Cytokines In Seminal Plasma

9 Major Causes of Low Sperm Count, Oligospermia Treatment

Tumour necrosis factor – and interleukin -1 were assayed in seminal plasma by the use of a commercially available chemiluminescence immunometric assay kit . The assay was performed according to the manufacturer’s instructions in an automatic analyser . The cytokine contents were expressed in pg/ml, according to standard curves ranging from 1.7 to 1000 pg/ml for TNF-, and from 1.5 to up to 1000 pg/ml for IL-1.

What Are The Symptoms

The main symptom of hyperspermia is producing a larger than normal amount of fluid during ejaculation.

One study defined this condition as having a semen volume of more than 6.3 milliliters . Other researchers put it in the range of 6.0 to 6.5 milliliters or higher.

Men with hyperspermia may have more trouble getting their partner pregnant. And if their partner does become pregnant, theres a slightly increased risk that she might miscarry.

Some men with hyperspermia have a higher sex drive than men without the condition.

What Are The Available Treatments For Bph

As men get older, their prostates tend to become enlarged. This is usually from a lifetime of bad habits. However, prostate enlargement does not equate to prostate cancer. Having an enlarged prostate does not inevitably mean you will get cancer.

Once diagnosed, doctors usually recommend medication. It is essential you know that some of the drugs used to treat BPH have side-effects. Some of these include erectile dysfunction and a diminished sex drive.

Men who take alpha-blockers and alpha-reductase are prone to develop erectile dysfunction.

Studies show that approximately 3% of men who took Dutasteride and 6.4% of men who took Finasteride experienced a loss of libido.

Therefore, it is not uncommon for men to experience flagging sexual desire and compromised erectile after initiation of treatment.

Recent scientific research has pointed out that medicine such as blue pills will also have a relaxing effect on the prostate.

30% of men facing BPH will encounter compromised sexual function. One-third of men suffering ED have troubling symptoms related to the prostate.

Chronic Prostatitis And Fertility

Chronic prostatitis is a poorly understood disease, yet it’s the most common type of prostatitis. Researchers are investigating the possible link between chronic prostatitis and infertility.

The prostate gland is the walnut-sized gland that makes up part of a man’s reproductive system. It secretes a fluid that contributes to the semen that sperm swim in. The term prostatitis refers to inflammation of the prostate gland. Acute bacterial prostatitis is simple. It is an infection of the prostate that responds to antibiotics.

Unfortunately, chronic prostatitis is not so simple and its a lot more common than acute prostatitis. Its also more difficult to understand. Doctors tend to have trouble both diagnosing it correctly and treating it successfully. To add to the complexity, several studies have found a link between chronic prostatitis and fertility problems in men.

“Part of the confusion with chronic prostatitis and infertility is that what we call chronic prostatitis is not one but several different diseases,” says Daniel Shoskes, MD, a urology specialist at the Cleveland Clinic in Ohio, “and not all types of chronic prostatitis cause infertility.”

How Can Azoospermia Be Prevented

What is Hypospermia

There is no known way to prevent the genetic problems that cause azoospermia. If your azoospermia is not a genetic problem, doing the following can help lessen the chance of azoospermia:

  • Avoid activities that could injure the reproductive organs.
  • Avoid exposure to radiation.
  • Know the risks and benefits of medications that could harm sperm production.
  • Avoid lengthy exposure of your testes to hot temperatures.

Preventing Or Controlling Symptoms

Doctors cant always identify what causes prostatitis, which makes it difficult to avoid prostatitis completely. However, men can take steps to try to reduce the likelihood of experiencing the condition. The same actions can help control symptoms of chronic prostatitis.

  • Stay hydrated. Drinking plenty of water helps men to urinate frequently, which flushes out the urethra .
  • Avoid irritating the urethra. Avoiding or limiting caffeine, spicy foods and alcohol can help to avoid prostatitis.
  • Reduce prostate pressure. Men who ride a bicycle frequently might consider wearing padded shorts and using a split seat to reduce pressure on the prostate region.
  • Stay sexually active. Some physicians advise that men ejaculate regularly as another way to flush the urethra.

Lymphoproliferation Assay Against Prostate Antigens

The proliferation assay was performed as described previously . Peripheral blood mononuclear cells from each patient and controls were divided into two aliquots to use as responder and as antigen-presenting cells . Syngeneic peripheral blood mononuclear cells irradiated at 3000 cGy served as APC. The APC were pulsed for 2h at 37°C with different prostatic antigen sources: human prostate extract , seminal plasma obtained from pooled donor samples having a normal semen analysis , PSA or PAP . After 2h of incubation, pulsed APC were washed twice and added to plates containing 1.5×105 responder cells . All cell combinations were set up in quadruplicate. Plates were incubated for 5 days at 37°C in water saturated with 7.5% CO2 atmosphere. Subsequently, as a positive control of proliferation, the PBMC were incubated with the mitogen Concanavalin A at 5mg/ml. DNA synthesis was measured by adding 1 mCi of thymidine per well 18h before harvesting the cells onto glass fibre filters. Labelled material was automatically harvested and counted in a -plate scanner . The response was expressed as a stimulation index calculated from c.p.m. incorporated in antigen-pulsed cultures/c.p.m. incorporated in cultures with medium. Threshold value for positivity for each antigen assayed was set above the mean SI + 3 SD of the SI observed in the healthy control individuals.

