Which is Better – Surgery vs. Radiation for Prostate Cancer?

If youve been diagnosed withprostate cancer, your doctor will consider many factors before recommending the besttreatment. For many men, that may mean a prostatectomy. In this surgery, doctors remove the entire prostate.

The Johns Hopkins Hospital performs more of these procedures than almostanywhere else in the world. One of the most common questions they hear frompatients: What should I expect after surgery?

Johns Hopkins urologistMohamad Allaf, M.D., explains the surgery and recovery.

How Will Sex After Prostate Cancer Be Different

One of the treatments of prostate cancer is to remove a portion of the prostate or the totality of the gland before cancer spreads beyond it You may have heard that the tumor is inside the capsule, but moving fast.

The procedure is known as radical prostatectomy and involves incisions, either above the perineum or on the lower abdomen.

Newer surgeries are sometimes robot-assisted, and we also have laparoscopic prostatectomies that do not require a large incision.

The end result is the removal of a portion or the totality of the prostate, and certain surgical approaches may be associated with a lower risk of nerve damage than others. Youll see this referred to as nerve-sparing prostatectomies throughout this guide.

As soon as a man is informed that he will need to go through a Radical Prostatectomy, the likelihood is that one of the very first questions will be about the effect that it will have on their sex life. In a lot of mens minds, the surgery is thought of like its castration.

However, now that there are multiple types of nerve-sparing prostate surgery, many men have found they can maintain sexual activity after surgery.

Likewise, there are plenty of sexual activities open to those who do find they still have the desire, even with some physical changes. We want to help you understand the changes that men can experience after prostate surgery, whether that is physical or mental.

Is There A Role For Focal Ablation Of Prostate Cancer

One of the unique characteristics of prostate cancer is the spectrum of the disease as it relates to disease aggressiveness, age range of those affected, and treatment priorities. The age range in a large personal series of approximately 5000 RP performed by one of the authors of this review is 36 to 81 years. The risk and extent of disease ranges from men with a single biopsy core with 1-mm GGG 1 cancer to all biopsies showing GGG 4 and 5 disease. The priority of some men is to cure the disease whereas others are motivated to preserve quality of life. It is reasonable to propose that the optimal management of prostate cancer should include AS, whole-gland treatment, and an alternative offering potential for oncological control with preservation of quality of life. We believe this option is FA of prostate cancer.

Robotic Prostate Operations Cost Up To $30k Are They Worth It

Jill Margo

As the fashion for expensive, high-tech surgery for prostate cancer sweeps the country, a landmark Australian study has shown it provides no extra value for men over traditional surgery.

This is the first study in the world to present high-level evidence directly comparing robotic surgery, which can cost up to $30,000 in a high-end private hospital, with traditional open surgery for prostate cancer, which can cost $3000 to $5000.

The Brisbane study found three months after surgery that there was no significant difference in cancer control, sexual function, urinary continence, bowel function or time away from work for men who have the operation.

A new study has found robotic surgery for prostate cancer offers no extra value for patients. Gabriele Charotte

These findings negate claims repeatedly made to promote the robotic surgery technique, which is now used in 60 per cent of operations to remove a cancerous prostate in Australia.

“The findings were surprising,” says Professor Robert Gardiner of the University of Queensland’s Centre for Clinical Research who led the study out of the Royal Brisbane & Women’s Hospital. Funded by Cancer Council Queensland, it was published today in The Lancet.

Professor Gardiner said his team had believed that sexual and urinary function would be better for men who had robotic surgery and that their return to work would be earlier.

The out-of-pocket expenses of an open operation in a private hospital ranges from $3000 to $5000.

What Is Robot Assisted Surgery

Robotic surgery, or robot-assisted surgery, is an advancement in surgical techniques where a surgeon uses a computerized robotic arm with small tools attached to it to carry out complex surgeries.

Robotic surgical system provides the surgeon a high-definition, magnified , 3-D view of the surgical site.

The surgical instruments are controlled by the surgeon using the console, to direct special surgical instruments which are smaller in size, and more flexible than the human hand.

The robot does not perform the surgery on its own. The movement of the surgeons hand just mimicked by the instruments attached to the robotic arms, while this is completely controlled by the surgeon. Robot assisted surgeries offer precise action that is devoid of hand tremors, enabling the surgeon to perform the most complex procedures with enhanced precision, dexterity and control
