although vegetable fats do contain substances that could, hypothetically, help to keep cancer in check by suppressing cell damage and inflammation.

    Another possibility is that the men benefited from what they were eating less of, which is carbohydrates. Men who replaced 10% of their total calories from carbohydrates with calories from vegetable oils were 29% less likely to die from prostate cancer over eight years of follow-up.

    Does this mean that low-carb diets fight cancer? Again, we can’t be sure. The study tells us that men who eat the most vegetable fats, as opposed to animal fats and carbs, are at lower risk of developing lethal cancer. It does not tell us why.

    SEATTLE – Move over, tomatoes! All vegetables – especially broccoli, cabbage and their cruciferous cousins – may substantially reduce the risk of prostate cancer, according researchers at the Fred Hutchinson Cancer Research Center. Eating just three servings of vegetables a day can cut a man’s risk of prostate cancer nearly in half. While carrots, beans, greens and cooked tomatoes all were found to decrease risk, the strongest effect was for cruciferous vegetables. These findings will appear in the Jan. 5 issue of the Journal of the National Cancer Institute.

    “The bottom line is that if you eat a lot of vegetables, you can cut your risk of prostate cancer by about 45 percent,” says Alan Kristal, Dr.P.H., co-investigator of the study. “And, if some of those vegetables are from the cruciferous family, like broccoli and cabbage, you may reduce your risk even further.”

    Kristal and Jennifer Cohen, Ph.D., from the Center’s Cancer Prevention Research Program, led the data analysis. Janet Stanford, Ph.D., head of the Center’s Prostate Cancer Research Program, also participated. All are from the Center’s Public Health Sciences Division.

    Men who ate three or more servings of vegetables a day had a 48 percent lower risk of prostate cancer, compared to men who ate fewer than one serving a day . This association was independent of other dietary factors, such as fat intake, and for medical factors, such as history of prostate cancer in a father or brother.

    Tomatoes And Prostate Cancer

    But back in 2003 researchers at Ohio State University had already compared the cancer preventative properties of tomato products with those of lycopene. In prostate cancer prone rats, they found that those fed a diet containing tomato powder took significantly longer to develop the disease, and were 26% less likely to die from prostate cancer compared with those on a “control” diet containing no tomato or lycopene foods. When the rats were fed a diet of lycopene alone, the death rates from prostate cancer were similar to those fed a control diet. Obviously, there is more to the health benefits of tomatoes than just lycopene. Dozens of compounds in tomatoes have physiological activity but lycopene became the glamour boy because it is relatively easily measured and it is an antioxidant. And we have been deluged by the overly simplistic notion that oxidation is at the heart of cancer. In the current study, researchers at the Fred Hutchinson Cancer Research Center took blood samples from over 28,000 men and eight years later compared blood lycopene levels in those who had developed prostate cancer to those who had not. Like in the previous rat study. they found no protection from lycopene. Unlike the rat study, however, they did not investigate tomato consumption!
