Up to 10,000 men with an enlarged prostate could receive a new treatment which improves their quality of life and preserves their sexual function, under new guidance from NICE.

NICE has recommended the use of Rezum, a minimally invasive procedure that involves injecting steam to destroy excess prostate tissue.

Some current treatments use heat to destroy excess prostate tissue and this can have considerable side effects.

Benign prostatic hyperplasia is the medical term used to describe a noncancerous enlargement of the prostate. Symptoms like needing to pass urine more frequently, trouble starting to urinate and loss of bladder control affect more than a third of men over the age of 50.

Clinical evidence analysed by NICEs medical technology advisory committee shows that using the Rezum procedure relieves lower urinary tract symptoms caused by BPH in men with moderate to severe symptoms who have a moderately enlarged prostate.

Evidence also shows that using Rezum is associated with improved quality of life and a low risk of sexual dysfunction.

Rezum is cost saving compared with standard treatments such as transurethral resection of the prostate and holmium laser enucleation of the prostate by more than £550 per person over 4 years.

Follow The Diet Your Healthcare Provider Recommends

Depending on the type of ablation procedure youre having, you may need to follow a light diet or a low residue diet. Your healthcare provider will check the box below next to the diet you should follow. Start following it the morning of the day before your procedure.

  • Light dietFollow a light diet, such as small sandwiches, eggs, toast, crackers, soup, or cereal. Limit the amount of dairy products and avoid fried foods and foods with a lot of seasoning.
  • Low-residue dietFollow a low residue diet. This diet limits high-fiber foods, such as whole grain breads and cereals, nuts, seeds, and raw or dried fruits and vegetables. You should also limit the amount of dairy products you have.

How Turp Is Performed

TURP is carried out using a device called a resectoscope, which is a thin metal tube containing a light, camera and loop of wire. This is passed along your urethra until it reaches your prostate, which means no cuts need to be made in your skin.

The loop of wire is then heated with an electric current and used to cut away the section of your prostate causing your symptoms. A thin tube called a catheter is then inserted into your urethra to pump fluid into the bladder and flush away pieces of prostate that have been removed.

Urinary sheaths can also help with dribbling. They look like condoms with a tube coming out of the end. The tube connects to a bag that you can strap to your leg under your clothing.

The only other thing I was drinking in the first 6 months after my diagnosis was green tea. I would drink about 4 glasses of green tea daily to go along with the increased water intake. I wasnt putting any other liquid into my body for the first 6 months. This was a big help in starting my road to recovery.

Once I started feeling better then I added organic soy milk to my diet as well. Soy milk isnt much like regular milk but once you get used to it then its not bad at all. To this day these are the only 3 liquids I have in my diet. To recap the 3 liquids I drink today are purified water,green tea,& organic soy milk. I put no other liquids into my body period.

Now, I want to chat a little more about meat & other aspects of a proper diet. As I said we dont need meat to live. I thought cutting or limiting meat in my diet would be to hard to accomplish. Well again my thinking was wrong. Was it easy? No! However, after a couple weeks then things were starting to get easier. I didnt cut all meats out of my diet but I did cut certain meats & eat moderate amounts of all others.

One meat that needs to be completely cut or at least very minimized is red meat . Too much Red meat consumption is not good for prostate health. I was eating a lot of fast food burgers & also red meat at home. I will say to at least cut red meat completely out of your diet until you get your prostate health back.

There Are Two Key Steps To The Procedure:

Step 1. Creating a surgical map

Every prostate is unique in size and shape, so it is important for a surgeon to customize the surgery to the patients specific anatomy.

Aquablation therapy is the only procedure that combines a camera with ultrasound imaging, giving the surgeon the ability to see the entire prostate in real time.

All other surgical procedures use only a camera, limiting the surgeons ability to see.

The addition of ultrasound imaging in Aquablation therapy allows the surgeon to map which parts of the prostate to remove and which parts to avoid.

The surgeon will work to map around the parts of the prostate that cause irreversible complications like erectile dysfunction, ejaculatory dysfunction, and incontinence.

Step 2. Removing the prostate tissue

  • Follow your healthcare providers instructions for taking your antibiotics and tamsulosin .
  • Dont shower for the first 24 hours after your procedure. After 24 hours, you can go back to showering as usual. Dont take a tub bath or submerge your lower body in water for 2 weeks after your procedure.
  • Dont lift anything heavier than 20 pounds for at least 48 hours after your procedure. Your healthcare provider will tell you if you need to wait longer than 48 hours.

How Does Aquablation Therapy Compare To Other Bph Treatment Options

Beyond medications, the two categories of treatments for BPH include:

  • Minimally invasive surgical therapies , including prostatic urethral lift procedure, water vapor therapy and a temporarily implanted nitinol device
  • Surgery, including aquablation, laser vaporization and laser enucleation therapies, as well as transurethral resection of the prostate and open prostatectomy

Because aquablation is performed in an operating room and requires an overnight hospital stay, it’s considered surgery.

What Are The Risks

When Is Bph Treatment Necessary

The course of BPH in any individual is not predictable. Symptoms, as well as objective measurements of urethral obstruction, can remain stable for many years and may even improve over time for as many as one-third of men, according to some studies. In a study from the Mayo Clinic, urinary symptoms did not worsen over a 3.5-year period in 73% of men with mild BPH. A progressive decrease in the size and force of the urinary stream and the feeling of incomplete bladder emptying are the symptoms most correlated with the eventual need for treatment. Although nocturia is one of the most annoying BPH symptoms, it does not predict the need for future intervention.

If worsening urethral obstruction is left untreated, possible complications are a thickened, irritable bladder with reduced capacity for urine infected residual urine or bladder stones and a backup of pressure that damages the kidneys.

Advantages And Disadvantages Of Surgery

What may be important for one person might be less important for someone else. The advantages and disadvantages of surgery may depend on your age, general health and the stage of your cancer.

Advantages

  • If the cancer is completely contained inside the prostate, surgery will remove all of the cancer.
  • The prostate is looked at under a microscope to give a clearer picture of how aggressive your cancer is, whether it has spread outside your prostate and if you need further treatment.
  • Your health professionals can get a good idea of whether your cancer was completely removed during surgery. Your PSA level should drop so low that its not possible to detect it at six to eight weeks after surgery.
  • If there are signs that your cancer has come back or wasnt all removed, you may be able to have further treatment.
  • Some men find it reassuring to know that their prostate has been physically removed, although you will still need to have follow-up tests to make sure no cancer cells have spread outside the prostate.

Doctors have a newer method for treating an enlarged prostate, which can squeeze the urethra and cause urinary problems for men, especially as they age. The treatment, Aquablation, uses an intense jet of water to remove part of the prostate, relieving the pressure.

Patients with very enlarged prostates traditionally have limited options, but in a properly selected patient, the outcomes of Aquablation are equivalent and sometimes superior to traditional surgeries, says Emil Kheterpal, MD, a urologist who performs surgeries at UNC Rex Healthcare and worked on clinical trials for Aquablation, which was approved by the Food and Drug Administration in 2017.

Compared with other surgical methods for enlarged prostate, Aquablation is a quicker procedure with less time under anesthesia, a lower risk of side effects and typically a faster recovery.

If you are a man who makes frequent trips to the bathroom, or who has problems starting or stopping your flow of urine, you may have an enlarged prostate, known as benign prostatic hyperplasia, or BPH. Talk to your doctor to find out, because BPH usually does not resolve on its own and your problems could get worse as you get older.

Surgical procedures to remove the diseased prostate are usually necessary. Surgical procedures are not always necessary.
