Surgical Approaches: Artificial Urinary Sphincter
Surgical intervention for incontinence is typically deferred for at least one year following prostatectomy. Traditionally, the AUS has been the gold standard surgical treatment for SUI after prostatectomy since its introduction several decades ago, offering the advantage of both durability and effectiveness for even severe degrees of incontinence. Initially conceptualized by Foley in 1947, the modern AUS design has evolved over several iterations, with closer resemblance to designs introduced in the 1970s by Scott and Rosen. The present models are fashioned on the concept of using an inflatable fluid-filled cuff surrounding the urethra to control continence in addition to a hydraulic pressure-regulating balloon reservoir and control pump. In the resting activated state, the cuff is inflated, thereby occluding the urethra. When the control pump, implanted in the scrotum, is squeezed manually, the cuff is deactivated. This pumps fluid out of the cuff into the reservoir, thereby depressurizing the cuff and enabling the patient to void. Unless locked in the deactivated state, the cuff automatically reactivates over the subsequent 45 to 90 seconds following deactivation to prevent further flow of urine through the urethra.
What Is The Prostate’s Role In Urination
When urine is emptied into the bladder from the kidneys, it’s kept inside the body by a couple of valves that stay closed until your body tells them to open when you urinate.
If youve had your prostate completely removed, youve had a surgery called radical prostatectomy. Along with the prostate gland, the surgeon has to remove one of the valves outside the prostate that open and close to let urine out or keep it in. Usually having one working valve is enough, but there might also be also be some effect on the nerves and muscles in the area from the surgery that allows urine to leak. Approximately 6% to 8% of men who’ve had surgery to remove their prostates will develop urinary incontinence.

What You Can Do About Urine Leakage During Orgasm
1. Strengthen the pelvic floor
The pelvic floor and its function should be the first thing that you address to help overcome the issue of leaking urine during sex. The recovery of your urinary control and erectile function is significantly improved if you undertake pelvic floor therapy. Pelvic floor exercises have been shown to reduce the time of experiencing sexual based urine leakages following prostate surgery. It is recommended that you complete an individualised pelvic floor program of 16 weeks to give you the best chance of a full recovery.
2. Empty the bladder
This one is quite straightforward, emptying the bladder before sex or masturbation is a good idea to avoid any unwanted leaks. If there is less volume applying pressure within the bladder you will have a greater chance of maintaining its control.
3. Wear protection
Wearing protection might be a thing of the past or you might be hesitant to use them for any number of reasons, but it can be a sureproof way to manage your leakage during sex. Commonly recommended strategies include wearing a condom or a specialised latex ring applied at the base of the penis during sex.

What Happens After Urethral Sling Surgery
Patients usually recover from this surgery quickly. It’s best to limit demanding activities for approximately six weeks after surgery to avoid having the sling loosen before healing is complete.
After surgery, there may be swelling that makes it difficult to urinate. You’ll have a catheter coming out of the urethra for two to three days. After this, the catheter is removed and just about everyone is able to void on their own. In rare cases, the catheter may have to be reinserted for a few days or up to a week if you cannot urinate.
After the swelling goes down, youll gradually be able to urinate on your own and empty your bladder. However, your normal urination pattern may not return for a few weeks.
The majority of men who’ve had this procedure have been cured of their urinary incontinence and no longer use pads. The others usually improve to the point where they don’t use as many pads as they were before.

Urine Leakage Long After Prostate Removal
Leaking urine after prostate surgery is a common problem. Most men report gradual improvement over several months following surgery. However, some men remain incontinent. Certain types of incontinence can be improved with conservative treatment.
There are two main types of incontinence following prostate surgery: urge incontinence and stress incontinence.
I have to go now! Urge incontinence occurs when there is a strong urge to urinate followed by urine leakage. The strong urge to urinate usually indicates an overactive bladder. The prostate is located below the bladder, around the urethra. Some prostate conditions involve slow growth of the prostate resulting in narrowing of the urethra. The bladder must contract strongly to empty through a narrow urethra. This can cause the bladder to become irritable. Unfortunately, in some cases, the bladder remains irritable after the urethra is opened with a prostatectomy.
There are several conservative treatments for urgency. Bladder training involves urinating in the toilet on a timed schedule. Techniques are used to avoid urinating too often. Pelvic floor muscle exercise can also help relax the bladder. Some types of fluid also irritate the bladder and should be eliminated from the diet. A combination of treatments is usually most helpful.
Determination Of Evidence Strength
The categorization of evidence strength is conceptually distinct from the quality of individual studies. Evidence strength refers to the body of evidence available for a particular question and includes individual study quality in addition to consideration of study design consistency of findings across studies adequacy of sample sizes and generalizability of samples, settings, and treatments for the purposes of the guideline. Investigators graded the strength of evidence for key comparisons and outcomes for each Key Question, using the approach described in the Agency for Healthcare Research and Quality Evidence-based Practice Center Methods Guide for Comparative Effectiveness and Effectiveness Reviews.1 Strength of evidence assessments were based on the following domains:
- Study limitations, based on the overall risk of bias across studies
- Consistency of results across studies
- Directness of the evidence linking the intervention and health outcomes
- Precision of the estimate of effect, based on the number and size of studies and confidence intervals for the estimates
- Reporting bias, based on whether or not the studies defined and reported primary outcomes and whether or not we identified relevant unpublished studies

