What You Need To Know About The Prostate Gold Seeds In Prostate Cancer

The main purpose of the prostate is to produce semen, a milky fluid that sperm swims in. During puberty, the body produces semen in a large number of cases, including enlarged prostate. This fluid causes the prostate to swell and cause a number of bladder-related symptoms. This is why the prostate is important to the body. It can be caused by many factors, including infection and inflammation.

A enlarged prostate can also cause blockages in the urethra. A blocked urethra can also damage the kidneys. A patient suffering from an enlargement of the prostate may have pain in his lower abdomen and genitals. If pain is present, a digital rectal examination will reveal hard areas. A doctor may prescribe surgery or perform an endoscopic procedure. If the enlarged prostate is not completely removed, it will shrink.

While the size of an enlarged prostate will influence the extent of urinary symptoms, men may experience a range of urinary symptoms. Some men have minimal or no symptoms at all. Some men will have a very enlarged prostate, whereas others will have a mild enlargement. Generally, the symptoms can stabilize over time. Some men may have an enlarged prostate but not notice it. If they have an enlarged colon, their physician can perform a TURP procedure.

About Fiducial Markers And Rectal Spacers

Fiducial markers are tiny metal objects . They help your healthcare providers line up the beams of radiation and make sure your radiation therapy is delivered exactly the same way each time. This helps them target the tumor and avoid your nearby healthy tissue. The fiducial markers will stay in your prostate after your treatment.

Youll get a rectal spacer called SpaceOAR hydrogel. It is a gel thats placed between your prostate and rectum to move your rectum away from your prostate. This protects your rectum from radiation and reduces some side effects of radiation therapy. The rectal spacer will stay in place for 3 months. Then, itll be absorbed by your body and come out in your urine.

  • Work in a hospital or nursing home.

Who Can Have Permanent Seed Brachytherapy

On its own

Permanent seed brachytherapy on its own may be suitable for men with CPG 1 localised prostate cancer. This is because the radiation from the radioactive seeds doesnt travel very far, so will only treat cancer that is still inside the prostate.

It may also be suitable for some men whose cancer have a CPG 2 or CPG 3 of spreading.

With other treatments

If you have CPG 4 or CPG 5 localised prostate cancer, you may have brachytherapy together with external beam radiotherapy and hormone therapy. This is sometimes called a brachytherapy boost. Having these other treatments at the same time as permanent seed brachytherapy can help make the treatment more effective. But it can also increase the risk of side effects.

Some men with intermediate risk localised prostate cancer or locally advanced prostate cancer may be offered a brachytherapy boost.

When is permanent seed brachytherapy not suitable?

Permanent seed brachytherapy wont be suitable if your cancer has spread to other parts of your body .

It may not be suitable if you have a very large prostate. If you do have a large prostate you may be able to have hormone therapy before treatment to shrink your prostate.

Not all hospitals offer permanent seed brachytherapy. If your hospital doesnt do it, your doctor may refer you to one that does.

The Initial Causes Gold Seeds In Prostate Cancer

One of the first symptoms of prostate issues is pain or tenderness in the groin or lower back. This can be the result of a noncancerous condition called enlarged prostatic tissue, or it could be an infection of the bladder. In either case, its important to see a doctor as soon as possible. If youre suffering from prostate pain, you may want to consider reducing your caffeine intake.

Another symptom of a potentially enlarged prostate is difficulty starting a stream of urine, leaking, or dribbling. These symptoms are not serious, but theyre still alarming. Most men put up with an enlarged prostate for years before seeking medical attention, but they typically seek treatment as soon as they notice symptoms. Even if you dont have symptoms, its worth getting checked to determine if you have any prostate issues.

If you experience nightly bathroom runs, you may be experiencing an enlarged prostate. You may be having difficulty starting a stream of urine, or you may even be dribbling or leaking during the day. These problems arent life-threatening, but can become a nuisance. You should not ignore these signs and seek treatment as soon as you notice them. If you feel any of these symptoms, you should consult a doctor.

Transperineal Insertion Of Gold Seeds And Spaceoar

This is almost identical to the transperineal biopsy of the prostate which was performed to diagnose prostate cancer previously. The gold seeds will assist the radiation oncologist in accurately targeting the prostate as well as minimizing the radiation dose to the nearby rectum.

Gold seeds

Gold seeds are markers seen on an x-ray to assist the radiation oncologist in localizing the prostate gland. This enables accuracy when delivering the radiation treatment. The prostate is a mobile pelvic organ and can be clearly seen on daily imaging, which enables advanced radiation treatments with confidence.

