Researchers Say Men Show Improvement in Sexual Function 2 Years After Prostatectomy
June 1, 2010 — If your sexual function’s not what it used to be a year or two after prostate surgery, hang in there.
Men who are having trouble achieving erections after prostatectomy for prostate cancer can achieve improvement in function that’s sufficient for sexual intercourse more than two years later, researchers say.
“The message to patients who have erectile dysfunction — even those who have failed to have erections after surgery — is that improvement does occur in a substantial number of men,” says researcher Jeffrey Schiff, MD, a resident in urology at the State University of New York Downstate Medical Center in Brooklyn, New York.
Overall, one-third of men with marginal erectile function and one-fourth of men with serious erectile dysfunction 24 months after surgery continue to have improvement in erectile function on follow-up visits, he tells WebMD.
The findings were reported here at the 105th annual meeting of the American Urological Association .

How Soon After Prostate Cancer Treatment Can You Be Sexually Active Again

Impotence is simply described as the inability to achieve or maintain an erection suitable for sexual intercourse. Impotence affects 20 million American men or about 10% of the entire male population and 35% of the men over the age of 60. An erection is created when the penis fills with blood much like a tire fills with air. Blood is pumped into the penis and not allowed out, and the more blood that is pumped in, the firmer the erection. The reasons for lack of erections are either the lack of appropriate filling of the penis or inappropriate emptying.
For men who have had radical prostatectomy for prostate cancer, the nerves that control the flow of blood into and out of the penis may be cut or removed. These nerves do not control the sensation to the penis, nor are they responsible for the orgasm that occurs with sexual intercourse. Only the firmness of the penis is affected.
If nerve sparing was attempted during your procedure, the return of erections could take as long as 6 to 18 months. Partial erections may return earlier and may be a sign of subsequent complete return of function, although not a guarantee. Partial recovery is actually quite common and lends itself to some other options for treatment.

If you have prostate cancer or an enlarged prostate, you might undergo prostate surgery also called a prostatectomy. Like all surgeries, this one comes with potential risks. The Prostate Cancer Foundation reports that erectile dysfunction is the most common side effect of a prostatectomy. Fortunately, there are ways to restore sexual function during prostate surgery recovery.
But first, learn about why you might need a prostatectomy, what happens during the surgery, and what to expect during the recovery process.
Why Does Prostate Surgery Cause Erectile Dysfunction
An erection is caused by the brain sending sexual arousal signals to nerves in the penis which stimulate muscles to relax. This allows blood to flow to tissue in the penis, which causes it to become erect.
Erectile dysfunction refers to a condition that occurs when a man cant achieve an erection or maintain an erection for sexual intercourse. Many factors can contribute to erectile dysfunction including problems with the nervous system, blood vessels and hormones.
Prostate cancer is commonly a slow-growing cancer. Doctors will recommend surgery if they believe that the cancer is contained in the prostate gland and hasnt invaded the surrounding areas. One surgery used to remove the prostate gland is called a radical prostatectomy. Your surgeon will remove your prostate gland, the surrounding tissues and the tubes that carry semen.
There are two small clusters of nerves which control erections that are located on both sides of the prostate. These nerves may become injured during the operation. Your surgeon may also purposefully remove these nerves if theres a chance that the cancer has invaded them. If these set of nerves are removed, then you wont be able to become erect without being assisted by medical devices.
There is also a type of prostate surgery called nerve sparing prostatectomy, where both bundle of nerves around the prostate are avoided. However, not everyone is suitable for this surgery, and eligibility also depends on the size and location of the cancer.
