Dr. Benson is the George F. Cahill Professor of Urology at P& S, a professorship established in 1957 to honor the former chairman of the Department of Urology. He is also attending physician and Director of Urologic Oncology at NewYork-Presbyterian/Columbia.

Radical Prostatectomy Survival Rates

Men who undergo radical prostatectomy have a high survival rate and low rates of cancer recurrence, cancer spread and death, according to a study of 10,332 men who had surgery between 1987 and 2004. The research showed that between 5 and 20 years after having the surgery, only 3% of the patients died of prostate cancer, 5% saw their cancer spread to other organs, and 6% had a localised recurrence.

Another large Scandinavian study compared men who chose active surveillance with those treated by radical prostatectomy. The results suggest that in the long term, younger men with higher-risk tumours who have a radical prostatectomy, have a definite survival advantage.

Results from another study at Johns Hopkins Hospital in Baltimore confirmed that 82% of men undergoing radical prostatectomy were free of recurrence at 15 years. The data from the research also indicated that in those men whose PSA level starts to rise again after surgery, the recurrent prostate cancer spreads in only around one-third of the men. In addition, unless a man had an aggressive grade of prostate cancer, the spreading of the disease would not become life-threatening for several years and would be amenable to treatment.

Surgeon Experience Affects Outcomes Following Prostatectomy

Results from some clinical studies have indicated that surgeons who are experienced in specific surgical procedures may provide their patients with better outcomes than surgeons who are less experienced in the procedure. Researchers from Memorial Sloan-Kettering Cancer Center conducted a clinical trial to evaluate outcomes of prostatectomies and surgeon experience with the procedure.

This study included 7,849 men with early prostate cancer who underwent a prostatectomy between 1987 and 2003 at one of the following four cancer centers: Cleveland Clinic, Wayne State University, Baylor College of Medicine, or Memorial Sloan-Kettering Cancer Center. None of the patients had received chemotherapy, hormone therapy, or radiation therapy prior to surgery. This study included 74 surgeons.

Patients had better outcomes if their prostatectomy was performed by a surgeon experienced in the procedure:

  • At 5 years, progression-free survival was 88% for patients whose surgeons had performed 250 or more prostatectomies.
  • At 5 years, progression-free survival was 79% for patients whose surgeons had done only 10 prostatectomies.
  • The rate of patients experiencing a rise in prostate-specific antigen levels following surgery increased significantly among patients whose surgeons had performed fewer prostatectomies.

WASHINGTON, D.C. – United States Senator Bob Casey says his prostate cancer diagnosis came last month.

Senator Casey and I went to the same high school together. He was three years older than me, but most men should start screening for prostate cancer at age 50, unless there is a family history, and then we usually recommend start screening five or ten years earlier, said Dr. Patrick Colarusso with Tower Health.

The doctor hopes the news reminds others to get checked themselves and encourages loved ones to do the same.

“Prostate cancer is the most common cancer in the the United States, with roughly 250,000 new cases identify each year, said Dr. Colarusso. One of the most common, but also treatable if caught early.”

“Treatment, thankfully, has cured patients, because 95% of the time the cancer is focused primarily in the prostate gland, said Colarusso.

According to the statement by the U.S. senator and son of the late governor, Casey is planning to undergo surgery in the coming months.

“So the primary modalities for cure are either surgery or radiation therapy, and for most men, they have a choice of radiation therapy or surgery, said Dr. Colarusso.

The risk of some side effects caused by a radical prostatectomy appear to be reduced if a patient is treated at a high-volume hospital and by a surgeon who performs a high volume of procedures annually

Recently, investigators from the Health Outcomes Research Group, Memorial Sloan-Kettering Cancer Center and the Applied Research Branch of the National Cancer Institute evaluated data from the Surveillance, Epidemiology, and End Results -Medicare database to evaluate the incidence of postoperative complications and death in relation to the volume of patients treated at a hospital or the volume of procedures performed by a surgeon. Four separate categories were evaluated: postoperative complications, including life-threatening events during the 30 days after surgery involving cardiac, respiratory, vascular, renal or bleeding events, shock or the need for reoperation late urinary complications including bladder obstructions, fistulas, strictures, pelvic abscess or early incontinence that occurred 31 to 365 days after surgery postoperative death and long-term incontinence lasting over one year following surgery.

This data appears to indicate that patients undergoing a radical prostatectomy may have a reduced risk for developing some side effects if treated in a high-volume hospital and by a surgeon who performs a large number of prostatectomies annually.