These seeds are not radioactive. The gold seeds are made of gold or platinum and will stay in your prostate permanently. They are smaller than a grain of rice.

SpaceOAR Hydrogel

The SpaceOAR is an absorbable water-based gel that temporarily creates a space between the prostate and the rectum, protecting the rectum from radiation exposure during prostate radiatiotherapy. The prostate and rectum are very close and are naturally separated by a small space.

The hydrogel stays in the body for about 3 months and is naturally absorbed in about 6 months, well after the last radiation treatment

Before the procedure

Please inform your doctor or specialist nurse if you are

  • taking any blood thinning medications such as aspirin, warfarin, clopidogrel, rivaroxaban or dipyridamole
  • have bleeding problems
  • have allergies to any medications, including anaesthetic
  • have an artificial heart valve

What Type Of Technology Does This Treatment Use

Prostate Cancer Radiation Fiducial Marker and SpaceOAR Gel Procedure ABS Virtual Reality (360 VR)

We use real time, 3-dimensional imaging to carefully place each seed at a predetermined location and depth. In this way, we cover the entire gland and the cancer site. The number of seeds implanted into your prostate will depend on your unique anatomy.

We place an ultrasound probe in your rectum to image the prostate. We also use continuous X-ray imaging. Together, these provide a magnified, 3D view of your prostate during the procedure. After the implantationand while you are still anesthetizedwe will check your urethra and bladder and retrieve any seeds found in your bladder.

Surgical procedures to remove the diseased prostate are usually necessary. Surgical procedures are not always necessary. If the disease is caused by bacterial infections, a doctor can treat the symptoms using alpha-blockers or surgery. Physical therapy, relaxation exercises, and warm baths are all recommended. A physician may also prescribe antibiotics to cure the infection. A bacterial infection can also cause a recurrence of the condition.

An enlarged prostate can be uncomfortable for both men and women. Some of the symptoms of an enlarged male reproductive organ include a weakened urine stream, urgent need to urinate, and urinary tract infections. BPH can also cause damage to the kidneys. A sudden inability to urinate can be life-threatening, as it can lead to bladder and kidney damage. Unfortunately, most men with enlarged prostrates put up with the symptoms for years before they seek treatment. However, many of the men with symptoms finally decide to go to a doctor for proper gynecological evaluation and to begin enlarged prostatic therapy.

Permanent Seed Brachytherapy For Prostate Cancer

Permanent seed brachytherapy is when your doctor puts very small radioactive metal seeds into your prostate gland. The seeds slowly release a low level of radiation into the area of the prostate over a few months. It is a type of internal radiotherapy.

With brachytherapy, your doctor puts the radioactive source or seeds as close to the cancer as possible. So that the cancer gets a high dose but nearby tissues and further away the levels of radiation are low.

Overview Of The Gold Seed Procedure

This is a surgical procedure we do in the operating room under anesthetic. Some centers may do this step with local anesthetic only, but we find it is more comfortable to use deep sedation. In the procedure, your legs are placed in footrests and your legs are spread apart and your knees are bent up. We place an ultrasound in the rectum so that we can see the prostate gland in real time during the procedure. We use only needles there is no cutting and no suturing required. The procedure takes about 10 minutes if we are just placing gold markers, and it takes about 20 minutes total if we are also placing SpacOAR.

The needles go through the perineum, which is the patch of skin in between the testicles and the anus. The needles do not go through the rectum.

Many radiation oncologists have a urologist place the gold seeds and SpaceOAR, but I prefer to do it myself. If a urologist places the marker seeds, he may insert the needles through the rectal wall to deposit the seeds. This is not bad and it will work fine, but I believe that you can aim better if the needles are placed through the perineum skin instead of through the rectum, and there will be less chance of infection. It can be finicky injecting the SpaceOAR into the perfect spot, so it is good to have a lot of experience with this.

CyberKnife in the Treatment of Prostate Cancer: A Revolutionary System ...

The success rate depends on the risk category of the prostate cancer and the type of success thats being measured. The most common measurement is related to prostate-specific antigen assessments after treatment. Grading is done with a system called the Gleason score. Using this method, the five-year success rates for the various risk groups are:

  • Low-risk = 95%.
  • Intermediate-risk = 90%.
  • High-risk = 74%.

It is important to know that these numbers are heavily dependent on how frequently the PSA test is drawn after therapy. PSA tests should be done every six months after therapy. In reports of similar rates from other sources, one must make sure that the PSA tests are being drawn at least this frequently or a comparison cant be made.