Low Sperm Count And Motility

Low Sperm Count Causes and Natural Treatment

The term oligoasthenoteratozoospermia, or OAT, is used when all three of the following factors are present:

  • An abnormally low level of sperm of a healthy shape
  • An abnormally low amount of sperm
  • An abnormally low amount of sperm with good motility

Idiopathic oligoasthenoteratozoospermia is used when the cause of the condition is unclear. Up to 30% of male infertility cases are said to have idiopathic OAT.

Can Infertility Caused By Chronic Prostatitis Be Treated

Shoskes says that chronic prostatitis is a rare cause of infertility, but one that may be reversible. “If chronic prostatitis is caused by bacteria, antibiotics may reverse the inflammation and the infertility. In cases where there are white blood cells without bacteria, there is some evidence that treatment with antioxidants may help,” he explains.

Vitamins C, E, A, and other antioxidants may help protect sperm cells from the effects of reactive oxygen species. Antioxidants can be taken as nutritional supplements, but they are also found in many foods, especially fruits, vegetables, nuts, and grains.

Fertility is a complicated process that depends on many bodily functions working properly. Male infertility accounts for at least one-third of these cases. We are still learning about the possible links between chronic prostatitis and fertility. Further research should help unravel the mystery of chronic prostatitis and how it may be contributing to male infertility.

Lifestyle And Home Remedies

There are steps you can take at home to increase your chances of getting your partner pregnant, including:

  • Increasing the frequency of sex. Having sexual intercourse every day or every other day beginning at least four days before ovulation increases your chances of getting your partner pregnant.
  • Having sex when fertilization is possible. A woman is likely to become pregnant during ovulation which occurs in the middle of the menstrual cycle, between periods. This will ensure that sperm, which can live several days, are present when conception is possible.
  • Avoiding lubricants. Some products such as Astroglide or K-Y jelly, lotions, and saliva might impair sperm movement and function. Ask your doctor about sperm-safe lubricants.

Alternative medicine

Evidence is still limited on whether or how much herbs or supplements might help increase male fertility. None of these supplements treats a specific underlying cause of infertility, such as a sperm duct defect or chromosomal disorder. Some supplements might help only if you have a deficiency.

Supplements with studies showing potential benefits on improving sperm count or quality include:

  • Alpha-lipoic acid

The Treatments For Low Sperm Count Include:

  • Surgery: For example, a varicocele can be corrected via surgery. Doctors can also reverse prior vasectomy that if affecting the fertility. In cases where no sperm are present when it ejaculates, sperms can be retrieved directly from the testicles via the sperm retrieval techniques.
  • Treating infections: Antibiotics can also cure an infection of the reproductive tract, but it doesnt always restore fertility. The antibiotics are able to cure the problem quickly and there would be no problem for the patient as there is no side effect.
  • Treatments for sexual intercourse problems: Medication can also help to improve the fertility in conditions like erectile dysfunction or premature ejaculation.
  • Hormone treatments and medications: The doctor can recommend a hormone replacement in some cases where the infertility is due to hormonal imbalance.
  • Assisted reproductive technology: ART treatments involve obtaining the sperm via the normal ejaculation, surgical extraction or via the donor individuals which depends on the specific situation. The sperm is then inserted into the female genital tract.
  • Treatment For Benign Prostate Hyperplasia

    Low Sperm Count, Motility, Morphology

Treatment for benign prostate hyperplasia can involve:

    • Lifestyle changes, such as limiting a night-time consumption of liquids, avoiding caffeine and alcohol, avoiding medication such as diuretics, decongestants and antihistamines and getting more exercise, as well as dietary improvements, such as eating more fibre, avoiding artificial sweeteners and sweetened beverages.
    • Medication, such as alpha-blockers or 5-alpha-reductase inhibitors.
    • Interventions, such as the Urolift® system, a non-surgical proprietary device that uses tiny implants to lift the prostate away from the urethra and thereby relieve BPH symptoms that result from pressure on the urethra.
    • Surgical procedures, such as a prostatectomy , transurethral microwave thermotherapy , prostate laser, and Transurethral Resection of the Prostate .
    • Herbal prostate treatments, such as saw palmetto extract. Saw palmetto extract is made from the fruit of the saw palmetto, which is native to the southeast and Gulf coasts of North America and may potentially assist in reducing urinary frequency and increasing urinary flow.