The operation went fine. I went back to work very quickly, and in most respects I felt fine. I was incontinent immediately after surgery, but I was led to believe that the problem would straighten itself out within a few weeks or months. But it didnt.
Did you share your concerns about incontinence with your surgeon?
I did, during follow-up visits after the surgery. I probably visited him three to four times during the first six months after surgery. He told me the problem would get better, and for the first month or two, I believed that. But as time went on, nothing was getting any better.
And he didnt seem to care. In a typical visit, I waited a half hour or an hour to see him for literally five minutes, and then he moved on to the next person. So I finally gave up on him.

Problems After High Intensity Focused Ultrasound
Youre more likely to get urinary problems after HIFU if youve already had other treatments for prostate cancer. Possible urinary problems include:
- difficulty urinating
- urine infections.
Difficulty urinating
HIFU usually causes the prostate to swell for a week or two, so youll have a catheter to drain urine from your bladder until the swelling has gone. HIFU can also cause the urethra to become narrow, making it difficult to empty your bladder .
Leaking urine
HIFU can cause some men to leak urine when they cough, sneeze or exercise . This may be more likely if youve already had radiotherapy. There are ways to manage leaking urine.
Urine infections
Some men get a urine infection after HIFU. If this happens, youll be given a course of antibiotics to clear the infection.

Pelvic Floor Physical Therapy After Radical Prostatectomy
Pelvic floor physical therapy is extremely effective at reducing incontinence after radical prostatectomy. With pelvic floor physical therapy most patients have regained continence within 3 months, without pelvic floor PT incontinence after radical prostatectomy usually takes about 12 months to resolve. Why would you wait a year when you could stop urine leaks in only 3 months with pelvic floor physical therapy?
You might think you can just do some Kegels with the handout your doctor gave you. The problem with that is most people do not know how to do a proper Kegel. This makes perfect sense you cannot just see a Kegel being done, and your urologist does not have time in the clinic to make sure you are performing them correctly. With men, I use biofeedback to make sure that there is a nice strong pelvic floor muscle contraction. This ensures that you know what they should feel like and how to get the most from your exercise.
Not all pelvic floor physical therapists are comfortable or trained to work with men after radical prostatectomy. You need to make sure you have a pelvic floor physical therapist with experience treating male pelvic floor dysfunction.

How Do You Retrain Your Bladder After A Catheter Is Removed
Gradually increase the amount of time between bathroom breaks. Delay urination. When you feel the urge to urinate, hold it for another five minutes or so. Then gradually increase the amount of time by 10 minutes, until you can last for at least three to four hours without having to go to the bathroom.

Treatment Options For Post
Loss of bladder control after prostate surgery is a devastating complication, which has a significant negative impact on quality of life. The good news is that with appropriate evaluation and treatment, the incontinence problem is usually treatable with significant improvement in quality of life.
It is not unusual that lack of bladder control is a problem for the first few months following radical prostatectomy. A biofeedback program may be helpful during this time period to help restore bladder control. When urinary incontinence persists more than 3-6 months after radical prostatectomy, appropriate bladder testing, called urodynamics, is critical to evaluate the function of the bladder and sphincter muscle to determine the exact cause of the post-prostatectomy incontinence . This urodynamic testing is performed in the office and takes about 20 minutes. The test involves filling the bladder through a special catheter inserted in the penis while measuring the pressures in the bladder. During the test, various maneuvers are performed to evaluate the bladder function, demonstrate the urinary incontinence, and thus specifically define the cause of the urine loss.
Normally, as the bladder fills to capacity, there is very little change in bladder pressure and the sphincter remains closed allowing the man to stay dry. When incontinence occurs following prostatectomy, this normal balance of bladder and sphincter function is disturbed.

They are gradually withdrawn as symptoms improve.In cases of persistent urinary incontinence, surgical procedures, including a sling that relieves pressure buildup in the abdomen or artificial sphincters provide long lasting results.
What Treatments Are Available To Me If My Incontinence Doesnt Go Away After A Year
While kegels and behavioral therapy work well for most men with mild to moderate leaking, they may not be completely effective for some. Luckily, there are still some options for treating bladder leakage after prostate surgery.
Another surgery is sometimes needed when bladder leaks persist for more than a year after surgery. This may consist of having a urethral sling procedure, or an artificial urinary sphincter.
With a urethral sling procedure, a synthetic mesh tape is implanted to support the urethra. Up to an 80% improvement has been seen with this procedure and some men stop leaking completely.
An artificial urinary sphincter is used in patients who have more severe urinary incontinence that is not improving, or for those patients who may have had a lot of damage to the sphincter muscle after prostate surgery. An artificial urinary sphincter is a mechanical ring that helps close the exit from the bladder.
As will all surgeries, these come with pros and cons and potential complications. Be sure to discuss these options with your doctor.
