Assessment Of Therapeutic Response
Many authors have confirmed the utility of both 11C and 18F-choline PET in predicting early therapeutic response to antiandrogen therapy. The findings were concordant with a decline in PSA levels. According to De Giorgi et al., PET was the only predictor of progression-free survival and overall survival in metastatic castrate-resistant tumors treated with abiraterone whereas a decline in PSA did not. Ceci et al. observed that an increase in PET uptake after docetaxel chemotherapy in castrate-resistant cancer indicated disease progression despite a decline in PSA values.
Genetic Testing For Some Men With Prostate Cancer
Some doctors now recommend that some men with prostate cancer be tested to look for certain inherited gene changes. This includes men in whom a family cancer syndrome is suspected, as well as men with prostate cancer that has certain high-risk features or that has spread to other parts of the body. What Is It Like Having A Nuclear Scan
In most cases you will be given a tracer that sends out small doses of radiation. If its put into your blood, a needle will be used to put it into a vein in your hand or arm.
Because of the special materials and equipment needed, these scans are usually done in the radiology or nuclear medicine department of a hospital. You might be able to wear your own clothing or you might be given a gown to wear during the test. Youll need to remove all jewelry or metal items that could interfere with the scans.
The scanner has a hole in the middle and looks like a large doughnut. You lie on a padded table which moves back and forth through the hole in the scanner. You will need to be very still while the scans are done. The technician may ask you to change positions to allow different views to be taken. The table may become uncomfortable after a while.

Greater Accuracy And Changing Treatment
Approximately 300 men were enrolled in the Australian trial, all with newly diagnosed localized prostate cancer , and all were considered to have high-risk disease. For all men in the trial, the planned treatment was either surgery or radiation therapy to the prostate only.
Half the men were randomly assigned to initially undergo a CT and bone scan, and the other half to PSMA PET-CT.
Based on the imaging, PSMA PET-CT was 27% more accurate than the standard approach at detecting any metastases . Accuracy was determined by combining the scans sensitivity and specificity, measures that show a tests ability to correctly identify when disease is present and not present.
PSMA PET-CT was more accurate for both metastases found in lymph nodes in the pelvis and in more distant parts of the body, including bone. Radiation exposure was also substantially lower with PSMA PET-CT than with the conventional approach.
The trial investigators also tracked how imaging results influenced clinicians treatment choices. Based on imaging findings, the initial treatment plan was changed for 15% of men who underwent conventional imaging compared with 28% of men who underwent PSMA PET-CT.
Another key finding, Dr. Hofman noted, was that PSMA PET-CT was much less likely to produce inconclusive, or equivocal, results .
Thats important, he continued, because if you have a scan with equivocal findings, it often leads to more scans or biopsies or other tests.

Getting The Results Of The Biopsy
Your biopsy samples will be sent to a lab, where they will be looked at with a microscope to see if they contain cancer cells. Getting the results usually takes at least 1 to 3 days, but it can sometimes take longer. The results might be reported as:
- Positive for cancer: Cancer cells were seen in the biopsy samples.
- Negative for cancer: No cancer cells were seen in the biopsy samples.
- Suspicious: Something abnormal was seen, but it might not be cancer.
If the biopsy is negative

Pet Scans May Improve Treatment For Esophageal Cancer
Clinical trial results presented at the Gastrointestinal Cancers Symposium in San Francisco suggest that the use of PET scans to assess a cancers response to initial chemotherapy may provide doctors with useful information to further tailor chemotherapy.
Standard treatment of patients with stage II-III esophageal and gastroesophageal junction cancers includes chemotherapy with radiation , followed by surgery. Physicians may use several different chemotherapy regimens without knowing which particular chemotherapy will be most effective.
In the current study doctors administered chemotherapy before chemoradiation and used PET scans after the initial chemotherapy to assess response to treatment in order to see if the early PET scan can help doctors make quick course corrections to maximize patient benefit from chemotherapy.
A total of 257 patients with stage II-III esophageal and GEJ adenocarcinoma were treated with one of two different chemotherapy regimens: either modified FOLFOX-6 or carboplatin/paclitaxel. A PET scan was repeated in all patients after a couple cycles of chemotherapy. If the PET scan demonstrated that the chemotherapy was working, no change in treatment was made. If the PET scan however demonstrated that the chemotherapy was not effective, the chemotherapy was changed to the other regimen.

Getting My Scan Results
Your doctor or nurse will tell you how long it will take for the results of all the tests to come back. Its usually around two weeks.
Your doctor will use your scan results to work out the stage of your cancer in other words, how far it has spread. This is usually recorded using the TNM system.
- The T stage shows how far the cancer has spread in and around the prostate.
- The N stage shows whether the cancer has spread to the lymph nodes.
- The M stage shows whether the cancer has spread to other parts of the body.
Why Axumin Is So Important
Being able to detect early metastatic disease with a scan offers two important therapeutic advantages. First, the knowledge of where the cancer is located can help guide effective therapy to that specific area of the body and limit damage to other areas of the body. The scan detects where the cancer is not present and where treatment is not needed.
The second valuable contribution that an accurate scan offers is a deeper insight into the disease process itselfrevealing whether or not the cancer has metastasized, and if it has metastasized, to what degree.
Recurrent cancer signaled by a rising PSA is not always due to metastases. Sometimes the cancer remains near or in where the prostate used to be, so PSA is coming from cancer recurring in the prostate gland after radiation or in the prostate fossa after surgery , which is known as a “local recurrence.”
PSA can also be elevated due to growing cancer that has metastasized to the lymph nodes or bones. This is called a systemic recurrence. Systemic recurrences are tremendously more dangerous than local recurrences. Why? A metastasis shows that the cancer has the biologic capacity to spread around the bodya process that ultimately leads to death in more than half of prostate cancer patients. Thus, knowing the location of the recurrence answers an extremely important question: whether the recurrent disease is aggressive enough to metastasize.
