Whos Eligible For This Procedure
Permanent implants are relatively low-energy sources, and therefore have limited tissue penetration. A well-done implant treats the prostate and the surrounding few millimeters of adjacent tissue.
The best candidates for this procedure are patients with a cancer within or near the prostate. Patients with prostate cancer thats invading nearby structures like the bladder or rectum arent appropriate for this technique.
What Are The Side Effects
Urinary symptoms are the most common. These include frequent urination and a need to get to the bathroom quickly. Some men have a burning with urination and, in a few cases, an inability to empty the bladder completely.
These symptoms can usually be managed with medicine, and they improve over time. Temporary self-catheterization may be necessary to help drain the bladder.
Urinary incontinence from brachytherapy is rare. The risk may be somewhat increased in patients who have undergone a previous surgical procedure to remove a part of the prostate called a TURP . A doctor can minimize this risk by doing a careful prostate ultrasound before the procedure to determine how much prostate tissue is still present to implant the seeds.
Rectal bleeding occurs in less than 1% of patients. Diarrhea is rare.
The impotence rate at five years after the procedure is about 25% using brachytherapy alone. If hormone therapy is added, the impotence rate rises depending on the duration of the hormonal treatment.
Bowel problems can sometimes happen and include rectal pain, burning pain and diarrhea.

What Type Of Technology Does This Treatment Use
We use real time, 3-dimensional imaging to carefully place each seed at a predetermined location and depth. In this way, we cover the entire gland and the cancer site. The number of seeds implanted into your prostate will depend on your unique anatomy.
We place an ultrasound probe in your rectum to image the prostate. We also use continuous X-ray imaging. Together, these provide a magnified, 3D view of your prostate during the procedure. After the implantationand while you are still anesthetizedwe will check your urethra and bladder and retrieve any seeds found in your bladder.
Who Can Have Permanent Seed Brachytherapy
On its own
Permanent seed brachytherapy on its own may be suitable for men with CPG 1 localised prostate cancer. This is because the radiation from the radioactive seeds doesnt travel very far, so will only treat cancer that is still inside the prostate.
It may also be suitable for some men whose cancer have a CPG 2 or CPG 3 of spreading.
With other treatments
If you have CPG 4 or CPG 5 localised prostate cancer, you may have brachytherapy together with external beam radiotherapy and hormone therapy. This is sometimes called a brachytherapy boost. Having these other treatments at the same time as permanent seed brachytherapy can help make the treatment more effective. But it can also increase the risk of side effects.
Some men with intermediate risk localised prostate cancer or locally advanced prostate cancer may be offered a brachytherapy boost.
When is permanent seed brachytherapy not suitable?
Permanent seed brachytherapy wont be suitable if your cancer has spread to other parts of your body .
It may not be suitable if you have a very large prostate. If you do have a large prostate you may be able to have hormone therapy before treatment to shrink your prostate.
Not all hospitals offer permanent seed brachytherapy. If your hospital doesnt do it, your doctor may refer you to one that does.

Prostate cancer is the most common form of the disease among men. It will strike one in six. And one in 35 will die of it, according to the American Cancer Society.
The latest treatment at the Exeter Hospitals Center for Cancer Care hopes to lower that rate. Gary Proulx, MD, the director of the Radiation Therapy Program at the hospital, says hes seeing great results, thanks to a tiny gold seed, about the size of a grain of rice.
The treatment is known as the fiducial marker and is considered more accurate and effective than other forms. Its a fairly recent procedure being introduced to the Seacoast for the first time, he says.
The seeds are not radioactive. I want to emphasize that, says Proulx. The improve the imaging in treatment for prostate cancer. Youre able to locate the prostate before treatment and therefore adjust treatment so youre more accurate, it reduces side-effects as well.

What Does Treatment Involve
You will be referred to a specialist who treats cancer with radiotherapy, called a clinical oncologist. The treatment itself may be planned and carried out by other specialists including a therapy radiographer, a radiologist, a urologist, a physicist and sometimes a specialist nurse.
Each hospital may do things slightly differently but you will usually have:
- an appointment to check the treatment is suitable for you
- a planning session, to plan the treatment

The planning session and the treatment itself may be on the same day , or on two separate hospital visits .
Before the planning session, let your specialist know if you are taking any medicines, especially medicines that thin your blood such as aspirin, warfarin or clopidogrel. Dont stop taking any medicines without speaking to your doctor, nurse or radiographer.
One-stage procedure
Many hospitals offer treatment in just one visit, where you will have the planning session and the seeds put in at the same time under the same anaesthetic. You may not need to stay in hospital overnight.
You will have had an appointment one or two weeks before your procedure to check that the treatment is suitable for you. At this appointment, youll have had an ultrasound scan to check the size of your prostate and work out how many seeds you need. You wont need an anaesthetic for this scan.

Gold Seeds In Prostate Cancer Treatment
The only other thing I was drinking in the first 6 months after my diagnosis was green tea. I would drink about 4 glasses of green tea daily to go along with the increased water intake. I wasnt putting any other liquid into my body for the first 6 months. This was a big help in starting my road to recovery.
Once I started feeling better then I added organic soy milk to my diet as well. Soy milk isnt much like regular milk but once you get used to it then its not bad at all. To this day these are the only 3 liquids I have in my diet. To recap the 3 liquids I drink today are purified water,green tea,& organic soy milk. I put no other liquids into my body period.
Now, I want to chat a little more about meat & other aspects of a proper diet. As I said we dont need meat to live. I thought cutting or limiting meat in my diet would be to hard to accomplish. Well again my thinking was wrong. Was it easy? No! However, after a couple weeks then things were starting to get easier. I didnt cut all meats out of my diet but I did cut certain meats & eat moderate amounts of all others.
One meat that needs to be completely cut or at least very minimized is red meat . Too much Red meat consumption is not good for prostate health. I was eating a lot of fast food burgers & also red meat at home. I will say to at least cut red meat completely out of your diet until you get your prostate health back.
What Is The Radioactive Seed Implant Procedure
Permanent radioactive seed implants are a form of radiation therapy for prostate cancer. The terms “brachytherapy” or “internal radiation therapy” might also be used to describe this procedure.
During the procedure, radioactive seeds are implanted into the prostate gland using ultrasound guidance. The number of seeds and where theyre placed is determined by a computer-generated treatment plan tailored for each patient. About 100 seeds are commonly implanted.
The implants remain in place permanently, and become biologically inert after about 10 months. This technique allows a high dose of radiation to be delivered to the prostate with limited damage to surrounding tissues.
Radioactive seed implants are an outpatient procedure. Compared to external radiation which requires up to seven to seven-and-a-half weeks of daily treatments convenience is a major advantage of this treatment option.