How Is Chronic Pelvic Pain Syndrome Managed Or Treated

Prostatitis treatments vary depending on the cause and type. Asymptomatic inflammatory prostatitis doesnt require treatment.

For chronic pelvic pain syndrome , your healthcare provider may use a system called UPOINT to classify symptoms into six categories. Your provider uses multiple treatments at the same time to treat only the symptoms youre experiencing.

Approximately 80% of men with CPPS improve with the UPOINT system. The system focuses on these symptoms and treatments:

    • Urinary: Medications, such as tamsulosin and alfuzosin , relax muscles around the prostate and bladder to improve urine flow.
    • Psychosocial: Stress management can help. Some men benefit from counseling or medications for anxiety, depression and catastrophizing .
    • Organ: Quercetin and bee pollen supplements may relieve a swollen, inflamed prostate gland.
    • Infection:Antibiotics kill infection-causing bacteria.
    • Neurologic: Prescription pain medicines, such as amitriptyline and gabapentin , relieve neurogenic pain. This pain can include fibromyalgia or pain that extends into the legs, arms or back.
    • Tenderness: Pelvic floor physical therapy may include myofascial release . This therapy can reduce or eliminate muscle spasms.

Autoimmune Response Against Prostate Antigens In Patients With Chronic Prostatitis

In order to investigate whether an autoimmune cause might be the reason for discomfort seen in all chronic prostatitis patients enrolled in our study, we performed PBMC proliferation assays in response to different prostate antigens. As a source of human prostate antigens we chose seminal plasma proteins , a normal prostate tissue extract and also purified prostate proteins such as PSA and PAP.

None of the patients from group A who were coursing with an infection at the time of the study showed positive lymphoproliferative response against the autoantigens assayed. In contrast, we detected a positive proliferative response against most of the antigens examined in 10 patients . These patients had no signs of infection. The rest of the patients examined, who also did not have signs of infection, showed negative proliferation against prostate antigens . When secretion of INF- and IL-10 in culture supernatants from PBMC stimulated with SP, PSA or PAP was assayed, only supernatants from antigen-stimulated PBMC from group B showed elevated levels of INF- .

    Fertility After Prostate Cancer Treatment

11 Warning Signs and Symptoms of Low Sperm Count, Oligospermia

It is nearly impossible for a man to retain his ability to father children through sexual intercourse after initial treatment.;During prostatectomy, both the prostate and the nearby seminal vesicles are removed. Seminal vesicles and the prostate provide semen that carries the sperm down the urethra and out the penis during ejaculation. The loss of semen following surgery makes ejaculation impossible, so the sperm cannot physically leave the body.;Radiation therapy nearly always impairs fertility. Radiated prostate cells and seminal vesicles tend to produce semen that cannot transport the sperm well. Sperm can also be damaged, though this occurs far less frequently with more accurate dose planning.

    Targeted Therapy And Immunotherapy

Targeted therapy and immunotherapy drugs;attack cancer cells differently from standard chemo drugs. Little is known about their effects on fertility or problems during pregnancy.

Males taking thalidomide or lenalidomide have a high risk of causing birth defects in a fetus exposed to these drugs, which can stay in semen for a few months after treatment ends. Oncologists recommend that males and any sexual partner who is able to get pregnant use extremely effective forms of birth control, for example a condom for the man and a long-acting hormone contraceptive or IUD for the woman.

See Targeted Therapy and Immunotherapy to learn more about these cancer treatments.

Factors Influencing The Chances For A Complete Cure Of Chronic Pelvic Infections In The Male:

  • Duration of the disease.
  • Number of different antibiotic regimens administered and the total length of therapy. The prevalence of multi-drug resistant bacteria is a significant and growing concern.
  • Sonographic appearance of the prostate, seminal vesicles, epididymis and testes. The extent of scarring is inversely related to positive therapeutic outcome.

    Low Sperm Count Diagnosis

When you see a doctor because youre having trouble getting your partner pregnant, he or she will try to determine the underlying cause. Even if your doctor thinks low sperm count is the problem, it is recommended that your partner be evaluated to rule out potential contributing factors and determine if assisted reproductive techniques may be required.

Testing and diagnosis may involve the following:

General physical examination and medical history

This includes examination of your genitals and asking questions about any inherited conditions, chronic health problems, illnesses, injuries or surgeries that could affect fertility. Your doctor might also ask about your sexual habits and your sexual development.

    Semen analysis

A low sperm count is diagnosed as part of a semen analysis test. Sperm count is generally determined by examining semen under a microscope to see how many sperm appear within squares on a grid pattern. In some cases, a computer might be used to measure sperm count.

Sperm are very temperature-sensitive. If collection is done at home, the sample should be kept at body temperature by keeping it next to the body during transportation. It should NOT be left at room temperature for an extended period of time and should NOT be refrigerated.

Sperm motility decreases after ejaculation; thus, timing and temperature are critical to obtaining accurate results. If the sample is poor, repeat testing might be needed.
